Objective To compare the sustained release characteristics of paclitaxel chitosan membranes prepared by two different methods.Methods We prepared PTX-Suc-HECTS sustained release film and P-PTX-SRM release film by different methods.Then we measured the PTX loaded in chitosan membrane with FTIR, measured the best of factors for equilibrium swelling degree in the two sustained release membranes with multifactors orthogonal experiment, measured the standard curve of PTX, measured the two sustained release membrane in vitro PTX release curve.According to the standard curve, we also measured and compared the drug rate between two kinds of sustained-release film release in vitro.Results PTX was loaded in the chitosan sustained release film.The best of equilibrium swelling factors for PTX-Suc-HECTS sustained-release membrane were 250 mg Suc-HECTS, 1 mg/m L EDC-HCl and DMAP aqueous solution, 1h the cross-linking time, 1∶100cross-linking ratio.The best of equilibrium swelling factors for P-PTX-SRM sustained release membrane were 1%dilute acetic acid, 1h crosslinking time, 1 ∶100 crosslinking ratio; the two sustained release membranes had bursting diaphragm part and sustained release part in vitro, but burst rate of PTX-Suc-HECTS was lower than P-PTX-SRM (P<0.05) , and there was no statistical difference between the release parts, (P>0.05) .The higher crosslinking ratio, the less drug release in burst release stage in 48 h, and more drug release in sustained release stage in unit time.Conclusion PTX-Suc-HECTS sustained release membrane and P-PTX-SRM sustained release membrane have good sustained-release characteristics, P-PTX-SRM has the advantages of simple preparation process, short preparation time and saving experiment time, so it can be a preferred method to prepare the chitosan membrane.
Objective To investigate the value of Loop-mediated Amplification (LAMP) in the gene diagnosis of common pathogens in lower respiratory infection.Methods One hundred and twenty-two cases of lower respiratory tract secretions and bronchoalveolar lavage fluid were collected from patients with suspected lower respiratory infection.The pathogens were tested by using LAMP, isolation and cultivation of microbial pathogens, real-time FQ-PCR.The results were compared with different specimen types, age and detection methods.Statistical analysis was performed using the SPSS software and Kappa test.Results In 122 cases of specimensfrom patients with respiratory infection, the positive rate of sputum was higher (55.56%) than that in bronchoalveolar lavage fluid (46.88%) .The main pathogens were Methicillin-resistant Staphylococcus (MASA) and mycoplasma pneumoniae (MP) (P<0.05) respectively.In different age stage, MASA was mainly detected in the elder group (>60 years) , and MP was the main pathogen in the younger patients (≤20 years) (P<0.01) .The detection rate of LAMP (50.00%) was higher than the microbial Isolation and cultivation (36.67%) .The detection rates of Klebsiella pneumoniae (Kpn) and Bauman Acinetobacter (CSAB) by two different methods were consistent (kappa>0.4) , however the consistencies in other pathogens were low (kappa<0.4) .And there was a high consistency between LAMP and real-time FQ-PCR in the detection of MP (kappa >0.7) .Conclusions LAMP has the advantages of rapidity, sensitivity, high effciency and specificity, high throughput and wide coverage of pathogens, which may be a quick and reliable laboratory diagnosis method.And using different respiratory specimens or methods may have different results.They have their own advantages and disadvantages, which are needed to be paid close attention by clinicians.
Objective To explore the regulation of CYP3A4 gene expression by CYP3A4 intron 10 and CYP3A4*1G.Methods CYP3A4 eukaryotic expression vector which promoted by CYP3A4 promoter was constructed.The forward and reverse full-length segments of CYP3A4 intron 10 (with CYP3A4*1G allele wild/mutant type) were ligated into the recombinant eukaryotic expression vector above at upstream of CYP3A4 promoter to construct CYP3A4 eukaryotic expression vector which promoted by CYP3A4 promoter and regulated by CYP3A4 intron 10 with different genotypes and directions.The vectors were transfected into Hep G2 respectively.The levels of CYP3A4 m RNA were detected by quantitative real-time PC.Results These insertion sequences and their directions were exactly correct.The expression level of CYP3A4 m RNA was higher when CYP3A4 intron 10 was inthe forward than in the reverse direction (P<0.01) ;the wild group showed higher CYP3A4 m RNA level than the mutant group (P<0.01) ;while the forward mutant group showed lower CYP3A4 m RNA level than the positive control group.Conclusion CYP3A4 intron 10 is like an enhancer functionally, which could influence the transcription of CYP3A4 promoter depending on its direction and regulation by CYP3A4*1G allele.
Objective To investigate the anti-inflammatory and analgesic activity and acute toxicity of Bulleyaconitine A and Foresaconitine and Yunaconitine from Aconitum bulleyanum Diels.Methods After intragastric administration, the anti-inflammatory and analgesic activities of Bulleyaconitine A and Foresaconitine and Yunaconitine using Acetic acid writhing test and Dimethylbenzene auricle swelling test in mice were compared.The acute toxicities of three iterpenoidalkaloids were also compared according to LD50.Results (1) The LD50 values of Bulleyaconitine A and Foresaconitine and Yunaconitine were:4.06 mg/kg, 2.81 mg/kg and 12 mg/kg; (2) Single intragastric administration of three kinds of alkaloids all had obvious inhibitory effect on acetic acid-induced writhing pain.The 2 mg/kg group of Bulleyaconitine A had the strongest effect, the inhibition rate was higher than the same dose of Foresaconitine and Yunaconitine, there was significant difference compared with Foresaconitine group (P<0.05) ; (3) Bulleyaconitine A and Yunaconitine had obvious inhibiting effect on auricle swelling induced by dimethylbenzene, there were significant differences compared with the vehicle group (P <0.05) , Foresaconitine with the same dose had no obvious effect; 0.4 mg/kg Yunaconitine had the strongest anti-inflammatory effects, there were significant differences compared with the same dose of Bulleyaconitine A and Foresaconitine groups ( P <0.01) .Conclusion ( 1) Comparison of acute toxicity: Yunaconitine >Bulleyaconitine A >Foresaconitine; (2) Three alkaloids all have analgesic effects in different degree, Bulleyaconitine A is the strongest; (3) Yunaconitine is better than Bulleyaconitine A and Foresaconitine in anti-inflammatory effect with same concentration.
Objective To investigate the significance of pretreatment peripheral blood HPV16E6m RNA and CK19m RNA in diagnosis and their correlations to the clinicopathologic features of cervical carcinoma.Methods One hundred cervical carcinoma patients underwent pretreatment peripheral blood HPV16E6m RNA and CK19m RNA evaluation;40 healthy women were subjected as control.The specificity and sensitivity of HPV16E6m RNA and CK19m RNA as diagnostic indexes were analyzed; their correlations to clinicopathologic features were investigated.Results The specificity of HPV16E6m RNA and CK19m RNA in the diagnosis of cervical cancer were 97.5%.The sensitivity of HPV16E6m RNA and CK19m RNA in the diagnosis of pre treatment were 15% and39%, respectively and the difference was statistically significant (P <0.05) .HPV16E6m RNA elevation was associated with clinical stage (P= 0.012) and pelvic lymph node metastasis (P=0.026) . CK19m RNA was associated with deep muscular invasion (P=0.031) and pelvic lymph node metastasis (P=0.027) .Conclusion Peripheral blood HPV16E6m RNA and CK19m RNA are the valuable tumor markers of cervical cancer.The value of CK19m RNA in the diagnosis of cervical cancer is better than that of HPV16E6m RNA.
Objective To reserch the expression of Beclin 1 and LC3 in malignant fibrous histiocytoma.Methods The sp immunohistochemical method was taken in 21 cases with Malignant fibrous histiocytoma, then the expression of Beclin 1, LC3 was detected.Results The expression levels of Beclin 1 and LC3 were significantly high in all cases.Conclusion Beclin 1 and LC3 might play a role in the occurrence and development of malignant fibrous histiocytoma.
Objective To investigate the situation of job satisfaction for hospital staffs in one pubic hospital of Yunnan province, analyze the influencing factors which affect the job satisfaction, and provide scientific basis for improving job satisfaction among hospital staff.Methods Self-designed questionnaire was adopted in on-site simple random sampling survey among hospital staffs in one public hospital in Yunnan province.Results The hospital staffs expressed a higher satisfaction to their jobs, however, the hospital staffs in different characters showed different satisfaction to their jobs.The factor affecting the satisfaction for the job among medical staff included awareness of hospital management system and performance appraisal system, changes of income and confidence in hospital development.Conclusion For further enhancing the satisfaction to the reform among hospital staff, more attention should be paid to establish and improve staff's motivation system, and make rational hospital management, income distribution and performance appraisal system.
Objective To improve our understanding of brucellosis by analyzing its epidemiological and clinical features, laboratory examination of brucellosis in Yunnan Province.Methods Clinical and laboratory data of brucellosis patients in Qujing First People's Hospital from 2013 to 2014 were retrospectively analyzed with its relevant literature reviewed.Results A clear epidemiological history was found in all the 8 brucellosis patients, and they were manifested as fever, sweat, weakness, hepatosplenomegaly and lymphadenopathy.They got to their first appointment in all kinds of clinical departments, such as Urology, Endocrinology, respiration, infectious diseases, oncology, Hematology, neurology and other clinical departments.Among the other accompanied various symptoms and signs of the patients included fever and muscle ache, fever of unknown origin, fever with white blood cells reduction, right abdominal pain, fever accompanied cough and sputum, discontinuity headache and no obvious inducement of left testis gall pill.Cultured venous blood from 8 patients was positive for brucella.Blood white blood cell count and liver function are normal.Conclusion Brucellosis shows various clinical manifestations, and the brucellosis patients are first diagnosed at different clinical departments.So, clinical and laboratory physicians should pay more attention to brucellosis combining with the epidemiological characteristics, as early as possible to perform the blood bacterial culture for patient with long-term fever due to unknown reasons.
Objective To study the correlation of micro inflammation state with serum lipid metabolism, calcium and phosphorus metabolism in hemodialysis patients and its clinical significance.Methods We selected31 uremic peritoneal dialysis patients (peritoneal dialysis group:PD group) , 31 continuous hemodialysis patients (hemodialysis group:HD group) , and 18 healthy subjects (control group) .ELISA method was used to detect serum level of high sensitive C reactive protein (hs-CRP) .Automatic biochemical analyzer was used to detect the serum creatinine, urea nitrogen, total cholesterol, high density lipoprotein cholesterol, low density lipoprotein cholesterol, triglyceride, serum calcium, phosphorus, parathyroid hormone, and the results were analyzed.Results Compared with the normal control group, HD group and PD group patients had higher blood hs-CRP, the difference was statistically significant (P<0.05) .Total cholesterol, serum phosphorus, parathyroid hormoneand calcium phosphate product of patients in HD and PD groups were higher than those of the control group, the difference was statistically significant (P <0.05) .There was no statistically significant difference between the normal control group and the HD group or the PD group in low density lipoprotein cholesterol and triglyceride (P>0.05) .PD group of patients had a positive correlation between triglyceride and hs-CRP (r=0.36, P<0.05) , there was no significant correlation between other indexes serum lipid, calcium phosphorus metabolism and hs-CRP (P>0.05) .Conclusions Micro inflammatory state exsists in patients with chronic dialysis, accompanied by abnormal metabolism of serum lipids and calcium and phosphorus metabolism, but hs-CRP is not significantly correlated with lipid, calcium and phosphorus metabolism.
Objective To investigate the expression and significance of serum VEGF (vascular endothelial growth factor) and MMP-9 (matrix metalloprotoinase-9) in eldly patients with advanced NSCLC (non-small cell lung cancer) in hypercoagulative state.Methods Serum VEGF and MMP-9 were detected separately by ELISA (enzyme-linked immunosorbent assay) in 30 specimens of the eldly advanced NSCLC patients with hypercoagulative state and without hypercoagulative state.Results The expression level serum VEGF and MMP-9 was 536±103pg/m L and 474±76 ng/m L in the eldly advanced NSCLC patients with hypercoagulative state;which were higher than that in without hypercoagulative samples (433±74 pg/m L and 362±43 ng/m L) (P<0.05) .The higher expression of serum VEGF in the elder advanced NSCLC patients with hypercoagulative was positively correlated with the higher expression of MMP-9 (r=0.478) P<0.05, and which was correlated with the age, the clinical TNM stage and the number of accompanying disease (P<0.05) .Conclusion VEGF and MMP-9 are highly expressed in the eldly advanced NSCLC patients in hypercoagulative state, further confirming that hypercoagulative states to some extent may promote the invasion and metastasis of elderly patients with advanced lung cancer.
Objective To investigate the effects of goal-directed fluid therapy with different ratios of Lactated Ringer's (LR) / Hydroxyethyl Starch (HES) on coagulation function in hemorrhagic shock dogs.Methods 48 healthy beagle dogs were randomly assigned to six groups (n=8) :sham group, 3∶1 group, 2∶1group, 1∶1 group, 1∶2 and 1∶3 group.Sham group (S group) just received endotracheal incubation.While3∶1 group, 2∶1 group, 1∶1 group, 1∶2 group and 1∶3 group were regarded as treatment groups.These treatment groups were administrated hemorrhagic shock model with bleeding and then resuscitated with goal-directedfluid therapy according to different LR/HES ratios: 3 ∶1, 2 ∶1, 1 ∶1, 1 ∶2 and 1 ∶3.Targeted goal of resuscitation is stroke volume variation (SVV) below 10%, a fluid bolus 2 m L/kg of different ratios was infused within 5 min when SVV exceed 10%.Autologous blood was administrated and the hemoglobin was maintained between 10 to 11 g/d L in treatment groups.The coagulation function test (PT, APTT, TT, and Fib) was measured at baseline (T0) , 1 h after shock (T1) , 2 h after resuscitation (T2) and 4 h after resuscitation (T4) .After 4 hours of fluid resuscitation, the total amounts of fluid (the LR amounts plus the HES amounts) and the urine output were recorded.Results There was no significant statistical difference in coagulation parameters (PT, APTT, TT, and Fib) in all groups at all time points.The HES amounts of the 1∶3 and 1∶2 groups were more than the 3∶1 group;the LR amounts, the total amounts of fluid, and the urine output in the 1∶3 and 1∶2groups were lower than in the 3∶1 group.Conclusion Different LR/HES (3∶1, 2∶1, 1∶1, 1∶2 and 1∶3.) ratios' goal-directed fluid therapy have no effect on the coagulation function of hemorrhagic shock dogs.
Objective To analyze the application of multi-slice spiral CT in evaluation of the pharyngeal and upper airway stenosis in patients with obstructive sleep apnea syndrome (OSAS) .Methods We selected 46 cases of obstructive sleep apnea syndrome as the observation group and 46 healthy subjects as the control group.All enrolled patients and healthy subjects were given neck CT scan and measurement analysis, the differences of the parameters of the upper airway were compared between the two groups.Results The sagittal diameter and transverse diameter of airway in the posterior and posterior lingual regions of OSAHS patients were significantly smaller than those in the control group (P<0.05) , while the thickness of pharyngeal wall and posterior wall were larger than those of the control group (P<0.05) .Soft palate length and thickness were significantly greater than the control group (P<0.01) .Conclusion Multislice spiral CT can evaluate the airway stenosis of patients with obstructive sleep apnea syndrome, so it is helpful for the clinical diagnosis and efficacy evaluation of OSAS.
Objective To investigatethe the effect of taking-off gout stone in different body parts on the level of serum uric acid of gout patients through retrospective anlysis.Methods We selected 72 gout patients admitted to Yunnan TCM hospital to get rid of the joint gout stones by taking-off gout stone operation, met the inclusion criteria and comleted the follow-up.According to the operation location and the part with most gout stones, the enrolled patients were divided into 2 groups:35 cases in knee joint group (35cases) and 37 cases in other-joint group.Before operation, 2 weeks later and 3 weeks after operation, patients in the two groups received the serum uric acid tests.The two-sample T test was used to analyze the s UA difference between two groups before operation and the relationship between s UA level and gout stone location was investigated.The s UA levels of patients in two groups were retested 2 weeks and 8 weeks after operation, and the two-sample T test was used to analyze the short-term and long-term effect of operation on s UA levels, and compare the difference in the s UA level of patients between two groups at 2 weeks and 8 weeks after operation.Results (1) Compared with the other joint group, the s UA of knee joint group at 1 day before operation was relatively higher, and the difference had statisticalsignificance (P<0.05) .There was no significant difference in the s UA levels of patients 2 weeks after operation between two groups (P=0.0534) .Compared with the other joint group, the s UA of knee joint group at 8 weeks after operation was significantly lower, and the difference had statistical significance (P<0.05) . (2) Compared with 1 day before operation, the s UA level of patients in knee joint group at 2 weeks and 8 weeks after operation was significantly lower, and the difference had statistical significance (P<0.01) .There was no significant difference in the s UA levels of patients in other joint group between 2 weeks after operation adn 1 day before operation (P>0.05) .Compared with 1 day before operation, the s UA level of patients in other joint group at 8 weeks after operation was significantly lower, and the difference had statistical significance (P<0.01) .Conclusion For the gout stone in knee joint, taking off and getting rid of them might be necessary in lowering the s UA and protecting knee joint.However, for the gout stone in other joints, early taking off gout stone can't stop the redeposition of urate crystal, so it may be proper to take off the gout stone before the occurrence of irreversible joint injury.
Objective To compare the clinical effects between nasal high flow oxygen therapy and non-invasive ventilation care for post-extubation mild-to-moderrate hypoxemia after abdominal surgery In ICU.Methods Forty-eight patients with post-extubation hypoxemia after abdominal surgery in our ICU were randomly divided into 2 groups: high flow oxygen therapy group and non-invasive ventilation group.Patients in the two groups were treated with HFNV and NIV, respectively, for 1 day.Then we compared the data including: 1.objective respiratory and circulatory parameters (R, PaO2/FIO2, PaCO2;MABP) during at pre-extubation (T0) and post- extubation 30 minuts (T1/2) and the 24 hours (T24) as well as a subjective rating of dyspnea and discomfort by the patients on a number10-points scale of 0 (lowest) to 10 (highest) in the end of 24 h observation period;2. Devices related benign side effects (nose and mouth dryness, skin pressure redness or ulcer, nose or mouth mucosal lesions, thirsty.3. ICU stay and mortality.Results There were no significant differences in the baseline characteristics of the patients between two groups including age, sex, APACHEII score, SOFA score, major etiology and previous illnesses ratio, pre-extubation mechanic ventilation times, P>0.05) ;the PaO2/FIO2 in NIV group at T1/2, T24 were better than HFNV group (222±36.5) , (267±88.9) mm Hg versus (195±23.4) , (224±82.7) mm Hg, P<0.05;T0, T1/2, T24 the circulatory parameters and others respiratory parameters had no significant differences, P >0.05; in contrast, on getting the number10-points scale of 0 (lowest) to 10 (highest) in the final study time interview from the patients, HFNV group (5.7±2.8) was significantly better than NIV group (2.8±1.5) , P<0.05.Devices related side effects in the NIV group (14 cases, 58.3%) was more obvious than the HFNV group (4 cases, 16.7%, P<0.01) ;3 cases, 12.5% in group NIV were changed to HFNV care, in contrast, there was no case changed to the NIV treatment, P<0.05;without any cases in two groups, they were reintubated for invasive ventilation.One old male case in HFNC group died from acute myocardial infarction, but there was no significant differences in ICU stay and mortality.Conclusion Compared with the NIV, in general, the HFNC therapy may offer a better choice for post-extubation mild-to-moderrate hypoxemia after abdominal surgery in ICU.
Objective To explore the effectiveness, safety and the possible mechanism of Celecoxib Capsules (Celebrex) in treatment of female stress urinary incontinence.Methods From January 2014 to December 2015, 80 female patients with stress urinary incontinence, from the outpatient department of our hospital, were selected in experimental group, who took celecoxib orally 200 mg, Bid for 3 months.The other 80 female patients with stress urinary incontinence were selected in control group, who took placebo (medical starch) orally 1 piece, Bid for 3 months.The two groups of patients were treated with behavioral therapy and functional exercise of pelvic floor.Both groups of patients were assessed with the urine pad test and quality of life scale 3months later.The study was approved by the Institutional Review Board of the second Hospital of Kunming Medical University and followed the principles expressed in the Declaration of Helsinki.The written informed consent was obtained from every patient.Results The difference between the experimental group and the control group in the age, the course of disease had no statistical significance (P>0.05) .The effectiveness of experimental goup was better than the controlled group.The urine volume in experimental group is less than that in control group in urine pad test.And the score of experimental goup in QOL-36-SF was higher than that of control group both in physical dimension and the psychological dimension.The overall effective rate was 96.3% in experimental group [21 (26.3%) cases were improved markedly, 56 (70%) patients improved, 3 (3.8%) patients was failed] and88.6% in control group [15 (18.8%) cases were improved markedly, 56 (70%) patients improved, 3 (3.8%) patients was failed].Tthe difference has statistical significance (P<0.05) .No obvious side effects were observed in both groups and no one stooped the treatment by herself.Conclusion Celecoxib can attenuate the symptoms of female stress urinary incontinence, and has the good safety and tolerability.
Objective To explore the MRI imaging appearance of the normal anatomy and primary mass in hepatoduodenal ligament and to evaluate the diagnostic value of its abnormalities by MRI.Methods From August2004 to October 2013, 11 cases of lesions in hepatoduodenal ligament confirmed by drug therapy (antituberculosis therapy) or surgery and pathology were collected.All were performed plain and enhanced MRI.Results Eleven cases were benign lesions including lymph nodes tuberculosis in 3 cases, neurilemmoma in 2 cases, neurofibroma in1 case, teratoma in 1 case, lymphangioma in 2 case and simple cyst in 2 cases.There was no relapse after 3months5 years.Conclusion MRI has a good diagnostic value in locating the lesions in hepatoduodenal ligament and its multi-aspect image formation is very useful in differentiating other tumor developing from biliary tract, liver, pancreas or duodenum.Its good soft tissue resolving power is helpful for qualitative diagnosis and differential diagnosis.
Objective To explore the efficacy of different non-biological artificial liver supporting system on liver function failure by retrospective study.Methods One hundred and six patients with liver failure were selected, who were diagnosed in the Third People's Hospital of Kunming City from January, 2014 to January, 2015, and during this period 240 person-time of the artificial liver treatment were completed.The patients weredivided into two groups according to the types of artificial liver, 62 patients in plasma exchange group (referred as PE) , 44 patients in plasma exchange combined with hemodiafiltration group (referred as PE+HDF) .Before and after the artificial liver treatment, we recorded the artificial liver laboratory indexes of the patients and model for end-stage liver disease (MELD) score, so as to evaluate the efficacy of two groups of treatment.Results (1) For these two groups, after treatment, the serum total bilirubin (TBIL) , aspartate aminotransferase (AST) , alanine aminotransferase (ALT) , total bile acid (TBA) levels were significantly reduced, and the difference was statistically significant (P<0.05) ; (2) Total bilirubin and bile acid clearance rate after artificial liver treatment was equal to 1-after treatment before treatment, compared with the group of PE, total bilirubin and bile acid clearance rate of PE+HDF group was higher, and the difference was statistically significant (P<0.05) .After PE treatment, the clearance rates of total bilirubin and bile acid were 54.07±11.03% and 85.95±30.22% respectively;after PE+HDF treatment, the clearance rates of total bilirubin and bile acid were 63.03±11.15% and 92.40 ±40.13% respectively; (3) After treatment, prothrombin activity (PTA) index significantly increased and International Normalized Ratio (INR) value decreased both in PE group and PE+HDF group, and the difference of PTA and INR before and after treatment in each group was statistically significant (P<0.05) , suggesting that both the two models had significant effects on improving the coagulation function; (4) Compared with the group of PE, the promotion of the PTA in PE+HDF group was better, and the difference was statistically significant (P<0.05) ;The PTA increased by 88±65% in group PE while increased by 102±73% in PE+HDF group; (5) By comparing the changes of blood ammonia before and after treatment between the two groups, blood ammonia had no improvement after treatment in group HE;And blood ammonia dropped significantly after treatment in PE+HDF group, with the statistically significant difference before and after ammonia treatment (P<0.05) .Blood ammonia decreased by 50.58±23.58% after treatment in group PE+HDF; (6) Renal function was damaged after treatment compared with that before treatment in PE group, which could cause the serum sodium to increase and serum potassium to decrease, with the statistically significant difference (P<0.05) ;In group PE+HDF, the treatment could improve the renal function and correct hyponatremia and hyperkalemia, with statistically significant difference (P<0.05) ; (7) Model for end-stage liver disease (MELD) score decreased after treatment both in HE group and PE+HDF group, and the differences of each group before and after treatment were statistically significant (P<0.05) ;Compared with the group of PE, PE+HDF group improved more in the end-stage liver disease score, and the difference was statistically significant (P <0.05) .Conclusion Compared with plasma exchange, plasma exchange combined with hemodiafiltration can increase the removal of albumin bound toxins (total bilirubin and bile acid) , is more effective in ptomoting prothrombin activity, and increasing the clearance of small and middle molecules so that the treatment is more effective on patients with liver failure complicated with hepatic encephalopathy, liver and kidney syndrome and internal environment disorder, etc;Both plasma exchange and plasma exchange combined with hemodiafiltration can significantly improve the model of end-stage liver disease, and reduce the risk of death and improve prognosis, but plasma exchange combined with hemodiafiltration is better.
Objective To investigate the effects of electrical stimulation on peripheral by Ultrasound-guided on low muscular tesion after stroke.Methods Sixty-two stroke patients with low muscular tesion were divided randomly into a control group and a observation group.Both groups received routine clinic treatment and rehabilitation training, but the observation group was additional treated with peripheral nerve stimulation by ultrasound-guided.Modified Ashworth Scale (MAS) was used to evaluate the effects before and after treatment, and time of 2groups that the tension of biceps brachii was improved to 2 level was compared.Results Compared with the control group (79.5%) , the total effective rate of cases in the observation group (93.3%) was statistically higher (P<0.05) ;Compared with the control group (17.21±5.12) d, the time of the observation groups (10.15±4.36) d that the tension of biceps brachii improved to 2 level was shorter (P<0.05) .Conclusion Peripheral nerve stimulation by ultrasound-guided may improve muscle tension in flaccid.
Objective To observe the clinical result of treating upper urinary tract calculi greater than 2cm with combining flexible and rigid ureteroscopic Ho: YAG laser lithotripsy under the help of ESWL.Methods Forty patients with upper urinary tract calculi greater than 2 cm were observed, the diameter of the calculi of them was between 2 cm and 3.2 cm, and the average value was 2.6 cm.Twenty-one patients were treated by combining flexible and rigid ureteroscopic Ho:YAG laser lithotripsy, others were treated by flexible ureteroscopic Ho:YAG laser lithotripsy only, and ESWL was performed after the operation for some big remains.Results The average operative time of combining flexible and rigid ureteroscopic group was 96 minutes, and that of the other group was144 minutes, the difference of operative time between these 2 groups was significant (P<0.05) .In combining flexible and rigid ureteroscopic group 13 patients got a successful lithotripsy in the operation, 6 patients with big remains got a successful ESWL after the operation, the total success rate of this group was 90%;to the other group, 8 patients got a successful lithotripsy in the operation, 4 patients with big remains got a successful ESWL after the operation, the total success rate of this group was 60%, the difference was significant (P<0.05) .No seriouscomplications occurred in both groups.Conclusion Compared with the flexible ureteroscopic Ho: YAG laser lithotripsy, combining flexible and rigid ureteroscopic Ho:YAG laser lithotripsy can get a better result for shorter operative time, higher success rate, in treatment of upper urinary tract calculi greater than 2 cm, and the help of ESWL will make the results better.However, careful chosen of patients is important for a good result.
Objective To evaluate the efficacy of transcatheter arterial chemoembolization (TACE) in combination with intensity modulated radiotherapy (IMRT) in treatment of primary hepatic carcinoma (PHC) .Methods All 68 patients with PHC were randomized into treatment group (TACE plus IMRT) and control group (TACE alone) , with 34 cases in each group.Patients in the control group were treated with interventional therapy alone.Patients in the treatment group were treated with interventional therapy (transcatheter arterial chemoembolization) plus radiotherapy.Treatment group patients firstly received 2 to 3 times interventional therapy, then followed by radiotherapy with 2 to 4 week interval.Results The short-term efficacy was 82.4% in treatment group which was significantly higher than 61.8% in control group (P<0.05) .The survival rate was 76.5% in 1 year and55.9% in 2 years in treatment group, 58.9% and 32.4% in control group respectively.The difference between 2group was statistical significance (P<0.05) No serious toxicities were observes in the patients of the two groups.Conclusion The results suggest that the treatment effect of TACE plus IMRT is better than interventional therapy alone for patients with primary Hepatic carcinoma, and has well tolerance and fewer adverse reactions.
Objective To investigate the relationship between soluble cytokine receptors, matrix metalloproteinases and pancreatic cancer.Methods Fifty-nine patients with pancreatic cancer in our hospital from April 2014 to May 2016 were selected in the observation group, 59 healthy subjects were selected in the control group, then the serum soluble cytokine receptors and matrix metalloproteinases levels of control group and observation group were detected and compared, and the detection results of observation group with different Hemrick stages and pathological classifications were compared, then the relationship between soluble cytokine receptors, matrix metalloproteinases and pancreatic cancer were analyzed by the Logistic analysis.Results The serum soluble cytokine receptors and matrix metalloproteinases levels of observation group were all higher than the levels of control group, the detection levels of patients with higher Hemrick stages were all higher than the levels of patients with lower Hemrick stages, all P <0.05, while the detection levels of patients with different pathological classifications had no obvious differences, all P >0.05, the Logistic analysis showed that the soluble cytokine receptors and matrix metalloproteinases all had close relationship to the pancreatic cancer.Conclusion Thesoluble cytokine receptors and matrix metalloproteinases of patients with pancreatic cancer show higher expression state, and have a indication value for the Hemrick staging, these indexes have high monitoring value for pancreatic cancer.
Objective To explore the clinical effect and application value of video-assisted thoracic surgery (VATS) for mediastinal tumor by comparing with traditional thoracotomy.Methods Thirty-two patients with mediastinal tumor were treated by VATS from June, 2012 to June, 2016 and the clinical data of 50 patients with mediastinal tumor undergoing thoracotomy from January 2003 to June 2012 were retrospectively analyzed.Results VATS group vs Thoracotomy group:There were no conversion thoracotomy, postoperative death, no thoracotomy for hemostasis in VATS group.There were 6 cases of incision infection, 8 cases of pulmonary infection, 8 cases of pulmonary infection, and no other serious complications in two groups.The incidence of incision infection, pulmonary atelectasis and pulmonary infection rate in VATS group was lower than thoracotomy group (P<0.05) .There were differences in the intraoperation bleeding volume (188.7±28.77 vs 266.4±54.55) m L, postoperative drainage (212.3±28.48 vs 352.44±9.67) m L, drainage time (2.65±0.71 vs 3.64±0.92) d, analgesia time (1.35±0.49 vs 2.38±0.49) d, hospitalization time (8.74±0.89 vs 12.16±1.11) d between two groups, P<0.05.Conclusion VATS in the treatment of mediastinal tumor has obvious advantages compared with traditional thoracotomy surgery, so it has good clinical efficacy and the the value of promotion.
Objective To explore the clinical effect of internal fixation in treatment of sacroiliac joint complex injury.Methods Forty-two patients with sacroiliac joint complex injury who admitted to our hospitals from May 2013 to May 2014 were selected in this study.All enrolled patients were treated by percutaneous cannulated screw internal fixation, and the therapeutic effect was evaluated after treatment.Results All patients received X-ray scanning and CT scanningafter surgical treatment.The location of cannulated screw for internal fixation was accurate, and there was no position deviation and the phenomenon of sacral nerve injury.Among the 42 patients treated by surgery, 40 patients got fine pelvic recovery, the fine rate was 95.23%, 39 cases got good function recovery, the fine rate was 92.86%, and patients could get out of bed to receive rehabilitation training in one to four months after surgery.All patients were followed up for 6 months to 12 months, and all patients recovered well without complications such as inequality length of lower extremity and pelvic deformity.Conclusion Internal fixation for treatment of sacroiliac joint complex injury can get very good effect, and the operation is simple, safe and effective.
Objective To study the correlation between the ECG changes and coronary angiography of acute myocardial infarction (AMI) .Methods A total of 171 patients diagnosed as AMI in The first people's Hospital of Neijiang City was selected. Based on patients' ECG characteristics, they were divided into ST-segment elevation group and non-ST-segment elevation group.Their ECG changes and coronary angiography were analyzed.Results Compared with the coronary angiography, most ST-segment elevation group patients had LAD occlusion, total occlusion and single branchpathologically changed;while most non-ST-segment elevation group patients had LCX occlusion, middle tosevere stenosis and three main tubes pathologically changed.There were significant statistical differences (P<0.01) .However, the incidence of complications in the two groups had no significant difference.Conclusion The different ECG ST segment changes reveal the different coronary lumen stenosis, occlusion of blood vessels and different count under coronary angiography, but all belong to cardiovascular emergencies, which need to pay attention with.
Objective To compare the curative effect of Zina and Sextant screwing system in the treatment of thoracolumbar fracture.Methods A total of 53 patients included who underwent minimally invasive spine surgery for unstable thoracolumbar fracture without neurological deficit from Jan 2013 to Apr 2014.30 patients were treated with Zina system and 23 patients were treated with Sextant system.The two groups were compared in terms of perioperative indexes, Cobb angle, visual analogue scales (VAS) , Oswestry disability index (ODI) and hospitalization expenses.Results All patients were followed up for 1224 (mean, 16.9) months.Compare with Sextant group, Zina group had significantly shorter surgery time, less intraoperative X-ray exposure, blood loss and hospitalization expenses (P<0.05) .The cobb angle, VAS and ODI at one week, 3 and l 2 months after operation were significantly improved in all the patients (P<0.05) , but the differences between the two groups were not significant (P>0.05) .Conclusions The two minimally invasive pedicle screw systems are effective in the treatment of thoracolumbar fractures, but compared with the Sextant system, Zina system can shorten the operation time, reduce the intraoperative bleeding, X-ray exposure and hospitalization cost.Therefore, the Zina system may be preferable in the treatment of thoracolumbar fracture without neurological deficit.
Objective To study the clinical effects of Cook double balloon and Oxytocin in promoting cervical ripening and increasing successful induction rate of labor at the third trimester of pregnancy.Methods A total of 200 primiparae at third trimester of pregnancy with indications of induction labor in our hospital from January2013 to December 2015 were recorded and analyzed.They were divided into two groups: Cook double balloon group (n=103) and oxytocin group (n=97) , Then we compared the effects of Cook double balloon group with Oxytocin group in promoting cervical ripening and increasing successful induction rate of labor.Results Cook double balloon group was better than Oxytocin group in promoting cervical ripening and increasing successful induction rate of labor (80% vs 45%, 80% vs 30%, P<0.01) .Conclusions Cook double balloon has a better effect in promoting cervical ripening and increasing successful induction rate of labor than only received intravenous infusion of oxytocin.

Sebaceous gland is the vital appendix organ of skin, and the abnormal of its function is associated with the development of acne and other diseases involving the sebaceous gland.The establishment of human sebocytes culture models in vitro provides a good platform for the study of sebaceous gland function and related diseases.At present, there has been already developed the immortalized human sebaceous gland cell lines, like SZ95 sebocytes, SEB-1 sebocytes, Seb-E6E7 sebocytes, which become unique tools for research on functions and activity regulations of sebaceous gland, as well the pathogenesis and therapy of acne and other sebaceous gland-associated disorders.This article summarizes the foundation of human sebocytes culture models in vitro and the development of immortalized human sebaceous gland cell lines, also discusses its applications in clinic researches.
Objective To research the application effect of PBL in combination with the camera feedback method in molecular biology experiments teaching for foreign students.Methods The research objects were five-year medical international students of 20132014.Students were divided into two groups:experimental group using PBL in combination with the camera feedback method, and the control group using the traditional teaching method.The examination scores, spoken English skills and active learning ability were compared between the two groups.Results TThe operating grades, theoretical test and operational test scores of the experimental group were significantly higher than those of the control group (P=0.001) ;The final examination scores of the experimental group were significantly higher than those of the control group (P=0.01) ;The abilities of spoken English, active learning of the experimental group were significantly higher than those of control group, et cetera (P=0.001) .Conclusion The effect of PBL in combination with the camera feedback method in molecular biology experiments teaching for foreign students is very good.
Objective Through standard collective lesson preparation, the objectives of drawing on the wisdom of the masses, complementing each other's advantages, integrating resources and improving the quality of imparting knowledge and educating people can be achieved.Methods The students' assessment scores of classroom teaching quality of teachers undertaking teaching tasks in the Teaching and Research Office during2014.032015.01 and 2015.032016.01 was used as control group and research group, respectively.The data was compared and statistical analyzed using SPSS.Results The final examination's average score of Medical Imaging Science in research group was 89.59, which is higher than that score in control group (83.13) . And the average score of teaching quality evaluation from students was 95.61, which got an obviously improved compared with the control group (92.26) . T test showed that there was a significant difference between control group and research group both in students' score and teaching quality (P<0.05) .Conclusion There are seven advantages of standard collective lesson preparation: (1) it is conducive to give play to teachers' collective wisdom and realize knowledge sharing; (2) it can help teachers master the key points and difficulties of teaching correctly and improve teaching levels; (3) it is conducive to students' reception of knowledge; (4) it is conducive to enhance the mutual understandings and communication between teachers, cooperative teaching consciousness and cohesive force and create favorable teaching atmosphere; (5) it is conducive to popularize excellent teachers' teachingexperience and make up the one-sidedness and limitation of individual lesson preparation as well as fully improve teachers' qualities; (6) it is conducive to save time, save labor and improve working efficiency; (7) it is conducive to enhance the precaution consciousness of teaching accidents and errors.
Objective To study the nursing methods for interventional treatment in induced abortion after placenta hemorrhage in patients with placenta previa in the mid-term gestation.Methods We selected 32 patients with placenta previa who received embolization treatment for induced abortion by rivanol amniotic cavity injection after placenta hemorrhage.According to the admission date, patients were equally divided into 2 groups:16 cases of patients in the research group received preoperative.intraoperative and postoperative care.and 16 cases in the control group were only given routine care, then the clinical outcomes were compared between two groups.Results Among 32 patients patients with placenta previa who received induced abortion after placenta hemorrhage in the mid-term gestation, 27 patients delivered fetals and affiliated tissues without postpartum hemorrhage, 5 cases of patients who experienced induced labor failure caused by scar uterus and the cervical dystocia were given embolism treatment and uterine-incision delivery 6 days later.The total effective rate was 87.50% (14/16) in the research group and 56.25% (9/16) in the control group, there was a statistically significant difference (P <0.05) .Conclusion Uterine artery embolization is effective in treatment of placenta hemorrhage in patients with placenta previa in the mid-term gestation.Strengthening preoperative, intraoperative and postoperative nursing for patients can effectively improve the success rate of interventional treatment and reduce the complications.
Objective To investigate the effect of position and activity guidance on the recovery of gastrointestinal function of patients undergoing gynecological laparoscopic surgery.Methods One hundred and thirty patients undergoing gynecological laparoscopic surgery were randomly divided into observation group and control group, 65 cases in each group.The control group used routine nursing, the observation group was given position and activity guidance based on the conventional care.The clinical curative effect, recovery of gastrointestinal function and quality of life were compared between the two groups.Results The clinical effective rate in observation group was 89.23%, it was significantly higher than that of the control group (75.38%) , the difference was statistically significant (P<0.05) . The degree of abdominal distention in observation group were milder than that of the control group, the recovery time of bowel sound and anal exhaust time were less than that of the control group, the incidence of appetite and constipation was lower than that of the control group, the differenceswere statistically significant (P <0.05) .After intervention, the quality of life of the observation group were significantly higher than that of the control group, the difference was statistically Statistically significance (P <0.05) .Conclusion The patients undergoing gynecologic laparoscopic surgery taking a scientific and proper position and activity guidance, can reduce the occurrence of abdominal distension, promote the recovery of gastrointestinal peristaltic function, improve the quality of life of patients.It is worth of clinical application.
Objective To study the correlation between the serum levels of platelet-derived growth factor-BB and carotid artery intima-media thickness in patients with essential hypertension. Methods Ninety essential hypertension patients without medication intake and other diseases were examined or blood and carotid artery color Doppler ultrasonogra.The serum levels of PDGF-BB were measured using ELISA.Carotid artery intima-media thickness was measured using color Doppler ultrasonogra.And, they was analyzed by linear correlation. Results The systolic blood pressure was 150.16±9.18 mmHg,diastolic blood pressure was 99.92± 6.32 mmHg,carotid artery color Doppler ultrasonogra was 0.78±0.43 mm,The serum levels of PDGF-BB was 25.78 ±12.76 ng/mL.The serum level of PDGF-BB was moderately positively correlated with carotid artery intima-media thickness ( r =0.483, P =0.000, P <0.001). Conclusion The serum level of PDGF-BB is moderately positively correlated with carotid artery intima-media thickness in patients with essential hypertension.
