Early predictive value of lymphocyte subsets for cognitive dysfunction in cerebral ischemia-reperfusion injury

目的 探讨淋巴细胞亚群对脑缺血再灌注损伤认知功能障碍的早期预测价值。 方法 将68例脑缺血患者进行研究,分为观察组(n = 30)例和对照组(n = 38)例。比较一般情况、淋巴细胞亚群检测结果及蒙特利尔认知评估(Montreal Cognitive Assessment,MoCA)评分差异,分析其相关性、灵敏度、特异度。 结果 观察组既往糖尿病史占比高于对照组,CD3+、CD8+及MoCA评分均低于对照组(P < 0.05),CD4+、CD4+/CD8+、NK、CD19+均高于对照组(P < 0.05);CD3+、CD8+与MoCA评分呈正相关(P < 0.05);CD4+、CD4+/CD8+、NK、CD19+与MoCA评分呈负相关(P < 0.05);淋巴细胞亚群联合检测对脑缺血再灌注损伤认知功能障碍诊断的灵敏度为93.45%,特异度100.00%。 结论 淋巴细胞亚群检测对脑缺血再灌注损伤认知功能障碍的发生有较好的预测价值,可作为特异性标志物。 Abstract:Objective Exploring the early predictive value of lymphocyte subsets for cognitive impairment in cerebral ischemia-reperfusion injury. Methods 68 patients with cerebral ischemia were divided into observation group (n = 30)cases and control group (n = 38)cases.The general situation, lymphocyte subsets detection results and Montreal Cognitive Assessment Scale (MoCA) score were compared. Their correlation, sensitivity, specificity were analyzed. Results The proportion of patients with diabetes and levels of CD3+, CD8+ and MoCA score in the observation group were lower than those in the control group (P < 0.05), the levels of CD4+, CD4+/CD8+, NK and CD19+ in the observation group were higher than those in the control group (P < 0.05); CD3+ and CD8+ were positively correlated with MoCA score (P < 0.05); CD4+, CD4+/CD8+, NK and CD19+ were negatively correlated with MoCA score (P < 0.05); The sensitivity and specificity of lymphocyte subsets combined detection for the diagnosis of cognitive dysfunction in cerebral ischemia-reperfusion injury were 93.45% and 100.00% respectively. Conclusion The lymphocyte subsets have good predictive value for the occurrence of cognitive dysfunction in cerebral ischemia-reperfusion injury. It can be used as a specific marker. -
