摘要: ［摘要］目的　观察防旋型股骨近端髓内钉与动力螺旋钉固定对骨质疏松性粗隆间骨折的临床疗效．方法　选择2012年1月至2013年10月屯昌县人民医院收治的骨质疏松性骨折患者80例为研究对象，遵循随机对照原则分为对照组和治疗组，每组各40例，2组同时给予在抗骨质疏松治疗，对照组给予动力髋螺钉（DHS）固定，治疗组予以防旋型股骨近端髓内钉固定（PFNA）治疗，比较2组患者的手术一般情况如手术时间、手术切口长短、术中出血量，对患者平均住院天数进行观察和记录，并且评价患者髋关节功能恢复情况．结果　治疗组手术时间缩短（1.3±0.75） h、切口较短（5.11±0.21）cm、术中出血量减少（244.53±130.34）mL、平均住院天数减少（12.43±2.92）d、而且髋关节功能恢复较好（Harris评分），2组之间的差异均具有统计学意义，P＜0.05．结论　在抗骨质疏松治疗的基础上，防旋型股骨近端髓内钉是目前治疗骨质疏松性股骨粗隆间骨折较好的方法，且手术创伤小恢复时间快，可作为骨质疏松性粗隆间骨折的首选．

Abstract: ［Abstract］Objective　To observe the difference between proximal femoral nail anti-rotation （PFNA） Internal Fixation and sliding dynamic hip screw（DHS）in the treatment of osteoporotic Intertrochanteric Fractures. Methods　The data of 80 patients with osteoporotic intertrochanteric fracture in our department （from November 2012 to November 2013） were retrospectively analyzed. By using the random parallel method，we divided 80 clinic patients into two groups: the treatment group（PFNA group）and the control group. Patients in the two groups were given anti-osteoporosis treatment， patients in the control group were given DHS internal fixation， patients in the PFNA group were given PFNA internal fixation. Then we compared the general surgery conditions， postoperative Harris scores and the clinical efficacy between the two groups．Results　Compared with the control group， the PFNA group was better in the operation time，the length of incision， amount of bleeding during operation， average hospital day and Harris grade. The differences were statistically significant（P＜0.05）. Conclusion　On the basis of anti-osteoporosis tveatment，PFNA internal fixation can thus be the first choice in the femoral intertrochanteric fracture operation.
