Abstract:
Objectives To observe the NMDA receptor subunit 2B and iNOS expression in rat striatum by injected methamphetamine (MA) and HIV-Tat protein and to explore the synergies of MA and HIV-Tat on neural injury mechanisms. Methods A total of 50 healthy male SD rats were randomly divided into four groups: MA group which received intraperitoneal injection of 10 mg/kg MA by twice per day for 4 days, HIV-Tat group which received intrastriatal injection of 10 μg HIV-Tat, MA + HIV-Tat group which received intraperitoneal injection of MA for 4 days and followed by intrastriatal injection of HIV-Ta) t, and control groups which received intraperitoneal or intrastriatal injection of saline. The rats were killed at 48 h or 7 d after the last time of injection.NMDA receptor subunit 2B and iNOS expression were examined by using immunohistochemistry and western blotting. The results were analyzed and average optical density was measured. Results Compared with those in control group, NMDA receptor subunit 2B and iNOS expression in rat striatum increased in the experimental groups (P <0.05) .Compared with those in MA group or HIV-Tat group, NMDA receptor subunit 2B and iNOS expression increased more significantly in MA + HIV-Tat group.Conclusion Synergies of MA and HIV-Tat protein can dramatically increase the expression of NMDA receptor subunit 2B and iNOS. NMDA acceptor subunit 2B and iNOS can participate in the process of MA and the HIV-Tat protein synergy nerve toxic mechanism.
Distribution of Metaldehyde in Some Tissues of Subacutely Poisoned Rats

Abstract:
Objective To develop the method for the determination of metaldehyde in biological material by gas chromatography/mass spectrometry (GC/MS) and GC-FID and to investigate the distribution of metaldehyde in main tissues of subacutely poisoned rats.Methods SD rats were given the oral administration of metaldehyde (40mg/kg, 140 mg/kg, 280 mg/kg) and executed by hypoxia after one week. Cardiac blood, liver, stomach and kidney were taken out and the contents of metaldehyde were detected. The rats in control group were given the oral administration of physiologic saline, and their blood and tissues were taken out correspondingly to be used as blank.The blood and tissues samples were extracted by dichloromethane. The metaldehyde was qualitatively analyzed by GC/MS and quantitatively analyzed by GC-FID.Results A GC/MS and GC-FID method for the determination of metaldehyde in biological samples was established. The limit of detection of GC/MS was 0.1ng while the GC-FID method displayed a satisfactory linearity within the concentration ranges of 0.010.1 mg/mL (R=0.9992) . The average recovery was over 75%.Conclusion The order of metaldehyde content in blood and tissues is: gastric tissues>cardiac blood>kidney>liver.This method is simple, quick, accurate, replicated and suitable for the determination of metaldehyde in biological samples.

Abstract:
Objective To observe the effect of Osteoking on blood biochemicalindicator of tree shrews with osteoporotic fracture (OPF) .Methods Totally 30 female tree shrews (Tupaiabelangeri) were randomly divided into the treatment group, the model group, and the control group, with 10 in each group. Bilateral ovaries and uterus were ectomized for 6 months in the treatment group and the model group, and right fibula fractures were induced after confirmed osteoporosis. Tree shrews in the treatment group were administered with Osteoking by gastrogavage, once per two days. Equal volume of normal saline was given to tree shrews in the model group and control group. Blood biochemical indexes were detected at 3 months after fracture.Results Compared with those in the control group, serum creatine kinase isoenzyme (CK-MB) , alanine aminotransferase (ALT) , magnesium (Mg2+) levels significantly increased (P<0.01) while total cholesterol (CHOL) and blood glucose (Glu) levels were significantly decreased (P<0.01) in the OPF model group. Compared with those in the model group, serum CK-MB and ALT significantly decreased (P<0.01) and Mg2+ significantly decreased (P<0.05) while CHOL and Glu significantly increased (P<0.05) in the treatment group. Conclusion Osteoking can protect the heart and liver function of tree shrew with osteoporotic fracture.

Abstract:
Objective To investigate the different expression of Caspase-9 in condylar cartilage and growth plate cartilage and to explore the role of Caspase-9 in the development of cartilage.Methods Animal models of condylar cartilage and growth plate cartilage of SD rat were established during different development stages (E15d-P30d) . The expressions of Caspase-9 were examined by immuno-histochemical staining. Results Positive xpression of Caspase-9 expression in condylar cartilage was stronger than that in growth plate cartilage (P<0.05) .The distribution of positive cells in growth plate cartilage is consistent with those in the condylar cartilage. Caspase-9 positive of fetal rat mainly expressed in the proliferative zone and expressed in the chondroblast zone and hypertrophic zone after birth. Conclusion Caspase-9 is involved in the growth and development of condylar cartilage and growth plate cartilage, which may play an important role in the development of cartilage. Caspase-9 may be of the same or similar molecular mechanism for the development of condylar cartilage and growth plate cartilage.

Abstract:
Objectives To study the component and thermal stability of the Antibacterial metabolite 108 from an fungal endophyte strain of Penicillium griseofulvum in Paris pohyphylla Smith var. yunnanensis (French.) Hand.-Mazz, and to compare its stability with that of amoxicillin. Methods The Staphylococcus aureus, Salmonella typhi and Endococcusfaecium were used as indicators to test the antibacterial activity of 108 in vitro (using paper disk test) . Then, thin layer chromatography (TLC) and column chromatography were used to separate and purify the secondary metabolites of P. griseofulvum. Finally, thermal stability of 108 was compared with amoxicillin's according to the diameters of inhibition zones. Results A compound with antibacterial activity named as 108 from metabolites of P. griseofulvum was isolated. The separated product 108 was found to be a single compound after ultraviolet scanning and thin layer chromatography. The diameters of inhibition zones against the indicators were 24 mm, 25.3mm and 7.6mm respectively at room temperature. Comparing with amoxicillin at the same temperature, this compound heated at 121.3℃ can keep steadier diameters of inhibition zones. Conclusion Compound 108, therefore, is considered to have tolerance of high pressure steam sterilization and strong thermal stability.

Abstract:
Objective To establish a simple, rapid, accurate and reliable method of gas chromatography-mass spectrometry (GC-MS) with water extraction in order to determine the paraquat in biomaterial. Method The biomaterial was firstly pre-treated by using water as reagent to extract paraquat, and then GC-MS was employed to determine these samples. The retention time of the paraquat in total ion current (TIC) and the characteristic fragment ions of paraquat in mass spectrogram were taken as qualitative basis. Result The method demonstrated several advantages: simple sample pre-treatment, little consumption of organic reagents, less background interference, good separation effect, and fast analytical speed. The retention time was 6.674 mins and the characteristic fragment ions was 156 (m/z =156) . The recovery rate was 91.42% and the relative standard deviation (RSD) of the intra-day and the inter-day were 4.83% and 6.87% respectively. The lowest limit of detection (LLOD) was 0.005mg/mL. Conclusion The method can be applied to determine the paraquat in biomaterial of forensic toxicological analysis.

Abstract:
Objective To investigate the effect of alcohol on hematological indexes of Tupaia belangeri chinensis.Methods A total of 40 Tupaia belangeri chinensis, half male and half female, were randomly divided into alcohol group and control group. Tree shrews in alcohol group were given star Erguotou with alcohol content of 56% by gavage, with the dosage of 0.38 mL per 100 g while those in control group were given distilled water by gavage with the same dosage. After 90 days of gavage, 0.5 mL blood from the heart were taken 12 hours after fasting.EDTA-K2 was used and mini TEK-II automatic hematology analyzer was applied to measure the indexes of hematology, and the results were analyzed with SPSS software. Platelet count (PLT) , white blood cell (WBC) , the absolute value of lymphocyte (LY#) , Results The absolute value of intermediate cells (Mid#) and plateletcrit (PCT) in alcohol group were significantly higher than those in the control group (P <0.05) . In alcohol group, white blood cells (WBC) , lymphocyte absolute value (LY#) , the absolute value of intermediate cells (Mid#) of male tree shrews were significantly lower than those of female tree shrews (P <0.05) .Conclusions Alcohol can cause changes in tupaia belangeri chinensis hematological indexes, and the changes of female tree shrews are more significant.

Abstract:
Objectives To explore the experimental approach for in vitro primary culture of chondrocytes of mouse cranial base synchondrosis and to investigate its basic biological features. Methods The spheno-occipital synchondrosis of new natal mouse was harvested through surgical dissection, from which chondrocytes were further isolated through enzymatic digestion with trypsin and type II collagenase. Chondrocytes of the primary and successive generations were cultured with serum containing Dulbeccomodified eagle's medium (DMEM) . Optical microscopic imaging, growth curve tracing and immunohistochemistry were conducted to observe the morphological features, growth behavior and type II collagen expression. Results Within five generations, the cultured cells consistently presented typical morphological and functional features of chondrocyte. Conclusion The surgical tissue dissection combined with double enzymatic digestion is a reliable experimental approach for the primary culture of chondrocytes of mouse cranial base snychondrosis.

Abstract:
Objective To investigate the changes in Hippocampus synapses caused by long-term depression (LTD) in old rats with neuropathic pain. Method Fifteen old male Wistar rats were randomly divided into 3 groups (n = 5 each) : sham operating group (group S) , neuropathic pain group (group NP) , and NP + AP5 group (group NA) . Neuropathic pain was produced by ligation of the right L4, L5 spinal nerve. The behaviour of the rats was evaluated. The pain threshold was measured by electric stimulation at 7, 14 and 21 d after surgery. The excitatory post-synaptic potential (EPSP) in hippocampal CA1 region was recorded at 3 d after the last measurement of pain threshold. The changes in EPSP and input/output curve and LTD induced by low frequency stimulation (LFS) of the CA1 stratum radiatum of the rat hippocampus were observed using stereotaxic technology and extracellular recording respectively. Group NA received infusion of AP5 (NMDA receptor antagonist) 6 μl by i.c.v 20 min before LFS. Results The pain threshold was significantly lower in group NP and NA than in group S.The amplitude of LTD was significantly lower in group NP than in group S and NA (P<0.05) , but there was no significant difference in the amplitude of LTD between group S and NA (P>0.05) . Conclusion The results suggest that the neuropathic pain by ligation of the right L4, L5 spinal nerve might facilitate LTD induction in the CA1 area of the hippocampus of old rats; mechanism might be related to the activation of NMDA receptors.

Abstract:
Objective To induce and block to induce neuroendocrine differentiation of colon adenocarcinoma cell HCT-116 by using the epidermal growth factor (EGF) and epidermal growth factor receptor (EGFR) inhibitor Cetuximab, and further explore the intrinsical changes and association with the characteristics of colonic cancer cells HCT-116 in proliferation, apoptosis, possible mechanism, and the adjustment factors of neuroendocrine differentiation. Methods HE and immunohistochemical stains were conducted to observe morphological characters of colonic cancer cultured cells HCT-116 and tumor in the nude mouse by expression of Cg A, Ki-67, Bcl-2, Syn, CEA, CA19-9, COX2, CDX2. Cell growth curve of HCT-116 was drawn by MTT, flow cytometry technique was used to determine the cell cycle of HCT-116, and Low Density Plate Clone Test was applied to measure the cloned forming rate of HCT-116. Results Morphology of HCT-116 in the induced courses was similar. EGF acted on cultured HCT-116 cells to strengthen the expression of Cg A, Ki-67 and Bcl-2, to increase cell proliferation rate and proliferation activity in cell cycle of S and G2 phase. The colony formation rate was also increased significantly compared with the control group. Difference with cetuximab group was statistically significant (P <0.001) . Changes of proliferation index of Ki-67 in cultured HCT-116 cells was statistically significant in EGF group compared with the other groups (P<0.01) . The differential expression of NSE, CEA, CA19-9, CDX2, COX2, and Syn in each group was no statistically significant (P>0.05) . Conclusion EGF can induce neuroendocrine differentiation in cultured HCT-116 cells NED, increase cell proliferation activity, strengthen antiapoptotic ability of colonic cancer cells. EGFR can block EGF to induce HCT-116 NED, and inhibit changes of relevant biological features. However, CEA, CA19-9, COX2, and CDX2 do not regulate NED, proliferation, and apoptosis.

Abstract:
Objectives To revise the second version of the Quality of Life Instrument for Cancer Patients-Stomach Cancer and to discuss the preliminary applications of the instrument. Methods A combination of common modules plus specific modules, literature review and analysis of Chinese culture, programmed decision processes were conducted, including information collection, group discussion, statistical analysis, and pre-tests, to revise items of the QOL instrument for patient with stomach cancer. Statistic methods of variation coefficient, correlation coefficient and variance analysis were applied. Results (1) A 23-item specific module was formed by reviewing the literature and collecting the opinions of core group. Then 20-item specific module was picked after further group discussion. (2) The final 10-item specific module was developed based on the data of pre-survey. Conclusion Development of the specific items of QLICP-ST (V 2.0) is clinically relevant and can be used for assessing the quality of life of gastric cancer patients in China as a clinical tool.

Abstract:
Objective To use TaqMan MGB probe technology to establish a detection methodology of Candida albicans (C.albicans) by real-time fluorescence quantitative PCR (RQ-PCR) and to provide a quick method for the detection of candida infection in clinical settings. Methods D1/D2 region of candida species which is a highly conserved sequence gene was selected as amplification target sequence and specific primers and probes for C.albicans were synthesized. The DNA of standard strains of C.albicans was extracted and rebuilt plasmid containing the target gene fragment was constructed. The DNA was amplified as a standard to establish the standard curve of C.albicans and methodological evaluation was conducted to identify the linear range, repeatability, sensitivity, specificity and so on. Results After sequencing the recombinant cloning vector, the sequence of the fragment inserted was consistent with the expected sequence, with the similarity of 100%. C.albicans standard curve Y =-3.824 x +41.36, R2= 0.990, which was in good shape and illustrated the detection method of C.albicans by real-time PCR had successfully established. Conclusion The detection method of C.albicans by real-time PCR has better sensitivity and specificity.

Abstract:
Objective To analyze the clinical effects of Ilizarov external fixation in arthrodesis on patients with end-stage complex ankle arthritis. Methods From January 2013 to May 2014, 19 patients with end-stage complicated ankle arthritis in our hospital underwent ankle arthrodesis with Ilizarov external fixation. X-ray were conducted regularly after the operation. Clinical outcomes were evaluated by using the American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot score and the Visual Analogue Scale (VAS) in pre-operation and the last follow-up. T-test was applied to analyze data. Results The 19 patients obtained an average follow-up of14.7 months (ranging from 6 to 25 months) , no injury to key vascular and nerves during operation. Ankle pain was obviously relieved, and deformity was improved significantly at the last follow-up. All patients achieved full arthrodesis according to the X-ray results after 11 to 16 weeks (average of 14.7 weeks) . In the Visual Analogue Scale, preoperative resting state was scored 4.9 and the weight state was 7.9, which were significantly reduced to 1.2 and 1.9 respectively after operation (P=0.000) . In AOFAS ankle-hindfoot score, the pre-operative score was 58.74±4.00, which significantly increased to 93.21±4.47 in the last follow-up (P=0.000) . Conclusions Ilizarov external fixator is effective in the arthrodesis of end-stage complex ankle arthritis. And its advantage shows in axial compression, smaller soft tissue injury, and higher fusion rate.
Effects of Dead Space Fraction on Prone Position Ventilation in ARDS Patients

Abstract:
Objective To evaluate the effects of dead space fraction (VD/VT) on prone position ventilation (PPV) in patients with severe acute respiratory distress syndrome (ARDS) . Methods A total of 30 patients with severe ARDS (PaO2/FiO2< 100 mm H) g were enrolled in this study and were divided into pulmonary insult ARDS group (ARDSp) and extrapulmonary insult ARDS group (ARDSexp) according to different etiological factors. PaO2/FiO2 and VD/VT were recorded in supine position and prone position at 2 h, 4 h, 6 h, 8 h and 10 h after ventilation. Results VD/VT decreased and PaO2/FiO2 increased with PPV. PaO2/FiO2 and VD/VT showed significant difference at different time points (P < 0.001) , with the biggest change of VD/VT at PPV 4 h and the best transformation of PaO2/FiO2 (increase of PaO2/FiO2>20 mm H) g after PPV 6 h. There was a positive correlation between VD/VT and PaO2/FiO2 by Pearson correlation analysis (t=0.67, P < 0.001) . In ARDSp group, there was no significant change of PaO2/FiO2 and VD/VT between supine position and PPV 2 h. At PPV 4 h, PaO2/FiO2 significantly increased (t=2.37, P=0.03) and VD/VT significantly decreased (t =5.26, P=0.001) . No significant difference was found in PaO2/FiO2 and VD/VT at PPV 4 h to PPV 6 h. In ARDSexp group, PaO2/FiO2 significantly increased (t=4.65, P=0.04) and VD/VT significantly decreased (t=3.56, P=0.009) at PPV 2h. There was no significant change from PPV 2 h to PPV 4 h. PaO2/FiO2 and VD/VT remained at the same level at PPV 6 h. Conclusion VD/VT can be used as an effective index to evaluate the efficacy of PPV in patients with ARDS and is more sensitive than PaO2/FiO2, so it can be used to guide the treatment of patients with pulmonary and extrapulmonary ARDS.

Objective To observe and analyze the surgical treatment of ischemic moyamoya disease (IMMD) in adult patients.Method The clinical data of 22 patients with surgical treatment of ischemic moyamoya disease (IMMD) from January, 2014 to December, 2015 were analyzed retrospectively, 15 cases with Encephalo-STA-STF-synangiosis (E-STA-STF-S) surgery and 7 cases with superficial temporal artery-middle cerebral artery bypass (STA-MCA) surgery. Results Follow-up was conducted 3-9 months after the operation.In 15 cases with E-STA-STF-S surgery, digital subtraction angiography (DSA) showed good compensatory blood supply of external carotid artery to brain and relieved symptoms in 12 patients and no obvious compensation was found in another 3 cases. In 7 cases with STA-MCA surgery, no obvious compensation was found 1 case. After the review of DSA, the compensatory blood supply rate was 81.82% and symptoms were improved in all the cases.Conclusions Surgical treatment can effectively improve the clinical symptoms in adult patients with IMMD. To develop individualized surgical plan is an important clinical measure for IMMD treatment in adult patients.

Objective To explore the correlation between MMP1 gene polymorphisms and risk of oral squamous cell carcinomas among Han nationality in Yunnan province and to provide experimental reference for discovery and screening of susceptibility genes of oral squamous carcinoma.Methods Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) was applied to detect individual MMP1 promoter-1607 1G/2G loci single nucleotide polymorphisms (SNP) in patients with oral squamous cell carcinomas and control group. The genotype of the patients with oral squamous carcinoma was obtained and compared with that in the control group. Results The genotype frequencies of MMP1 promoter-1607 1G/2G, 1G/1G, 2G/2G, 1G/2G were 11.11% and37.04%, 51.85% and 5.66%, 43.40% and 50.94% in control group and case group respectively. There was no significant difference between MMP1 SNP and the risk of oral squamous cell carcinomas (P>0.05) . No significant difference was found when MMP1 SNP was compared with high, medium and low differentiation of oral squamous cell carcinomas (P>0.05) . Conclusion There is no correlation between MMP1 promoter-1607 1G/2G loci SNP and the risk of oral squamous carcinoma among Han nationality in Yunnan province.

Methods A total of 1200 cases were selected from the department of gynecology in the Second Affiliated Hospital of Kunming Medical University and were divided into four groups: no“ erosion” group (group A) , mild“ erosion” group (group B) , moderate erosion“ group (group C) and severe” erosion "group (group D) . Liquid-based cytology test (LCT) and the Hybrid Capture II (HC-2) detection were applied on all the patients. Every positive colposcopic biopsy underwent pathologic examination. LCT and HC2 results and constituent ratios of CIN and cervical cancer were compared. The relationship between age and ectopic cervical columnar epithelium were analyzed.Results There was significant difference when the presence and absence of cervical erosion and the degree of cervical columnar epithelium ectopy among all age groups were compared (P<0.01) . There was no significant difference when the LCT the HC2 results and the prevalence of CIN and cervical cancer were compared among all age groups (P >0.05) . Conclusions There is no positive connection between cervical erosion and cervical lesions. Cervical columnar epithelium ectopy should not be regarded as a risk factor of cervical lesion.

Abstract:
Objective To analyze the therapeutic efficacy of chemoradiotherapy combined with recombinant human vascular endostatin (endostar) in the treatment of locally advanced head and neck squamous cell carcinoma.Methods From July 2011 to June 2014, data of the patients with locally advanced head and neck squamous cell carcinomas were collected, including 46 patients with chemoradiotherapy combined with endostar and 45 patients with chemoradiotherapy only and the two groups were divided: endostar groups (46 patients) and control group (45 patients) . Objective response rates, disease control rate, overall survival (OS) , and progress free survival (PFS) were compared. Quality of Life Questionnaire from University of Washington was applied to evaluate the life quality of the two groups. Results The objective response rate of endostar group and control group were 87.0% and 75.6% respectively (χ2=1.946, P>0.05) , disease control rate being 97.8% and 95.6% respectively (χ2=0.368, P>0.05) , the median PFS being 18.7 months and 14.7 months respectively (P<0.05) , and median OS being 26.8 months and 23.6 months respectively (P>0.05) . The adverse reaction rate were similar between the two groups. The scores of quality of life, including mood and anxiety domain and total score, in endostar group were higher than those in control group (P <0.05) . Conclusion Endostar combined with chemoradiotherapy can enhance the therapeutic effect of locally advanced head and neck squamous cell carcinoma, prolong free survival progress, improve the quality of life and does not increase adverse reaction rate. But the survival benefit for patients still needs further study.

Abstract:
Objectives To investigate the effects of anti-nuclear antibody (ANA) and nine anti-ENA antibodies (Ro-52, SSA, SSB, Jo-1, RNP/Sm, Sm, Scl-70, Sm, Scl-70, PM-Scl, ds DNA) on the detection of syphilis antibody in pregnant women. Methods A total of 253 cases was recruited, with 138 anti-nuclear antibody positive pregnant women (experimental group 1) , 55 anti-nuclear antibody negative pregnant women (experimental group 2) and 60 healthy non-pregnant women (healthy control group) and serum TP-Ab was detected with ELISA as screening method. The positive incidents of TP-Ab of ANA and its nine anti-ENA antibody were confirmed by TPPA method. Chi-square test was used to compared positive rate. Results There were 12 cases of TP-Ab positive in the experimental group 1, accounting for 8.70% (12/138) , which was significantly higher than that in the healthy control group (P<0.05) , while there was no significant difference (P>0.05) between experimental group 2 and control group, with TP-Ab rate of 1.82% (1/55) and zero re.positive antibody TP-Ab of Anti-ENV antibody were only detected in Ro-52 and SSA, which of positive incidence accounted for 14.71% (10/68) and 5.40% (2/37) respectively, there was significant difference between the two groups (P<0.05) . The results in 253 cases were totally confirmed negative through TPPA method. Conclusions ANA can interfere with the detection of TP-Ab, which caused false positive results, especially the risk of false positivity caused by Ro-52 is higher.

Abstract:
Objective To evaluate whether antineoplastic drugs are rationally used, in order to improve the ffectiveness and safety of the clinical use of antineoplastic drugs. Methods A total of 240 medical records of hospital patiens were collected by equal number of random sampling every quarter from January 2015 to December 2016. The records were reviewed and evaluated on the usage of antineoplastic drugs against national guidelines, including drug indications, drug choice, usage and dose, solvent selection suitability, order of medication, whether chemotherapy was clear or not, whether chemotherapy was performed as scheduled, whether antineoplastic drugs were managed by classification, and whether adverse reactions were recorded along with the patient's progress.Results 120 out of 240 records had unreasonable drug use, accounting for 50% of the total records. Ratio of unreasonable antineoplastic drugs utilization in each quarter was 65%, 60%, 50%, 60%, 50%, 44%, 50% and 35%, respectively. Conclusion Clinical pharmacists should actively strengthen the review and evaluation of antineoplastic drugs' clinical use and provide evidence for rational use of drugs to physicians, so as to reasonable, effective and safe drug use.

Abstract:
Objectives To analyze the influence of female basal testosterone (bT) level of infertile women with normal serum sex hormones on the pregnancy outcome of in vitro fertilization embryo transfer (IVF-ET) and to evaluate the predictive value of bT on pregnancy outcome and its clinical significance. Methods A total of 229 cases of infertile women with normal serum sex hormones were selected and divided into pregnancy group (group A, n=114) and non-pregnancy group (group B, n=115) . The bT levels of the two groups were compared.The influence of bT value on IVF-ET pregnancy outcome was explored. Receiver operating characteristic (ROC) was analyzed according to the differences of bT levels of all the patients to find the bT threshold. Based on the bT levels, the patients were divided into group C (bT≤bT threshold, n=129) and group D (bT>bT threshold, n=100) . The relationship between bT value and IVF-ET pregnancy outcome was further explored. Results (1) The bT levels, MⅡ egg rate and high quality embryo rate in group A were significantly higher than those in group with B [ (0.94 ±0.34) , (0.63 ±0.32) ], [ (1208/1419) 85.13%, (906/1168) 77.57% ], [ (432/828) 52.17%, (319/701) 45.51%) ] (P<0.05) . (2) There was a moderate predictive value of bT on pregnancy outcome (sensitivity: 51.8%, specificity: 35.7%) . The best bT threshold was 0.72 nmol/L. (3) MⅡ egg rate and pregnancy rate in group D were significantly higher than those in group C [ (1078/1173) 91.9%, (1036/1414) 73.27%], [ (59/100) 59 % (55/129) 42.64%] (P<0.05) . Conclusions The bT levels of infertile women with normal sex hormone levels may preliminary predict and assess the outcome of IVF-ET. For the patients with bT level under the threshold, exogenous androgen supplements may be considered to increase to raise the bT level to a fine level therefore improve IVF pregnancy outcome.

Objective To explore the expression and significance of glucose-regulated protein 94 in cervical lesions and HPV infection. Methods Patients from the department of gynecology of the second affiliated hospital of Kunming Medical University form October 2014 to June 2016 were chosen to accept cervical cancer screening (HPV and TCT) , and if any result was abnormal, biopsy under colposcope was performed and the cervical tissue was collected. Finally 40 cases with normal cervical tissues, 30 cases with CINI tissues and 30 cases with CINII-III tissues were selected according to pathological findings, with the age range from 25 to 55 years old. The patients who had ever done cervical lesions treatment or was pregnant were not included in the study. Western Blot method was applied to detect the expression of GRP94 protein in normal cervical tissues, CINI and CINII-III. Results The negative and positive expression of GRP94 accounted for 65.0 % and 35.0 % respectively in normal cervical tissues, 40. 0% and 60. 0% respectively in CINI tissues, and 26.6 % and 73.3 % respectively in CINII-III tissues. There was significant difference among the overall positive rates of the three groups. Pairwise comparison showed positive rate of CINII-III group was significantly higher than that of the normal group and no significant difference was found between the positive rate of the CINI group and that of the normal group and that of CINII-III group. Conclusion GRP 94 shows high expression in cervical intraepithelial neoplasias, on the contrary shows lower expression in normal cervical tissue. GRP94 is closely related to the occurrence of cervical lesion.

Objective To investigate the expression of Survivin in circulating tumor cells (CTC) and the association between CTC and clinicopathologic characteristics of bladder cancer. Methods Clinically confirmed 50 bladder cancer patients were enrolled in the study. Can Patrol technology was applied to determine different types of CTC. And expressions of Survivin in these CTC were detected by immunohistochemical procedures. C2 test and multi-variates Logistic regression were employed to determine the association between Survivin in CTC and clinicopathologic characteristics of tumors. Results The positive rate of CTC in bladder cancer was 42% (21/50) .The mesenchymal type CTC had the most expression of Survivin (90.9%, with P<0.05) . The presence of CTC and positive expression of Survivin were correlated with tumor size, number of tumor, pathological stage, and pathological grade (P<0.05) . Multivariate analysis confirmed that the presences of CTC and positive expression of Survivin were significant correlated with tumor size, pathological stage, and pathological grade (P <0.05) .Conclusion The presence of CTC and expression of Survivin may suggest the re-occurrence, progression and metastasis of bladder tumor.

Objective To explore the influencing factors of hyperlipidemia in elderly people of Yi Ethnic Peoplein Kunming City.Methods A total of 168 Yi Ethnic People were randomlyselected the study in Kunming.Questionnaire survey were performed to collect data on general conditions, personal signs, diet and reproductive history of females.Blood samples are taken to measure triglyceride and blood glucose level, and bone mineral density ( BMD) is examined by quantitative ultrasound instrument ( QUS) .X2test and binary logistic regressionwere employed to analyze data.Results The prevalence of hyperlipidemia in elderly Yi Ethnic People in Kunming was 69.6%. Binary logistic regression analysis showed that high blood pressure, osteoporosis, high weight and smoking were risk factors of hyperlipidemia in male (OR=18.652, 2.112, 1.100, and 7.864) , high body weight and hormone intake were risk factors of hyperlipidemia in female (OR =1.104, 15.969) . Height is a protective factor for hyperlipidemia in both male and female (male OR=0.852, female OR=0.915) .Conclusion The prevalence of hyperlipidemia is higher in elderly Yi Ethnic People in Kunming City.High blood pressure, osteoporosis, high weight and smoking are risk factors for hyperlipidemia in male, high body weight and hormone intake are risk factors for hyperlipidemia in female. Keeping normal body mass index could help to control people's blood lipids.

Objectives To clarify the related factors of osteoporosis in elderly patients with type 2 diabetes mellitus and to provide guidance for the prevention and treatment. Methods A total of 121 elderly patients with type 2 diabetes were divided into 2 groups according to China osteoporosis guidelines:case group (n=53) and normal group (n =68) . Different types of data, gender, age, body mass index (BMI) , life activities (aerobic exercise, smoking, drinking, physical activity, diet imbalance, calcium) , blood lipid, renal function, parathyroid hormone, calcium phosphorus product, C reactive protein, diabetic complications, and medications were collected and compared. Results There were significant differences between the two groups in age, BMI, duration of diabetes, parathyroid hormone, urea nitrogen, creatinine, urinary protein, aerobic activity, calcium and insulin treatment (P < 0.05) . Conclusion Age, duration of diabetes, parathyroid and urinary protein are the risk factors of osteoporosis in elderly patients with type 2 diabetes mellitus. Active aerobic activity and intake of calcium are protective factors of osteoporosis in elderly patients with type 2 diabetes.

Objective To compare the clinical effects of maxillary ostium with fenestration of lower nasal mucosal flap in treating maxillary sinus mucosa cyst. Methods We retrospectively analyzed the medical records of 63 patients with maxillary sinus mucosal cysts from July 2014 to July 2016. They were divided into two groups to be treated by fenestration of the lower nasal mucosa flap and the maxillary ostium. The operative time, intraoperative blood loss, postoperative in-hospital stay, surgical complications, and operative efficacy were compared between the two groups. T-test and Chi-square test were employed for data analysis. Results There was no significant difference in operation time, intraoperative blood loss, postoperative in-hospital stay, operative complications and surgical outcome (P>0.05) . Swelling in the inferior turbinate mucosa occurred in 2 cases treated by fenestration of the lower nasal mucosa flap, and lesion occurred in 1 case treated with maxillary ostium group (repeated surgery) .Conclusion Fenestration of the lower nasal mucosa flap had wider range of application than maxillary ostium. The treatment of maxillary sinus mucosal cysts should choose a reasonable surgical path.

Abstract:
Objective To compare the effect of different hemostatic methods on ovarian function in laparoscopic excision of ovarian endometriotic cysts. Methods From August 2014 to August 2016, 96 patients with laparoscopic excision of ovarian endometriosis were divided into two groups according to the number of random numbers. The control group was treated with bipolar coagulation hemostasis while the observation group was treated with suture hemostasis. The operation of the two groups was analyzed, and the ovarian function and natural pregnancy rate were analyzed before and after the operation. Results There was no significant difference in the operation time, intraoperative blood loss and pain score between the two groups (P>0.05) . The incidence of postoperative bleeding rate and postoperative complication rate in the observation group were significantly lower than those in the control group (P<0.05) . The levels of FSH, E2 and P in the observation group were lower than those in the control group (P>0.05) while the LH, AFC and PSV indexes were significantly higher than those in the control group (P<0.05) The natural pregnancy rate was slightly higher than that of the control group and the difference was not statistically significant (P>0.05) . Conclusion The use of bipolar coagulation and hemostasis in the treatment of laparoscopic ovarian endometriosis can effectively reduce postoperative bleeding and complications and improve the natural pregnancy rate, but the use of suture hemostasis is worth promoting for it can retain the normal ovarian tissue and reduce the impact on ovarian function as much as possible.

Objective To study and observe the oxygenation and cardio-pulmonary function of elderly patients with surgery after using dexmedetomidine. Methods A total of 82 elderly patients with surgery in the second People's Hospital of Panzhihua City from March 2015 to November 2016 were chosen and randomly divided into control group (conventional intervention group, 41 cases) and observation group (conventional intervention and dexmedetomidine group, 41 cases) . The oxygenation and cardio-pulmonary function indexes of the two groups at different time before and after the surgery were compared. Results There was no significant difference in oxygenation and cardio-pulmonary function indexes of the two groups before the surgery (P >0.05) while the oxygenation function and cardio-pulmonary function indexes of the observation group at 6 h, 12 h and 24 h after the surgery were significant better than those of the control group (P<0.05) . There were significant differences in the detection results after the surgery between the two groups. Conclusion Dexmedetomidine is good for the oxygenation and cardio-pulmonary function of elderly patients with surgery and it can improve the physical condition of the elderly patients with surgery.

Abstract:
Objectives To investigate and analyze the changes of antibiotic use for pediatric patients with bronchial pneumonia in recent eight years and to improve the awareness and the level of pediatricians in using of antibiotics in clinical work rationally. Methods Retrospective investigation was applied on 800 cases of pediatric bronchial pneumonia in Inner Mongolia People's Hospital from 2008 to 2011 and another 800 cases of pediatric bronchial pneumonia from 2012 to 2015 to analyze the use of antibiotics. The cases and related data were collected from the medical records. The use rate and combined use rate of antibiotics, white blood cell and neutrophil levels, pathogen detection rate, hospital stay and discharge rate were analyzed. Results There was no significant difference in leukocyte and neutrophil levels and hospital discharge rate between the two groups (P>0.05) . The use rate and combined use rate of antibiotics in 2012-2015 cases were significantly lower than those in 2008-2011 cases. The pathogen detection rate in 2012-2015 cases was significantly higher than that in 2008-2011 cases and the length of hospitalization in 2012-2015 cases was significantly shorter than that in 2008-2011 cases (P<0.05) .Conclusion Pediatricians should pay more attention to the administration of antibiotics on the children with bronchial pneumonia. Based on the results of etiological detection, safe, effective and rational choice of antibiotic can be helpful to reduce the unnecessary use of antibiotics, thereby reducing unnecessary damage to children.

Objective To measure the effect of case-based learning (CBL) method in medical English teaching and explore its the feasibility. Methods 127 fourth-grade undergraduate students, majoring in clinical medicine, were randomly allocated into two groups: group A (n=67) taught in traditional methods and group B (n=60) taught in CBL methods. At the completion of teaching, examination and satisfaction questionnaire were applied to measure the outcome of teaching in two groups. T-test and Chi-square test were employed in data analysis.Results The examination scores of group B was higher than that of group A (P<0.05) . The degree of satisfaction of group B was higher than that of group A (P<0.05) . Conclusion CBL teaching methods can improve the quality of medical English teaching and students' satisfaction.

Objective To evaluate the clinical effects of comprehensive nursing care on the complications, postoperative recovery, quality of life and satisfaction in patients with colon cancer in perioperative period. Methods Patients (n=40) were divided into observation group and control group according to nursing methods. Patients in control group were treated with routine nursing and the observation group received comprehensive nursing intervention on the basis of routine nursing. The incidence of complication, postoperative recovery, quality of life and satisfaction were compared between the two groups. Results The incidence of complication in the observation group was significantly lower than that of the control group (P<0.05) . The first exhaust time, first postoperative defecation time, postoperative off-bed activity time and average hospital stay time in the observation group were significantly lower than those in the control group (P<0.05) . The Gastrointestinal Quality of Life Index (GIQLI) scores in the observation group were significantly higher than those in the control group in week 1, week 2 and week 4 after the operation (P<0.05) . The satisfaction rate and satisfaction score in the observation group were significantly higher than those in the control group (P<0.05) . Conclusion Comprehensive nursing care can significantly reduce the incidence of complications, speed up the recovery of patients, reduce hospitalization time, and improve the quality of life and the satisfaction of patients with colon cancer. It is worthy to be extensively applied.

Objective To investigate the clinical effects of cluster nursing in mechanical ventilation treatment of HIV infection complicated by pulmonary tuberculosis. Methods A total of 68 patients with HIV infection complicated with pulmonary tuberculosis were treated with mechanical ventilation and randomly divided into observation group and control group. The control group received routine nursing while the observation group received cluster nursing based on routine nursing. Mortality rate, mechanical ventilation time, the incidence of ventilator-associated pneumonia (VAP) and incidence of pneumothorax were observed. Results The mortality rate in the observation group was 26.47% while it was 52.94% in the control group. The mechanical ventilation time in the observation group was 8.94 d while it was 13.35 d in the control group. The incidence of VAP in observation group was 8.82% while it was 23.53% in the control group. The incidence of pneumothorax in observation group was 2.94% while it was 14.70% in the control group. Conclusion Cluster nursing can effectively reduce the mortality rate, the incidence of VAP and incidence of pneumothorax, cut the mechanical ventilation time and improve the prognosis of the patients.
