基于快速康复外科理念的护理干预对人工跖趾关节置换术患者术后恢复的影响
Effect of Nursing Intervention Based on Fast Track Surgery Concept on Postoperative Recovery of Patients with Plantar Toe Joint Arthroplasty

摘要: 目的 探讨基于快速康复外科 (FTS) 理念的护理干预对人工跖趾关节置换术 (PTJA) 患者术后恢复的影响.方法 选取2013年12月至2016年12月上海市光华中西医结合医院确诊治疗的PTJA患者80例, 依据干预方法分为常规组和快速组, 每组40例, 常规组给予常规护理干预, 快速组在此基础上给予基于FTS理念的护理干预, 统计分析所有患者切口愈合时间、疼痛缓解时间、住院时间、并发症和干预前后疼痛视觉模拟评分法 (VAS) 、美国矫形外科足踝协会踝-后足评分系统 (AOFAS) 得分情况.结果 快速组切口愈合时间、疼痛缓解时间、住院时间、并发症发生率明显低于常规组, 差异有统计学意义 (P<0.05) ;快速组干预后VAS得分明显低于常规组, AOFAS得分明显高于常规组, 快速组干预前后VAS、AOFAS得分变化值明显高于常规组, 差异有统计学意义 (P<0.05) .结论 基于FTS理念的护理干预可有效减少PTJA患者并发症发生, 有利于患者术后身体及足功能恢复, 值得临床进一步推广.Abstract: Objective To discuss the effect of nursing intervention based on fast track surgery (FTS) concept on the postoperative recovery of patients with plantar toe joint arthroplasty (PTJA) .Methods Eighty patients with PTJA were selected from December 2013 to December 2016 in our Hospital, according to the intervention method, all patients were divided into normal group and fast group, 40 cases in each group.Patients in the normal group were given routine nursing intervention, patients in the fast group were additionally given nursing intervention based on FTS concept.Then we statistically analyzed the incision healing time, pain relief time, hospitalization time, complication and the before and after intervention pain visual analogue scale (VAS) , the American association of orthopaedic surgery ankle foot after ankle-scoring system (AOFAS) scores of all patients.Results Theincision healing time, pain relief time, hospitalization time, complication rate of fast group were significantly lower than those of normal group, the difference was statistically significant (P<0.05) .The VAS score after intervention of fast group was significantly lower than that of normal group, the AOFAS score was significantly higher than that of normal group, the before and after intervention VAS, AOFAS score change value of fast group were significantly higher than that of normal group, the difference was statistically significant (P<0.05) .Conclus ions Nursing intervention based on fast track surgery concept can effectively reduce the complications in patients with PTJA, it is beneficial to patients with postoperative body and feet functional recovery, it is worth for further clinical promotion.
