昆明市儿童感染肺炎支原体流行病学调查分析

摘要: [摘要]目的 调查小儿肺炎支原体感染(mycoplasma pneumonia,MP)在昆明的发病率、性别、发病季节及发病年龄分布的情况.方法 对2009年1月至2013年12云南省第一人民医院儿科门诊及住院部疑似肺炎支原体感染的呼吸道感染儿童11 473例做血清肺炎支原体抗体检测.结果 近5 a云南省第一人民医院儿科共有11 473例呼吸道感染患儿行支原体抗体检测,MP阳性数为2 952例,阳性率为25.73%,男性患儿阳性率21.89%,女性患儿阳性率30.87%,男性患儿低于女性患儿,差异有统计学意义(P<0.001);抗体阳性率最高季节,在冬季阳性率为28.41%,春季其次为28.24%,秋季最低为23.07%;0~1岁年龄阳性率最低为9.86%,其次为1~3岁年龄段和4~6岁年龄阳性率,>6岁年龄段阳性率最高为30.43%,趋势χ2检验χ2 = 178.208,P<0.001,可见儿童阳性率随着年龄增长而增长;年间比较,2011年MP阳性率最高为33.75%,与其他年间比较差异均有统计学意义(P<0.05).结论 2011年昆明地区曾有MP感染流行,儿童阳性率随着年龄增长而增长,感染的高峰期春季和冬季,临床上应加强对肺炎支原体肺炎的检测和防治.

Abstract: [Abstract]Objective To investigate the incidence , gender, season and age distribution of Mycoplasma pneumoniae(MP)infection in children in Kunming. Methods From January 2009 to December 2013,11,473 cases of respiratory infections with suspected Mycoplasma pneumoniae infection in our hospital were given serum test of the antibody of Mycoplasma pneumoniae.Results In recent five years, a total of 11,473 cases of respiratory tract infections in children in our hospital received MP antibody detection,the positive case was 2952 cases,the positive rate was 25.73%. The positive rate in male children and female children was 21.89% and 30.87%, respectively, the difference was statistically significant( χ2=118.294,P<0.001). The highest antibody positive rate(28.41%)was found in the winter season,followed by in spring(28.24%) and autumn(23.07%). The lowest positive rate(9.86%)was in 0-1year group,followed by 1-3 years group and 4-6 years group, the highest positive rate(30.43%)was in > 6 years group, the trend chi-square test χ2=178.208,P<0.001, showing that the positive rate was growing with the age. The highest positive rate(33.75%)was found in 2011, compare with other years, the differences were statistically significant. Conclusion There was MP infection prevalence in Kunming in 2011. The positive rate in children is growing with the age,the peak of infection is in spring and winter, so the detection and prevention of mycoplasma pneumonia should be strengthened.
