内镜辅助下治疗牙源性上颌窦病变
Endoscopy Assisted Treatment for Odontogenic Maxillary Sinus Disease

摘要: 目的 探讨内镜辅助下的微创方法治疗牙源性上颌窦病变的临床价值.方法 对2010年3月至2016年4月在云南省第二人民医院口腔外科手术治疗的43例牙源性上颌窦病变患者,按就诊顺序随机分为A、B组.A组(22例),B组(21例).A组采用内镜辅助下的微创手术治疗;B组采用传统的开放方式;比较2组患者的手术时间,术中出血量.术后面部肿胀的消退时间、面部麻木情况及住院费用等各项指标.结果 2组患者均顺利完成手术,内镜辅助组与传统开放组比较,手术时间短(P<0.05),出血量少(P<0.05),术后面部的肿胀消退时间短(P<0.05),面部麻木的发生率低(P<0.05);住院费用低(P<0.05).结论 内镜辅助下手术治疗牙源性上颌窦病变较传统开放手术,前者具有手术时间短,术中失血少,术后恢复快,并发症发生率低,住院费用低等优势,值得临床推广.Abstract: Objective To explore the clinical value of endoscopic minimally invasive treatment for odontogenic maxillary sinus diseases. Methods From March 2010 to April 2016, 43 patients with odontogenic maxillary sinus diseases were randomly divided into A and B groups according to the order of treatment. Group A(n= 22) and group B(n = 21). Group A was treated with endoscopic minimally invasive surgery. Group B was treated with conventional open procedure. The operation time and intra-operative blood loss were compared between the two groups. Postoperative facial swelling subsided time, facial numbness and hospital costs, postoperative satisfaction and other indicators were also compared. Results Compared with Group B, the operation time was shorter(P<0.05), bleeding volume was less(P<0.05), and the time of facial swelling was shorter after operation(P<0.05,) the hospitalization cost was lower(P<0.05), the postoperative satisfaction was higher(P<0.05) in Group A(P<0.05). Conclusions Endoscopic surgery is more effective than traditional open surgery in the treatment of odontogenic maxillary sinus surgery. The former has the advantages of shorter operative time, less blood loss, rapider postoperative recovery, lower incidence of complications and lower hospitalization cost.
