Abstract:
Objective To study the bibenzyls and phenanthrenes from Arundina graminifolia.Methods The compounds were extracted by 95% alcohol and isolated by column chromatography on silica gel and Sephadex LH-20.Their structures were identified by spectroscopic analysis (1 H NMR and13 CNMR) .Results Eleven compouds were obtained and identified as batatasin III (1) , arundinanin (2) , 2, 8-dihydroxy-4, 7-dimethoxy-9, 10-dihydrophenanthrene (3) , shancidin (4) , arundinan (5) , isoshancidin (6) , erianthridin (7) , lusianthridin (8) , eulophiol (9) , flavanthrin (10) , orchinol (11) .Conclus ion Compounds 3, 7, 9 were isolated from this plant for the first time.

Abstract:
Objective To explore the intereaction between hypermethylation and non-coding RNAs related to Hepatitis B virus X protein (HBx) .hypermethylation.Methods miRNA microarray and quantitative reverse-transcription polymerase chain reactions (qRT-PCRs) were performed to confirm that HBx suppressed miR-338 expression in HCC cells.Protein, mRNA, miRNA expression analyses and promotor methylation detections were performed to delineate the mechanisms of HBx/HBV associated miR-338 repression in HCC.Results HBx suppressed miR-338 expression in HCC tissues and cells, while upregulating expression of DNMT1 and DNMT3A.The promotor methlation status of miR-338 was significantly higher in HBV+HCC tumors than adjacent tissues, while demethylation treatment could rescue the miR-338 expression in HBx+HCC cells.Conclus ions HBx can downregulate the expression of miR-338, while upregulate DNMT 3 A and DNMT 1 expression.The HBx associated promotor hypermethylation of miR-338 is responsible for its silence.

Objective To investigate the effect of Roux-en-Y gastric bypass on intestinal thick-walled bacteria and APPL1.Methods The effect of postoperative and postoperative fecal transplantation on the metabolic index of rats was observed by Roux-en-Y gastric bypass.The thick-walled bacteria and APPL1 expression were detected by Western blot and qPCR.Results The body weight of the rats in the S+T group was significantly higher than that in the S group (P<0.05) .The body weight of the rats in the S+T group was significantly higher than that in the control group (P<0.05) The expression of TG in S+T group was significantly higher than that in S group (P<0.05) .The expression of TG in S group was significantly lower than that in group C (P<0.05) The expression of TCH in the T group was significantly higher than that in the S group (P<0.05) .The expression of TCH in the S group was significantly higher than that in the S group (P<0.05) The expression of FINS in S group was significantly higher than that in group C (P<0.05) .The expression of FINS in S group was significantly higher than that in group S (P<0.05) .The expression of ISI in S group was significantly higher than that in group S (P<0.05) .The expression of ISI in S group was significantly higher than that in group S (P<0.05) .The expression of S thick-walled bacteria in S was significantly lower than that in group C (P <0.05) .Western blot and mRNA detection showed that the expression of APPL1 in S group was significantly higher than that in group C (P<0.01) .The expression of APPL1 in S+T group was significantly lower than that in S group (P <0.05) .Conclus ion Roux-en-Y gastric bypass may be mediated by affecting the expression of thick-walled flora.

Objective To observe the effects of Osteoking on serum estradiol and bone metabolism index of tree shrew with osteoporotic fracture.Methods Tree shrews with osteoporotic fracture were randomly divided into treatment group and control group.The tree shrews in treatment group were given 0.3 mL of Osteoking by 100 g body weight every two days for 90 days, while the tree shrews in the control group were given the same dose of saline.The serum samples were collected from the heart.The contents of E2, OC, PINP, CTX and TRACP-5 b were measured by ELISA kit.Results The serum E2 content of tree shrews in the treatment group was significantly higher than the control group (145.16±21.76 vs 30.78±4.22) (P<0.05) , the serum OC (1546.67±219.07) , PINP (10.02±1.76) , CTX (176.47±24.56) , TRACP-5 b (1249.06±194.87) in the treatment group were significantly lower than the control group (2792.96±387.23) and (28.74±4.05) , (324.67±41.32) , (2106.36±322.99) (P<0.05) .Conclusion Osteoking can cause changes in estradiol and bone metabolism index of tree shrews with osteoporotic fracture, indicating it has a certain effect

Objective To analyze volatile components from ten different origin in fruits Foeniculum vulgare Mill, and detection of their relative contents.Methods The volatile oil was extracted by solid phase microextraction and analyzed by gas chromatography-mass spectrometry.Results Among the volatile of ten samples, 68 compounds were identified.9 compounds were same and other 59 compounds were different.But trans-anethole, methylchavicol and p-methoxybenzaldehyde were the main compositoin and the contents of them were above 80%.Conclusions The composition of the volatile oil from different regions are different.And this study provides a theoretical basis for the rational exploitation of Thymus mongolicus.

Abstract:
Objective To study the impact of quercetin on tumor recurrence after hepatectomy in athymic mouse hepatoma and the mechanism.Methods We established high-recurrent athymic mouse model simulating post-operation human HCC.For experimental group, we administered the mice with quercetin solution 75 mg/ (kg.d) by gavage, and observed tumor recurrence of the mice.Then we detected the expression of MMP-2, TIMP-2 in recurrent tumor using Western-blot.Results Compared with control group, mice in quercetin group had smaller liver recurrent focus volumn [ (25.49±9.78) mm3 VS (49.75±7.53) mm3, P<0.05) ], less liver recurrent focus[ (1.00±0.71) vs (2.20±0.84) , P<0.05].Western-blot was used to detect the expression of P65, MMP-2 and E-Cadherin.Compared with control group, the P65 and MMP-2 expression of the recurrent tumor in quercetin group decreased [ (170.07±6.61) vs (156.26±8.60) , P<0.05], [ (169.21±5.32) vs (158.17±4.80) , P<0.05], and the E-Cadherin expression in quercetin group increased [ (136.24±3.03) vs (144.70±5.02) , P<0.05].Conclusions Quercetin could restrain tumor recurrence in athymic mice after hepatectomy.The mechanism may be that quercetin could decrease the expression of P65 and MMP-2 and increase the expression of E-Cadherin in recurrent tumor.

Objective To study the role and molecular mechanism of miR-4328 in prostate cancer.Methods CCK-8 method was used to detect the proliferation of prostate cancer cells, and Transwell method was used to detect the migration of prostate cancer cells, and Real-time PCR and Western blotting method were used to detect the epithelial mesenchymal transition markers and the expression of ZEB1.Results The expression level of miR-4328 in prostate cancer tissues was significantly lower than that in paracancerous tissues.Overexpression of miR-4328 in prostate cancer cells significantly inhibited the cell proliferation (P<0.001) .And overexpression of miR-4328 also significantly inhibited the migration of prostate cancer cells (P<0.001) .And overexpression of miR-4328 up-regulated the epithelial cell marker E-cadherin, and down-regulated the mesenchymal cell markers including N-cadherin and Vimentin (P <0.001) transition of prostate cancer cells.Further studies found that overexpression of miR-4328 could down regulate the expression level of epithelial mesenchymal transition related transcription factor ZEB1 (P<0.001) .Conclusion Overexpression of miR-4328 can inhibit the proliferation, migration and epithelial mesenchymal transition of prostate cancer cells.

Objective To investigate the effect of left ventricular reconstruction on left ventricular remolding.Methods We selected twelve common dogs as experimental animals.We ligated the aeteria coronaria and established the model of ventricular aneurysm.We seleced the normal cardiac muscle as common group.After modelling and evaluating, the dogs of successful modelling were divided into two groups.The experimental group was rebuilt by the operation of left ventricular reconstruction (linear closure operation) .The control group was disclosed chest, yet it was not rebuilt by the operation of left ventricular (linear closure operation) ;after six weeks of the operation of left ventricular reconstruction, we obtained the cardiac muscle of nonoperating field in every group.Then we detected the content of AT1R mRNA, AT2Rm RNA of the cardiac muscle of every group by RT-PCR and observed ultrastructure of the cardiac muscle of every group by electron microscope.Results Compared with normal group, the expression of AT2R mRNA was significantly higher (P<0.05) , the expression of AT1R mRNA was significantly lower (P<0.05) , and AT2R mRNA/AT1R mRNA ratio was significantly higher (P<0.05) in experimental group and the control group.Compared with control group, the expression of AT2R mRNAwas significantly lower (P<0.05) , the expression of AT1R mRNA was significantly higher (P<0.05) , and AT2R mRNA/AT1R mRNA ratio was significantly lower (P<0.05) in experimental group;Compared with the common group, ventricular remolding (collagen protein obviously deposited in the mesenchyme of cardiac muscle, collagen fibers became obviously larger) and cardiac damage (the gap of myofibrilla became larger in myocardial cell, cytoplasm dropsy, the part of mitochondria cristas was broken) ultramicrostructure took place in the cardiac muscle of the experimental group and the control group;compared with the control group, the extent of ventricular remolding and Cardiac damage of experimental group was less than the control group in the electron microscope.Conclusion Left ventricular reconstruction (linear closure operation) can delay the process of left ventricular remolding to a certain extent, lower the extent of cardiac damage and help to prevent the development of ventricular remolding and improve the prognosis of ventricular remolding.

Objective To study the effect of dexmedetomidine on brain edema in rats with severe traumatic brain injury (TBI) .Methods SD rat traumatic brain injury model was established and was treated with dexmedetomidine.The changes of neurological impairment, brain water content and BBB permeability were observed within 72 hours after injury.Results The NSS score of different TBI rats in different groups:the difference of internal effect was statistically significant (P=0.041) , and the difference between subjects was statistically significant (P<0.001) .The water content of the brain tissue in different groups of TBI rats was statistically significant (P=0.032) , and the difference between subjects was statistically significant (P<0.001) .The correlation was found between neurological score and cerebral edema degree (r=0.61, P<0.001) .Conclusions TBI rat cerebral edema peaks at 24 h after TBI.Dexmedetomidine can reduce BBB permeability and reduce the edema of TBI rats in the early stage of injury, thereby promoting the recovery of nerve function.

Objective To study the senile proximal humeral comminuted fracture, the method curative effect and indications of artificial humerus head replacement.Methods From March 2007 to August 2013, a total of 17 cases, according to Neer classified three or four parts of proximal humeral fracture and dislocation patients Ⅰperiod artificial humeral head replacement, aged 64-79, the average age of 70.2 years, surgical approach all USES the deltoid muscle pectoralis major clearance into the road, according to preoperative and intraoperative situation patients were given individualized rehabilitation plan line functional recovery.Results 17 cases were followed up for 8-29 months, an average of 14.7 months, there was no infection, no nerve injury, no dislocation, no prosthesis loosening, sinking, 1 case of heterotopic ossification, according to Neer criteria:6 cases were excellent, 8 cases were good, 3 cases fwere easible, the excellence and good rate was 82.3%.Conclusion The application of artificial humerus head replacement in the treatment of senile proximal humeral comminuted fracture has good curative effect, but preoperative assessment, intraoperative reconstruction of cuhk nodules and rotator cuff, postoperative rehabilitation training of individuation are important factors of functional recovery.

Objective To study the composition of the placenta microbiota of normal term delivery.Method Three placenta specimens were collected from full-term infant birth by normal spontaneous delivery.The flora DNA was extracted by Tissue Kit for 16SrRNA gene high-throughput nucleotide sequencing, then the sequencing results were analyzed.Results The placenta microbiota of normal spontaneous delivery at the bacterial phylum level showed five bacterial phylum were detected, including Proteobacteria, Unclassified Bacteria, Actinobacteria, Firmicutes and Bacteroidetes.Proteobacteria was the main parts of bacteria and more than 90 percent in the placenta microbiota.At the genus level, 24 bacterial genus were detected including Proteobacteria, Actinobacteria and Firmicutes.Halomona, Pelagibacterium and Unclassified Phyllobacteriacea of Proteobacteria genus was the main bacterial genus in the placenta microbiota.Actinotalea, Escherichia/Shigella, Sphingomonas, Bacteroides, Streptococcus, Klebsiella, Bifidobacterium, Clostridium and Staphylococcus.Conclusion There are abundant bacterial flora colonized in the normal term placenta.

Objective To explore the application of carbon nano negative parathyroid imaging in radical operation of thyroid carcinoma.Methods 130 cases of thyroid cancer were randomly divided into experimental group and control group, each group had 65 cases, The experimental group was injected with nano carbon suspension to clean the ipsilateral VI lymph nodes according to the range of black stained lymph nodes, patients in control group were treated with VI lymph node dissection.Of the two groups of total lymph nodes, the number of lymph node metastasis, inadvertent parathyroidectomy, a transient hypocalcemia and permanent hypocalcemia cases, lymph node metastasis rate and thyroid resection rate were analyzed.Result In the experimental group, 406 lymph nodes were dissected, and the control group was given a total of 267 lymph nodes.The number of lymph node metastasis in the experimental group was 106, while the control group was 63, the difference between the two groups was statistically significant (P<0.01) .The lymph node metastasis rate of the experimental group and the control group were 26.11% and 23.61%, respectively.There was no significant difference between the two groups (P>0.05) .The experimental group did not find the parathyroid gland, transient hypocalcemia in 2 cases, no permanent hypocalcemia, find the parathyroid tissue after surgery in the control group of 4 cases, 12 cases of transient hypocalcemia, 4 cases of permanent hypocalcemia, differences between the two groups had statistical significance (P<0.05) .The experimental group and the control group were 0% and 1.08%, respectively, the difference between the two groups was statistically significant (P<0.05) .Conclusion Nano carbon in the radical operation of thyroid cancer can make the VI region lymph node is very good imaging, but the parathyroid is negative imaging, It is helpful for the dissection of lymph nodes, avoiding the wrong cutting of parathyroid gland, and improving the accuracy and safety of operation.

Objective To evaluate the clinical efficacy and safety of Flexible ureterorenoscopy in the treatment of functional solitary kindey stones, and evaluate the value of Flexible ureterorenoscopy in minimally invasive treatment of functional solitary kidney stones.Methods The clinical data of 24 patients with functional solitary kidney stones treated with Flexible ureterorenoscopy holmium laser lithotripsy in our hospital were analyzed retrospectively.All patients were 19 males and 5 females.Kidney stones in the function of multiple kidney stones at least one stone.Patients with functional kidney stones, patients under general anesthesia, preoperative indwelling ureter double J tube after 2 weeks, with soft ureteral function of multiple renal calculi.After the end of surgery conventional indwelling double J tube.All patients were followed up for 1 month.CT, KUB and renal function were reviewed and clinical therapeutic effect was evaluated.Results All the patients' operations ware completed successfully.No stone rate reached 92 %.A single operation time was from 59 to 110 minutes, with an average of (74±19) minutes.One male patients appeared septic shock, there was no serious complications in other patients, combining with follow-up after renal function.Postoperative renal function of patients was normal or improved than before.Postoperative hospital stay was 2 to 11 days.Conclusions Flexible ureterorenoscopy lithotripsy is a safe and effective method for the treatment of functional solitary kidney stones.It can protect renal function and has clinical application value

Objective To observe the number and variation of root and canal of mandibular first molar in Yunnan province by using cone-beam computed tomography (CBCT) , and to provide anatomical experimental basis for pulp treatment.Methods In this study, Cone-beam CT images were collected from 300 patients with a total of 600 mandibular first molars from imaging department of Yunnan Provincial Stomatological Hospital.The number of root and canals and the occurrence of distolingual root and the morphology of root canal were analyzed statistically.The root canal configurations were classified according to the method of Vertucci.Results CBCT image data of 600 mandibular first molars were obtained.The incidence of two-root was 71.2% (427/600) , and three-root was28.8% (173/600) .Two canals were found in 2.1% of mandibular first molars and 60.7% had three canals and36.8% had four canals.Most mesial roots were type IV configuration and most distal roots had a simple type I configuration and 100% of distolingual root was type Ⅰ.The incidence of distolingual root of population was 33.7% (101/300) .There was no significant difference between males and females (P>0.05) .71.3% (72/101) occurred bilaterally.The incidence of the right side of distolingual root was significantly higher than that on the left side (P<0.05) .Conclus ion The incidence of distolingual root is higher in the mandibular first permanent molar in Yunnan, and the incidence of right side is significantly higher than that of the left side. CBCT is easy to study the root canal, which can assist thetreatment of pulp and improve the success rate of root canal therapy.

Objective To analyze the clinical characteristics of dengue fever accompanied withthrombocytopenia, in order to enhance the clinician's further understanding of the disease.Methods The 64 cases of dengue fever treated in the third people's Hospital of Kunming during June 2012 to January 2016 were divided into two groups with thrombocytopenia and without thrombocytopenia.The clinical and laboratory data were compared and analyzed.Results Between patients with and without thrombocytopenia, the gender, age and the highest temperature had no significant difference (P >0.05) ;the duration of fever had statistically significant difference (P<0.05) ;The clinical manifestations, subcutaneous bleeding, gastrointestinal bleeding, nasal hemorrhage had statistically significant difference between two groups of patients (P >0.05) .There were statistically significant differences in the laboratory results in WBC, NE, PLT, AST and LDH levels between two groups of patients (P<0.05) .in the thrombocytopenia group, 26 cases (74.29%) had liver injury, and 17 cases (58.62%) had liver damage in the non thrombocytopenia group.The difference of AST/ALT ratio between the 2 groups was statistically significant (P <0.05) .Con clusions Early stage of dengue fever accompanied by thrombocytopenia is prone to lead to bleeding and tissue and organ damage, so it should be paid attention to early in the disease, so as to prevent the progression of hemorrhagic fever or shock syndrome.

Abstract:
Objective To evaluate the current related laboratory indicators of rheumatoid arthritis (RA) and screening out the optimal early RA-related indicators.Methods We selected 158 cases as RA group, 121 cases as early RA group, 100 cases as normal control group.The total rheumatoid factor and other 14 indicators were measured and analyzed statistically in each group.No statistical differences indicators were observed, the ROC curve was used to evaluate the diagnostic efficacy of the remaining indicators in early RA, AUC>0.75 as the standard, indicators with diagnostic efficacy were initially screened.The sieve out indicators were used for bivariate correlation analysis, significant correlates are classified as a same class.Results (1) Among the 14 indicators, except for RF IgA and CRP, the other indexes were significantly different in the three groups (P<0.05) .There were 6 indexes which exceeding 0.75 of area under the single index curve (AUC) :GPI (0.929) , anti-MCV (0.923) , RF IgG (0.904) , anti-CCP (0.901) , APF (0.891) and total RF (0.880) ; (3) By bivariate correlation analysis, there was a significant positive correlation between the six indexes and whether they were early RA, and classified as 3 groups:GPI group, anti-MCV group (including anti-CCP and APF) , RF IgG group (including total RF) .Conclusion In the 14 indicators, only 6 indicators (GPI class, anti-MCV class and RF IgG class) have higher diagnostic efficacy for early RA, we only need to choose one to carry out in each category in practice, so as to avoid clinical and laboratory duplication or low-performance testing.

Objective To observe the effect, toleration and safety of metoprolol in treatment of coronary heart disease angina.Methods Ninety-eight patients with confirmed coronary heart disease were randomly divided into two groups:control group and metoprolol treatment group.Patients in the metoprolol treatment group were given metoprolol sustained release tablet treatment on the basis of regular drug treatment, and the dosage of metoprolol was increased gradually to the maximum tolerated dose.Patients in the control group were given regular drug treatment.All patients were followed up for half a year.The heart rate, blood pressure, the frequency of the angina attack, cardiac function and ECG were observed to compare the clinical effect between two groups.Results The clinical effect in the metoprolol treatment group was significantly better than the control group.There were statistically significant difference between control group and metoprolol treatment group (P<0.05) .Conclusion Enough metoprolol sustained release tablets can significantly improve the clinical symptoms of coronary heart disease.

Abstract:
Objective To study the influence of family members' early rehabilitation nursing on short-term prognosis of patients with dysphagia after stroke.Methods We selected 40 patients from August 2016 to December 2016 in our hospital as control group and 41 patients from January 2017 to May 2017 in our hospital as research group for intervention study.The control group was treated by routine early rehabilitation nursing, while the intervention group was given by family members' participation in the whole process of conventional early rehabilitation nursing.Results The dysphagia scores of the two groups were significantly higher than before the intervention, and the intervention group was better than the control group (P<0.05) .The incidence of aspiration pneumonia and malnutrition in the intervention group was lower than the control group (P<0.05) .The length of stay in the intervention group was less than the control group (P<0.05) .Conclusion Family members' participation in early rehabilitation nursing can improve the swallowing function of patients, reduce the incidence of complications and the length of stay.It has a great clinical significance.
Objective To study the influence of family members' early rehabilitation nursing on short-term prognosis of patients with dysphagia after stroke.Methods We selected 40 patients from August 2016 to December 2016 in our hospital as control group and 41 patients from January 2017 to May 2017 in our hospital as research group for intervention study.The control group was treated by routine early rehabilitation nursing, while the intervention group was given by family members' participation in the whole process of conventional early rehabilitation nursing.Results The dysphagia scores of the two groups were significantly higher than before the intervention, and the intervention group was better than the control group (P<0.05) .The incidence of aspiration pneumonia and malnutrition in the intervention group was lower than the control group (P<0.05) .The length of stay in the intervention group was less than the control group (P<0.05) .Conclusion Family members' participation in early rehabilitation nursing can improve the swallowing function of patients, reduce the incidence of complications and the length of stay.It has a great clinical significance.

Objective To discuss the effect of nursing intervention based on fast track surgery (FTS) concept on the postoperative recovery of patients with plantar toe joint arthroplasty (PTJA) .Methods Eighty patients with PTJA were selected from December 2013 to December 2016 in our Hospital, according to the intervention method, all patients were divided into normal group and fast group, 40 cases in each group.Patients in the normal group were given routine nursing intervention, patients in the fast group were additionally given nursing intervention based on FTS concept.Then we statistically analyzed the incision healing time, pain relief time, hospitalization time, complication and the before and after intervention pain visual analogue scale (VAS) , the American association of orthopaedic surgery ankle foot after ankle-scoring system (AOFAS) scores of all patients.Results Theincision healing time, pain relief time, hospitalization time, complication rate of fast group were significantly lower than those of normal group, the difference was statistically significant (P<0.05) .The VAS score after intervention of fast group was significantly lower than that of normal group, the AOFAS score was significantly higher than that of normal group, the before and after intervention VAS, AOFAS score change value of fast group were significantly higher than that of normal group, the difference was statistically significant (P<0.05) .Conclus ions Nursing intervention based on fast track surgery concept can effectively reduce the complications in patients with PTJA, it is beneficial to patients with postoperative body and feet functional recovery, it is worth for further clinical promotion.
