子宫发育异常诊断的多种方法对比

摘要: [摘要] 目的 探讨多种方法联合应用提高子宫发育异常分型诊断并进行对比分析.方法 对临床有原发不孕、流产、早产、胎位异常等病史,阴道二维超声发现可疑先天性子宫发育异常的65例的患者进行三维超声,子宫输卵管碘油造影检查,并与宫腔镜,腹腔镜检查及手术结果对比.结果 65例患者中纵隔子宫45例,弓形子宫8例,单角子宫/残角子宫5例,双子宫4例,双角子宫2例,弓状子宫8例,正常宫腔1例.阴道三维成像技术与宫腔镜或腹腔镜手术结果基本一致;而子宫输卵管碘油造影术诊断子宫发育异常的类型有其局限性,易将纵隔子宫误诊为单角子宫,样本中单角子宫8例,双角子宫1例差异有统计学意义(P<0.05);经阴道二维超声,检查子宫畸形诊断符合率均较低,差异有统计学意义(P<0.05).结论 经阴道三维成像技术诊断明显优于经阴道二维超声、子宫输卵管碘油造影检查;超声因其简便、无创,可以作为首选的方法, 多种方法联合应用可提高子宫发育异常诊断正确率.

Abstract: [Abstract]Objective To explore a method to efficiently improve the diagnosis level of uterine dysplasia through a combination of a variety of methods, and compare its effect with other methods. Methods Three-dimensional ultrasonography and hysterosalpingography was performed in 65 patients with medical history of sterility,abortion,colporrhagia, premature birth and diagnosed with congenital Uterine dysplasia through transvaginal two-dimensional ultrasonography. Hysteroscope and laparoscope were also used in these patients with the comparison of their operative results. Results In 65 cases of patients with uterine dysplasia,there were 45 cases of mediastinal uterus,8 cases of arcuate uterus,5 cases of single Angle \ residual Angle uterus,4 cases of double uterus,2 cases of double Angle uterus, 8 cases of arcuate uterine, and 1 case of normal uterine cavity. Vaginal 3D imaging and hysteroscopy or laparoscopic surgery results were basically identical. Angiography in the diagnosis of uterine and uterine tubal iodine oil dysplastic type had limitations,mediastinum uterus was often misdiagnosed as single Angle's uterus,there were 8 cases of sample single Angle uterus and 1 case of double Angle uterus,difference was statistically significant (P < 0.05). The diagnosis coincidence rate of transvaginal two-dimensional ultrasound for uterine dysplasia was lower, the difference was statistically significant(P < 0.05). Conclusions The transvaginal three-dimensional ultrasonography is superior to transvaginal two-dimensional ultrasonography and hysterosalpingography technology in diagnosis of uterine malformation. And ultrasonic diagnosis could be regarded as the first choice because of its low cost and noninvasion, the combined application of various methods will improve the accuracy of diagnosis on uterine dysplasia.
