Analysis of the Influence of Maternal Systemic Factors on Endometrial Implantation Window in Women with Repeated Implantation Failure

  • 摘要:   目的  分析接受辅助生殖治疗中反复移植失败人群影响子宫内膜种植窗的母体全身因素。  方法  回顾性分析2020年12月至2022年1月在云南省第一人民医院生殖医学科诊断为反复种植失败并行子宫内膜容受性检测(endometrial receptivity test,ERT )病例的资料。根据ERT结果分为容受期组和非容受期组,比较2组间胰岛素抵抗、维生素D缺乏、血栓前状态、甲状腺自身免疫发生率以及抗核抗体、抗磷脂抗体阳性率。并将上述单因素分析结果进行Logistic回归分析,得出影响内膜种植窗的独立危险因素。  结果  2组患者一般资料的差异无统计学意义(P > 0.05),而2组患者的胰岛素抵抗发生率、抗磷脂抗体阳性率的差异均具有统计学意义(P < 0.05)。多因素Logistic回归分析显示胰岛素抵抗、血栓前状态、甲状腺自身免疫是子宫内膜种植窗发生的独立危险因素(P均 < 0.05)。  结论  母体全身因素中胰岛素抵抗、血栓前状态、甲状腺自身免疫是造成反复种植失败人群内膜种植窗移位的危险因素。
