细菌性呼吸机相关性肺炎抗生素使用时间的多因素分析

摘要: [摘要] 目的 了解细菌性呼吸机相关性肺炎抗生素使用时间的影响因素.方法 选取在2011年1月至2014年12月在成都市第五人民医院CIU 进行治疗的细菌性呼吸机相关性肺炎患者,调查可能影响抗生素使用时间的人口学特征、入院时APECHII评分、治疗方案、感控措施落实状况等.结果 出多重耐药菌、痰培养阳性、AEPCHII评分增高、气管是切开会延长抗生素使用时间,大蒜素联用抗生素会缩短使用抗生素的时间.气管切开对抗生素使用的时间影响最大,APECHII评分最小.结论 选用合理的抗生素,对患者积极进行使用大蒜素联用抗生素治疗,改善患者的危重程度有利于细菌性呼吸机肺炎的治疗.Abstract: [Abstract]Objective To learn the influencing factors of the time of using antibiotics for treating bacterial ventilator associated pneumonia.Methods We selected the patients with bacterial ventilator associated pneumoniain who admitted in ICU from January 2011 to December 2014 . Then we investigated the possible factors influencing the time of using antibiotics,such as demographic characteristics,APECHII score,the plan of treatment , the situation of Prevention and control,etc. Results The appearance of multiple drug-resistant bacteria, positive sputum culture,higher score of APECHII and tracheotomy extended the time of using antibiotics, while using the antibiotics with allicin shortened the time of using antibiotics. Tracheotomy was the most influencing factors of the time of using the antibiotics,while APECHII score was the less influencing factors. Conclusion Rational use of antibiotics and combined antibiotics with allicin is beneficial to treat the bacterial ventilator associated pneumonia, and improve the level of critically patients.
