Abstract:
[Abstract]Objective To explore the hepatitis B virus surface antigen(HBsAg)screening and infection of hepatitis B virus(HBV)among puerperants giving birth to children in hospitals in different areas of Yunnan. Methods We selected three cities by quota sampling.Data about HBsAg screening and infection of HBV among puerperants were reported to local hospitals from January to June of 2011,the data in the three cities were collected and analyzed. Results The gold immunochromatography assay(GICA)was used by most hospitals for HBsAg screening ,a total of 68753 puerperants were reported by the 332 hospitals and the HBsAg screening rate was 97.44 %.The consistent rate of screening was 98.83 %. The positive rate in those who had screened for HBsAg was 2.35% and the positive rate among three areas showed significant difference(P<0.01).The constituent ratio of both HBsAg-and HBeAg -positive puerperants in villages was higher than that in cities(P<0.05).The constituent ratio of both HBsAg-and HBeAg-positive puerperants in 15~29 year-old group was higher than that in 30~42 year-old group(P<0.01).Conclusions The antepartum HBsAg screening rate is high among those who gave birth to children in hospitals.GICA is mainly used to do HBsAg screening in health centers in different area of Yunnan during the project.Although screening results are credible, the undetected rate exists. So HBsAg screening and retesting with sensitive, stable and mature ELISA method is a better choice

Abstract(3224) PDF(321)
Abstract:
[Abstract]Objective To explore the relationship between the changes of plasma N-terminal pro-B-type natriuretic peptide(NT-proBNP)and left ventricular remodeling(LVRM)in early acute myocardial infarction(AMI)patients before and after PCI.Methods Seventy-two patients admitted to our hospital with AMI were randomly divided into two groups,35 were in medicine treated group(M-group)and another 37 were in the medicine plus emergent PCI group(PCI-group).The plasma NT-proBNP was measured within 24 hours after admission or before PCI and on the 10 th day after admission, and echocardiogram were performed for all of the patients at admission and on the 9th or 10th day after admission to evaluate the left ventricular remolding by measuring the left ventricular end-diastolic dimension (LVEDD), left ventricular ejection fraction(LVEF),left ventricular shortening(FS)and left ventricular end-diastolic volume(LVEDV)to analyze the relationship between the NT-proBNP and the morphology and function of the left ventricle. Results The levels of plasma NT-proBNP decreased,LVEF and FS increased after the treatment in both of the two groups than that before treatment significantly(P<0.05),and no significant difference were observed for LVEDD and LVEDV(P>0.05).No significant difference was observed for the plasma NT-proBNP,LVEF,FS,LVEDD,LVEDV between the two groups before the treatment(P>0.05).The levels of plasma NT-proBNP decreased and LVEF, FS increased significantly after the different treatment in PCI-group than that of M-group(P<0.05),but LVEDD,LVEDV decreased insignificantly(P>0.05).Conclusions The increased plasma NT-proBNP in AMI patients may reflect the left ventricular remodeling.PCI may decrease significantly the plasma NT-proBNP,imorove the left ventricular function and reduce the incidence of left ventricular remodeling

Abstract:
[Abstract]Objective To compare the effects of etomidate and propofol anesthesia induction on cardiovascular stability in elderly patients, and to guide drugs use in the anesthesia induction in elderly patients. Methods 60 elderly patients were randomly divided into two groups:acupuncture group etomidate(A),propofol needle group(B),induced by 0.2 mg/kg of midazolam maleate tallet.Patients in A group were given 0.3 mg/kg of etomidate, and patients in B group were given 1.5 mg/kg of propofol-pin. All patients were also intravenously injected 3 μg/kg of Norcuron and 0.1 mg/kg of Fentanyl.HR,SBP,DBP and MAP of patients were recorded before induction,1 min,2 min and 5 min after induction. The adverse reactions including nausea, vomiting, intravenous pain, involuntary muscle activity and allergic reactions were also observed. Results Compared with baseline values before induction of anesthesia,HR,SBP,DBP and MAP of patients in both groups were decreased in different degrees after induction of anesthesia. The decrease was statistically significant at 1 min and 5 min after induction of anesthesia(P<0.5).Compared with B group,The decrease of SBP and MAP was more obvious in A group at 2 mins and 5 mins after induction of anesthesia(P<0.5). There were no nausea, vomiting and allergic reactions in two groups. There was intravenous injection pain in B group and involuntary muscle activity in A group, there was a significant difference(P <0.05).Conclusion Both etomidate and propofol inhibit the cardiovascular stability in elderly patients in different degrees, but etomidate has less effect on the circulation of patients and vascular stability,so etomidate is more suitable as the preferred intravenous anesthetic induction drug for elderly patients
