Abstract:
Objective To investigate the the effects of geraniin on modulating the mRNA expression of corebinding factor α 1 (Cbfα 1) . Methods The m RNA expressions of Cbfα 1 were detected by real time fluorescence quantitative PCR. Results As compared with the sham group, the expressions of Cbfα 1 mRNA were markedly suppressed in the model group, 12 and 24 w after OPF (P < 0.05) . Compared with the model group, geraniin significantly increased mRNA expressions of Cbfα 1. Conclusion Geraniin can upregulate Cbfα 1 gene expression and improve the treatment for OPF.

Abstract:
Objective To investigate the functional recovery and the effect of CNTF antibody block to the bone marrow mesenchymal stem cells (BMSCs) transplantated by Abdominal aortic on the downstream signaling pathways STAT3/Caspase-9 in spinal cord ischemic reperfusion injury in rats.Methods Adult female SD ratswere assigned randomly to 4 groups. The neurological functional status of the animals was assessed with BBB scores, the Motor evoked potentials (MEP) and Cortical somatosensory evoked potentials (CSEP) .The IHC were used to detect the the expressional changes of CNTF, then Western blot and RT-PCR were used to detect the expressional changes of STAT3、p-STAT3、CNTF and Caspase-9 in the ischemic segments of spinal cord.Results Compared with the sham group, in the SCIRI rats, the BBB scores were markedly decreased at all time points (P<0.01) , the latency and the amplitude of MEP and CSEP was longer and lower at 14 d post operation (P<0.01) , and this change was the most significant in the control group the second in the CNTF block group, and the last in the transplantation group, Resutts between each two groups were statistically significant (P<0.05) . At 7 d post operation, compared with the sham group, the immunoreactive products of CNTF were decreased in the CNTF block group (P<0.05) , but were increased (P<0.05) in the control group and the transplantation group (P<0.05) , and results in the transplantation group were higher than in the control group (P<0.05) . At 7 d post operation, compared with the sham group, the m RNA and protein level of CNTF、STAT3、 p-STAT3 were decreased obviously in CNTF block group (P<0.05) , the levels were increased in the control group and the transplantation group (P<0.05) , and the levels in the transplantation group were higher than that in the control group (P<0.05) ; but the m RNA and protein level of Caspase-9 were only decreased in the transplantation group (P<0.05) , the level was increased in the CNTF block group and the control groups (P<0.05) , and the level in the CNTF block group was more significantly increased than that in the control group (P<0.05) . At 14 d post operation, in CNTF block group, the m RNA and protein level of CNTF、STAT3、p-STAT3 were significantly higher than that in the sham group and the control group (P<0.05) , and the m RNA and protein level of caspase-9 was higher than that in the sham group (P<0.05) , but lower than that in the control group (P<0.05) , there were not statistically different in the level of each factor compared with transplantation group (P>0.05) . Conclusions BMSCs, transplanted by the abdominal aorta, can promote the expression of CNTF in the injuried spinal cord and significantly improve the hind limb function recovery by CNTF-mediated signaling pathway downstream of STAT3/Caspase-9 SCIRI in rats, but the role of BMSCs can be weakening by CNTF block that inhibited STAT3/Caspase-9 signaling pathway.

Abstract:
Objectives To provide theoretical basis for the clinical rational using of drugs, biochemical characteristics of the liver injuries induced by the atorvastatin, simvastatin and lovastatin were analyzed and compared by replicating the rats model of liver injuries induced by statins, Methods 80 SPF SD rats (8 weeks of age) , half male and half female, were divided into four groups randomly: control group, simvastatin group, lovastatin group, atorvastatin group. Human equivalent doses were administered to the latter three groups of rats which were sacrificed to draw blood on the 10 th, 35 th and 55 th day respectively (via the femoral artery) for testing liver function index, including total bilirubin (TBIL) , direct bilirubin (DBIL) , indirect bilirubin (IBIL) , aspartateaminotransferase (AST) , alanineaminotransferase (ALT) , alkalinephosphatase (ALP) . Results (1) The increases in TBIL, DBIL, IBIL for treatment group in comparison with control group had statistical significance (P<0.01) .2) At the 55 th day of administration, there was a significant statistical difference between the simvastatin group and the lovastatin group in AST (P<0.05) ;3) Meanwhile, there was great statistical difference between the atorvastatin group and lovastatin group in ALP (P<0.01) . Conclusion The rats in the three experimental groups suffered from minor to moderate liver injuries, most of which being cholestasis type. It is speculated that this kind of liver injuries are closely related to the obstacle of transfering bile.

Abstract:
Objective To investigate CTGF changes in the expression of the gum tissue before and after orthodontic treatment, and to preliminarily explore the modification mechanism of gingival tissue and the effectiveness of the intervention measures. Methods 72 male Sprague-Dawley rats of 12-week-old, weight about 250 g-300 g, randomly divided into 6 groups: blank control group (A) , healthy teeth orthodontic group (B) , low functional group (C) , low occlusal function teeth orthodontic group (D) , combined intervention group (E) , bite orthodontic intervention group (F) . The results of the study were compared at 1 w, 2 w, 4 w, 6 w.Results (1) HE staining results showed the atrophy of the gingival tissue, which suggested that occlusal hypofunction SD rats model were successfully established. (2) Fluorescence quantitative results of CTGF in gingival tissue: 6 w: group B was higher than group D and group F (P<0.05) . Conclusion (1) the expression of CTGF in the low occlusion group was lower than that of the normal control group with time, and the gum tissue was vulnerable to atrophy. (2) It remains to be further studied whether the bite force recovery is effective.

Abstract:
Objective To understand the molecular typing and drug resistance of Salmonella typhimurium in Yunnan Province, to provide scientific basis for preventing and controlling diseases caused by Salmonella typhimurium. Methods Molecular typing was carried out according to Salmonella PFGE typing technology published by Pulse Net China. The MIC value of the drug sensitive plate was obtained from the analysis of the instructions, and the results of S, I and R were obtained according to the corresponding standards of CLSI.Results The 68 strains salmonella typhimurium in Yunnan Province showed 53 types of PFGE. The drug resistance rate of the 68 strains of salmonella typhimurium was the highest for the compound sulfonamide (SXT) , 45.59%, and the most sensitive to imipenem (IMP) , among which one strain was resistant to 11 of the14 antibiotics. Conclusion The molecular typing of salmonella typhimuriumin in Yunnan province is diverse.Compound sulfonamide is the most resistant antibiotic of Salmonella typhimurium in this region. Multiple drug resistance of Salmonella typhimurium in this region is atypical.

Abstract:
Objective To investigate the situation of chronic diseases and the effects on quality of life among different-aged elderly in community and provide evidence for targeted improvement quality of life.Methods During March to July 2015, the elderly who was aged 60 and over as respondents from two communities in Kunming were enrolled. Cross-sectional study and cluster sampling were used based on Activities of Daily Living scale (ADL) and Balthel index.questionnaire was designed and information of chronic disease in the elderly was colected.The effects on Balthel index in different chronic diseases and age groups were analysed.Results A total of 589 subjects were investigated, chronic disease prevalence was93.70%, and the highest prevalence is hypertension that was 27.50%.The quality of life was worse in the elderly who suffered from three kinds of chronic diseases and more older. The quality of life in different age groups elderly was statistically significant (P < 0.05) .Conclusion The quality of life in the community elderly is relate with age and the type of chronic diseases.In order to improve their quality of life health guidance should be strengthened.

Abstract:
Objective To understand the degree of psychological distress of breast cancer patients beford chemotherapy, and to analyze the causes and influencing factors. Methods A cross-sectional study was conducted on 95 patients with prechemotherapy for breast cancer from September 2017 to December 2017 using a general data questionnaire and distress management screening measure. Results The score of psychological pain before chemotherapy of breast cancer was (4.92 ±2.23) , among which 69 cases with significant psychological pain accounted for 72.63% (DT score was greater than 4) . The main causes of breast cancer patients' psychological distress ranking on the top 10 are: surgical scar, recurrence worry, arm activities difficulty, daily life was disrupted, anxiety, memory loss, housekeeping difficulties, poor communication with her husband, fatigue, change the appearance. Single factor analysis showed that age, family harmony had an effect on psychological pain scores (P<0.05) .It is found that family harmony is the protective factor of paychological pain when age and family relationship are included in multifactor analysis (OR>1, P<0.05) . Conclusion There is an obvious psychological distress in breast cancer patients before chemotherapy; The age, family harmony of the patient were independent factors related to psychological distress; body symptoms, emotions and family problems are the key issues to focus on before chemotherapy.

Abstract:
Objective To explore the treatment of percutaneous coronary intervention (PCI) , and to comparatively study on the protection of side support by using the active and passive expansion technique.Method 78 patients with coronary artery disease were protected by using balloon technique, there were two groups: the active expansion group (n=41) and the side support balloon (n=37) , the TIMI classification、myocardial perfusion rating (MBG) , TIMI myocardial perfusion rating (TMP) and the incidence of marginal vascular interlayer were observed and analyzed. Results After the main support was put in, the proportion of patients with side branches of TIMI、 MBG and TMP was at level 3 and the proportion of patients with active balloon expansion was 93%, the margin expansion group was 84%, the difference was not statistically significant (P>0.05) ; the incidence of intersecting vascular interlayer in active dilatation group was 24%, the margin expansion group was 8%, and there were statistical differences (P<0.05) . Conclusions Using the technique of holding balloon to protect the side branches, both the active expansion of the side support balloon and the expansion of the side support balloon can significantly reduce the risk of the main stent placement in the posterior branch of the blood vessel, however the incidence of side branch vascular interlayer was lower.

Abstract:
Objective To investigate the clinical value and the experience of retroperitoneal laparoscopic nephrectomy in primary hospitals. Methods A retrospective analysis of 20 cases of retroperitoneal nephrectomy performed in the people's hospital of Jianshui from July 2014 to December 2017 and 23 cases of open nephrectomy in the same period as the control group, the two groups of surgery and postoperative recovery were analyzed and compared. Results The two groups of patients were operated successfully. The surgery time of the laparoscopic group was (110.14 ±15.63) min and the control group was (100.33 ±10.58) min. There was no statistical difference between the two groups (P>0.05) . No complications occurred during the follow-up period between the two groups, and the difference was not statistically significant. The intraoperative blood loss in the two groups was (40.63 ±22.21) ml and (80.45 ±38.31) m L respectively, and the difference was statistically significant (P< 0.05) .The indwelling time of drainage tube in the two groups was (2-3) days and (6-7) days respectively. The difference was statistically significant (P< 0.05) , and the hospitalization time was (3.2 ±2.2) days and (6.4 ± 3.3) days respectively, the difference was statistically significant (P<0.05) . Conclusions Laparoscopic nephrectomy has the advantages of less trauma, faster recovery and shorter hospital stay. With the improvement of operation technique, it will bring more advantages and be worth popularizing.

Abstract:
Objective To investigate the relationship between the driver genes mutation and the survival in patients with pulmonary adenocarcinoma brain metastasis. Methods We enrolled 200 patients with pulmonary adenocarcinoma brain metastasis confirmed histologically from Jan 2013 to Dec 2015, and tested EGFR, KRAS, ALK, Her-2 gene mutation, analyzed the relationship between EGFR gene mutation and clinicopathological data and prognosis of the patients.Results The mutation rates of EGFR, KRAS, ALK and Her-2 gene mutation were 48.5%, 5.5%, 6.5%, 3.5%, respectively. Compared with EGFR gene mutation patients, the sex, age, BMI, differentiation were significant different (P<0.05) , however, the smoking was not significant different (P>0.05) . Patients with EGFR gene mutation who received targeted therapy survived longer than who did not receive targeted therapy, (28.0 ±4.5) months vs (11.2 ±1.4) months. By Log Rank (Mantel-Cox) , the median survival time between the two groups was statistically significant (P<0.05) .Conclusions The mutation rate of EGFR gene mutation was high in patients with pulmonary adenocarcinoma brain metastasis, and the patients will survivel longer by targeted therapy.

Objective To observe and study the changes of hemodynamic and respiratory mechanical parameters of patients with respiratory failure.Methods 75 patients with respiratory failure in our hospital during the time of March 2016 to May 2017 were set as the observation group, while 75 healthy persons with the same age were set as the control group.Then the hemodynamic indexes and respiratory mechanical parameters of the two groups were detected, and the detection results of observation group with different severity degree of disease were compared, and the relationship between the hemodynamic indexes, respiratory mechanical parameters and respiratory failure were analyzed with Logistic analysis. Results The hemodynamic indexes and respiratory mechanical parameters of observation group were all worse than those of the control group (P<0.05) , the hemodynamic indexes and respiratory mechanical parameters of observation group with servere disease were all worse than those of patients with mild and moderate disease, the results of patients with moderate disease were worse than those of patients with mild disease (P<0.05) , and the Logistic analysis showed the hemodynamic indexes and respiratory mechanical parameters all had close relationship to the respiratory failure. Conclusions The change of hemodynamic and respiratory mechanical parameters of patients with respiratory failure are greater, and the detection results of patients with different severity degree of disease have significant differences, so those items of patients with respiratory failure should be monitored and improveed.

Objective To measure the upper airway of obstructive sleep apnea hypopnea syndrome by 256 slice spiril CT, and to access the airway obstruction plane with the airway plane data in OSAHS patients.Through these measurements, we can provide assistance for clinical diagnosis and treatment.Methods This study randomly selected 178 patients diagnosed with OSAHS and 110 cases non-snoring healthy people as the control group by the Philips 256 slice CT.Under the nasopharyngeal area, velopharyngeal area, glossopharyngeal area, epiglottis area, we measured the cross-sectional area and volume of the narrowest plane in the two groups of quiet respiration and Müller status.Results The OSAHS group underwent quiet respiration and Müller status during CT scanning, and the two states about epiglottis area in cross-sectional area and volume had no significant difference, Other groups had differences between the parameters.There was a significant difference in the volume about velopharyngeal area and glossopharyngeal area. In the control group undergoing quiet respiration and Müller status during CT scanning, there was difference in velopharyngeal cross-sectional area.Other parameters had no significant difference.Conclusions The obstruction plate of OSAHS patients with 256-slice spiril CT measurement is mostly in the velopharyngeal area and glossopharyngeal area. The volume measurement of upper airway by CT can predict airway obstruction plate in patients with OSAHS.

Objective To study and observe the influence of atorvastatin for the CXCL chemokines and vascular endothelial damage indexes of patients with angina pectoris of coronary heart disease.Methods 86 patients with angina pectoris of coronary heart disease in our hospital from February 2016 to July 2017 were selected as the study objects, and all the patients were divided into the control group and the observation group with 43 cases in each group according to the random number table.The control group were treated with simvastatin on the basis of routine treatment, and the observation group were treated with atorvastatin on the basis of routine treatment.Then the treatment effect of angina pectoris, adverse reaction rates, CXCL chemokines and vascular endothelial damage indexes before and after the treatment of two groups were analyzed and compared.Results The total effective rate of observation group with angina pectoris was significantly higher than that of control group, so the difference had statistical significance (P<0.05) , the adverse reaction rates of two groups were compared, so the difference had no statistical significance (P>0.05) , the CXCL chemokines and vascular endothelial damage indexes of two groups before the treatment were compared, and the differenceshad no statistical significance (P>0.05) , the serum PGI2 of observation group after the treatment were lower than those of control group, the serum CXCL chemokines and other vascular endothelial damage indexes were lower than those of control group, and the differences had statistical significance (P<0.05) . Conclusions The treatment effect of atorvastatin in the patients with angina pectoris of coronary heart disease is better, and its safety is higher, and it has a better improvement role for the CXCL chemokines and vascular endothelial damage indexes, so it has a higher application value in the patients with angina pectoris of coronary heart disease.

Objective To study the genotypes of hepatitis C patients in three large hospitals in Shanxi Province, and to analyze the results of the classification. Methods Genotypes and related disease markers of253 cases of hepatitis C positive patients in a total of 437 cases were analyzed.Results There was a significant difference in the I content of each type of bile acid receptor and HCV gene content among all HCV Classification patients (P<0.05) while there was no significant difference in the therapeutic effects of pegylated interferon and Galebavirin in different subtypes of HCV (P>0.05) . Conclusions Hepatitis C patients in Shanxi province are mainly type Ib and type II A, and there are significant differences in each type of gene content (P<0.05) . The content of I of bile acid receptor is of great significance in the judgement and treatment of the disease.Type 3 b, type 3 a and type 6 a have higher cure rates and better recovery rates, while type1 b, type 1 a and type 2 a have lower cure rates and recovery rates.The use of oral drugs makes hepatitis C treatment better.

Objective To explore the significance of modified pediatric early warning score (MPEWS) in emergency early warning triage and classification. Method Selecting ill children who came to emergency department from February of 2017 to January of 2018 as objects of study, and triage nurses of emergency department gave MPEWS to the sick children under the demand of the filed data collection. Furthermore, according to the five levels of disease severity classification, doctors gave the disease assessment and classification to the sick children, finding the relevance between the MPEWS and the severity of the disease.Result Consequently, there is a relevance between the MPEWS scores and the severity of the disease indeed (rs=-0.630, P < 0.001) . The data show that the higher of the scores, the lower disease level, and the higher severity of the disease. ROC areas under the curve of the subjects is 0.996, and the confidence interval is 0.993-0.999 (P <0.05) . That indicates that MPEWS exists statistical significance of emergency children judgement. The optimal number is 4.5, the sensitivity is 96%, and the specificity is 99.9%.Conclusions MPEWS is valuable in emergency early warning triage, children emergency severity assessment and identifying critical ill children in time.

Abstract:
Objective To investigate the clinical effect of noninvasive mechanical ventilation in the treatment of chronic obstructive pulmonary disease (COPD) and the expression of insulin-like growth factor (IGF) Ⅰ, Ⅱand cathepsin B (CB) in patients.Methods 89 patients with chronic obstructive pulmonary disease from January2014 to June 2017 in the hospital for treatment were retrospectively chosen as the research objects. They were randomly divided into the control group (n = 43) and the study group (n = 46) . The control group was treated with conventional therapy. The study group was treated with noninvasive mechanical ventilation on the basis of the control group. The observation period was 10 days of treatment, the level of the indicators of lung function, arterial blood gas changes of the both groups were detected, and the clinical treatment efficiency was taken for statistics. At the same time, the levels of IGF-Ⅰ, Ⅱ and CB in patients were analyzed.Results (1) The lung function of the study group: the lung function VC [ (302.72 ± 40.79) m L], the maximum expiratory flow PEF (421.72 ± 37.59) L/min], and the first expiratory volume FEV1 [ (83.24 ± 11.65) %], were significantly higher than that of the control group (P<0.05) . (2) The arterial blood gas in the study group: arterial oxygen partial pressure (PaO2) [ (99.24 ± 10.16) mm Hg], partial pressure of carbon dioxide (PaCO2) [ (43.32 ± 5.37) mm Hg], oxygenation index (PaO2/FiO2) [ (281.02 ± 19.74) % ], were better than the level of detection in the control group (P<0.05) . (3) The effective rate of clinical treatment was 93.48% in the study group, which was significantly higher than that in the control group of 67.44% (P<0.05) . (4) The IGF-Ⅰ [ (11.47 ± 1.53) ug/L] and IGF-Ⅱ [ (28.27± 11.49) ug/L] in the study group were lower than those in the control group, while CB [ (1.03 ± 0.24) ng/ml]was higher than that of the control group (P<0.05) .Conclusions Noninvasive mechanical ventilation is helpful to clear the recovery of lung function in patients with chronic obstructive pulmonary disease, can alter the abnormal expression of insulin-like growth factor I, II and cathepsin B levels in patients, and the clinical treatment effect is significant.
A Study on Expression of Histone Deacetylase-2 in Humantongue Squamous Cell Carcinomas

Abstract:
Objective To study the expression of HDAC2 and the correlations with pathological parameters in human tongue squamous cell carcinoma, and to analyze the correlation between them and patients' clinical indicators and survival status.Methods A total of 96 cases of humantongue squamous cell carcinoma (SCC) , 24 dysplasia (ED) , 13 normal mucosa (NOM) were involved in this study. The expression of HDAC2 was evaluated by ICH. Kaplan-Meier was used to research cumulative surviving of SCC patients.Result Among 96 cases of SCC samples, 86 cases showed positive expression (89.6%) , The differences between the overexpression of HDAC2 and the pathological grading, lymph node metastasis, the patient's age and gender were insignificant (all P>0.05) . However, the differences of TNM presented statistically significant (χ2 = 18.600, P<0.001) . 14 out of 24 ED (58.3%) were positive expression of HDAC2, there were 7 cases (53.8%) of positive expression samples in NOM. The expression of HDAC2 in SCCs was significantly higher than that in ED and NOM (χ2= 118.407, P<0.00) . There was no significant difference of cumulative surviving between the positive and negative SCC patients (P = 0.184) . However cumulative surviving of patients with lymph node metastasis was significantly shorter than those with no metastasis (P<0.000) .Conclusions The expression of HDAC2 in SCC was significantly higher than that in ED and NOM, the TNM stage of patients presented the significant difference of expression of HDAC2, patients with lymph node metastasis were significantly shorter than those with no metastasis, and demonstration can be used as a reliable reference molecule for the evaluating prognosis of SCC.

Abstract:
Objective To investigate the recurrence rate, the efficacy and safety of auxiliary hepatic arterial chemoembolization after the treatment of microvascular invasion (MIV) of hepatocellular carcinoma. Methods A total of 104 patients with hepatocellular carcinoma clinical staging of early metaphase were collected from the First Affiliated Hospital of Xi'an Jiaotong University in Jan.2010 to Dec.2016, all treated with liver resection and follow-ups. Adopting the method of retrospective analysis, univariate and multivariate analyses were performed to investigate the risk factors for recurrence-free survival (RFS) and overall survival (OS) . Multiple logistic regression was used to identify the clinicopathological characteristics associated with MVI. The rates of RFS and OS were compared among patients with or without MVI treated with liver resection alone or followed by PA-TACE. Results In HCC patients, the gender HBs Ag, total bilirubin, and prothrombin time, AFP, HBV-DNA, the size of largest tumor, capsule, operation type, hepatic door block, Child-Pugh grade, classification comparison differences had no statistical significance between the two groups of with or without TACE (P>0.05) ; Age, nodules, tumor cell resection, and BCLC were statistically significant in the non-MIV infiltration group (P<0.05) .Hypersplenism and recurrence rate were statistically significant in MIV group (P<0.05) .Univariate and multivariate analyses demonstrated that AFP (> 400 ng/m L) , tumor diameter, membrane integrity, MVI, and most liver resection were significantly associated with overall survival rates (P<0.05) , the AFP (>400 ng/m L) , Child-Pugh, grade B, tumor diameter, hepatocellular carcinoma (HCC) and other abnormal degree, membrane integrity, MVI have significant correlation with disease-free survival rate (P<0.05) . Multiple logistic regression identified serum AFP level > 400 ng/L, tumor size > 5 cm, and incomplete tumor envelope was independent correlation factor (P>0.05) . In the HCC patients with MIV, both OS and DFS were significantly improved in patients who received PA-TACE as compared to those who underwent liver resection alone (P> 0.05) , but no significant effect in HCC patients wieh no MIV invasion (P> 0.05) .Conclusions There is a certain correlation between microvascular infiltration and the recurrence rate and survival rate of hepatocellular carcinoma, and the method of using adjuvant hepatic arterial chemoembolization is helpful to reduce the recurrence rate and prolong the survival period.
An Application of Norepinephrine in Renal Transplantation Guided by Hemodynamic Monitoring

Abstract:
Objective To investigate the use of noradrenaline under different monitoring methods in renal transplantation and its clinical significance in prognosis. Methods 40 patients under the surgery of renal allograft were randomly divided into two groups (20 patients each) : Flotrac-vigileo group (E) and the standard monitoring group (N) , then we recorded MAP (mean arterial pressure) , CVP (central venous pressure) and HR (heart rate) value divided by 5 periods: after induction of general anesthesia (T1) , before operation (T2) , external iliac vein occlusion (T3) , external iliac vein opening (T4) and the end of operation (T5) .Meanwhile, intraoperative norepinephrine dosage, intake and output volumnand renal function indicators during postoperative and after three days were also studies. Length of hospital stay were also compared in this essay.Results The amount of norepinephrine in group E during operation was significantly lower than that in group N (P<0.05) . The urine volume in group E during operation was significantly higher than that in group N (P<0.05) . There was no significant difference between the two groups in the amount of transfusion, blood loss and the indexes of kidney function. Conclusions Hemodynamics monitored by peripheral cardiac output significantly reduced the amount of norepinephrine used in renal transplantation, also in the risk of renal damage. Yet it increased the amount of intraoperative urine output, which provided more accurate and personalized management of intraoperative vasoactive drugs.

Abstract:
Objective To evaluate the clinical efficacy of sclera fixing surgery in suture less posterior chamber intraocular lens (IOL) . Method From March 2016 to February 2018, 60 patients undergoing IOL implantation in the Yuxi People's Hospital were selected as the study objects. They were randomly divided into thd study group (n=30) and the control group (n=30) .The control group was given suture and fixation of ciliary sulcus in intraocular lens. The study group was treated with sclero fixing surgery in suture less posterior chamber intraocular lens, and we analyzed the visual acuity, intraocular lens stability, and occurrence of adverse reactions in both groups.Result The visual acuity of the study group was significantly better than that of the control group (P<0.05) . The stability of the intraocular lens was better in the study group than in the control group (P<0.05) . The incidence of adverse reactions in the study group was significantly higher than that of the control group (6.67%) (P<0.05) . Conclusions The sclera fixation surgery in suture less posterior chamber intraocular lens is a safe and effective treatment method for posterior chamber IOL without lens implantation with full capsular support, which has less trauma and fewer complications than sclera suture fixation of posterior chamber intraocular lens.It can effectively improve the vision of patients, significantly reduce the incidence of adverse reactions, and is beneficial to to patient's prevention, and it is worth for promoting.

Abstract:
Objective To investigate the clinical significance and correlation of end-tidalcarbon dioxide partial pressure (PetCO2) and arterial blood carbon dioxide pressure (PaCO2) of children with different types of congenital heart disease in perioperative period of cardiopulmonary bypass.Methods 60 children undergoing selective cardiopulmonary bypass surgery for congenital heart disease were chosen (aged 1-14 years old, ASA class II or III) .According to echocardiographic results, the children were divided into two groups. Group A:Left-to-right shunt in children, involving 15 cases of atrial septal defect and 15 cases of ventricular septal defect;Group B: Right-to-left shunt in children, involving 30 cases of Fallot tetrad. All 60 children underwent intravenous general anesthesia and rapid induction of orotracheal intubation. All patients underwent ventilator-assisted ventilation in the surgical intensive care unit for 8 hours after surgery. The brachial artery blood was collected for arterial blood gas analysis to measure PaCO2 at the beginning of surgery (T1) , 15 min after anesthesia (T2) , before CPB (T3) , 15 min after CPB (T4) , end of surgery (T5) , time to transfer to SICU (T6) , 2 h after monitored in ICU (T7) , 4 h after monitored in SICU (T8) , 6 h after monitored in SICU (T9) , 8 h after monitored in SICU (T10) .The PaCO2 and PetCO2 were recorded at each time point. According to the result of PetCO2 and PaCO2, we studied the correlation of the two indexes at each time point.Results In group A, there was a positive correlation between PetCO2 and PaCO2 at each time point (the correlation was 0.55 to 0.87, P<0.001) . The correlation between PETCO2 and PaCO2 decreased gradually during T2-6, and the correlation gradually increased during T7-10. There was no significant correlation between PetCO2 and PaCO2 (the correlation was0.21 to 0.38, P > 0.05) before operation in group B until 15 minutes after CPB. From the end of surgery to 8 hours after SICU monitoring, PetCO2 was positively correlated with PaCO2 (the correlation was0.54 to 0.83, P < 0.05) .The correlation between the two indexes increased gradually during T5-10. Conclusions During the perioperative period of cardiopulmonary bypass, PetCO2 can be used as an evaluation indicator for clinical monitoring in the children with Left-to-right shunt of congenital heart disease.There was no significant correlation between PetCO2 and PaCO2 in the children with congenital heart disease of right-to-left shunting before the end of surgery. PetCO2 could not replace PaCO2.With the improvement of postoperative conditions, It is significant to dynamically observe changes in PetCO2 to guide the analysis of PaCO2.
Clinical Significance of Serum Vitamin D Levels Change in Patients with Breast Cancer before and after Chemoradiotherapy

Abstract:
Objective To investigate the clinical significance of serum vitamin D levels change in patients with breast cancer (BC) before and after radiotherapy and chemotherapy.Methods A total of 100 BC patients and 100 healthy controls were enrolled in the First Affiliated Hospital of Xi'an Jiaotong University from June 2016 to June 2017. All the patients received adjuvant chemotherapy and radiotherapy after operation. Serum 25-hydroxyvitamin D3 (25-OHD) levels were measured using the DRG ELISA kit.Results There was no significant difference in age, BMI, times of deliveries, hemoglobin level, total lymphocyte count and platelet count between the cases and the controls (P>0.05) . The incidence of vitamin deficiency was 100% in the case group and 85% in the control group. The level of serum vitamin D was (16.63 ± 1.83) ng/ml in the case group before radiotherapy and chemotherapy, and (25.56 ± 6.64) ng/m L in the control group. The level of vitamin D before radiotherapy and chemotherapy in the case group was significantly lower than that of the control group (P< 0.001) . The mean level of vitamin D was (11.35 ± 2.87) ng/m L after radiotherapy and was significantly lower than that before radiotherapy and chemotherapy (P<0.001) . Two different chemotherapy regimens were used in this study, FEC/T and CMF regimens. Before radiotherapy and chemotherapy, there was no significant difference in vitamin D levels between the two groups (P>0.05) . After radiotherapy and chemotherapy, vitamin D levels in both groups were significantly decreased (P<0.001) , but there was no significant difference between the two groups in vitamin D levels (P>0.05) . Multiple regression analysis showed that age, BMI and number of deliveries were significantly correlated with changes of vitamin D levels before and after radiotherapy and chemotherapy (P<0.05) , and correlations were as follows: age, BMI and times of deliveries.Conclusion Vitamin D deficiency may be associated with breast cancer. Patients' age, BMI and times of deliveries were significantly correlated with vitamin D levels before and after radiotherapy and chemotherapy.

Abstract:
Objective to compare and contrast the diagnosis results on 2D and 3D Ultrasonogrpahy against MRI. Method A 2D Ultrasonography was applied during a conventional prenatal sonography checking with a 3D Sonography assessment subsequently conducted to follow up on 49 fetus suspected of brain malformation.Furthermore, a MRI scan was taken within 24 hours after the 3D Sonography checking in our hospital as a final test. Data collections from all three assessments were completed, and an analysis of the comparisons of these three methods were done. Results Among these 49 fetus with confirmed or suspected brain malformation, there were two cases of misdiagnoses of Dandy-Walker Malformation assessed by 2D sonography, with a misdiagnosis rate of 40%, (P < 0.05) indicating a statistically significant result; misdiagnosis rate of Fetal Ventriculomegaly and Isolated Broadening Posterior Fossa were calculated as 26.7% and 33.3% respectively (P <0.05) , a statistically significant result. Overall, there were two cases with Cerebellum Malformation, from in which one case was identified by MRI, and the other one was misdiagnosed, with a misdiagnosis rate of 50.0%.In total, there were 2 cases of Holoprosencephaly, in which one was identified by Prenatal MRI, and the other was misdiagnosed (P < 0.05) , a statistically significant result. Conclusions All three assessments of 2D ultrasonography, 3D ultrasonography and MRI have their own advantages and disadvantages. In short, 2D Sonography is suggested to be applied for screening out cases with brain malformation, together with 3D Sonography as a complementary assessment. MRI can also be an effective and significant complement for sonography in completing and readdressing the final ultrasonic results.

Abstract:
As a recognized precancerous disease, the chronic atrophic gastritis (CAG) has a low diagnosis rate at an early stage due to its atypical symptoms. While massive studies have shown that changes in the serum gastric function parameters including pepsinogen (PG) and gastrin 17 (G17) levels which can be reflected by the functional and morphological status of the gastric mucosa, moreover, H. pylori plays a catalytic role in mucosal atrophy and intestinal metaplasia, indirectly affects the values of the above items. In this article, we discussd the progress in the diagnosis of atrophic gastritis and the risk assessment of gastric cancer based on serum gastric function tests.

Abstract:
Amphetamines abuse is defined as a chronic recurrent encephalopathy, and it is a global public health problem which seriously threatens the health of human and the social stability. Long-term abuse and addiction of amphetamines leads to structural and functional changes of specific encephalic regions. Further researches on these encephalic regions, the network of brain and biological information may be helpful to understanding drug abuse mechanism and possible therapeutic measures. Recently, a series of functional imaging techniques, including magnetic resonance imaging (MRI) , functional magnetic resonance imaging (f MRI) , magnetic resonance spectroscopy (MRS) , diffusion tensor imaging (DTI) , and positron emission tomography (PET) , were used to detect different brain structural changes of the volume and density of encephalic regions, functional changes of cerebral blood flow and brain cognition. The results showed functional imaging techniques play significant roles to detect different structural and functional changes of the brain. Based on these results, the researchers aim to clarify the mechanisms of drug abuse. That is the main focus of this review.

Abstract:
Objective To research the application effect of mobile network PBL in combination with the micro-lesson teaching in surgical practice.Methods The research objects were the clinical medical interns of grade 2012. Students were divided into two groups: the experimental group using mobile network PBL in combination with the micro-lesson teaching method, and the control group using the traditional LBL teaching method. The examination scores, case analysis and discuss performance were compared between the two groups.Results The tests grades, clinical cases analysis, discuss performance of the experimental group were significantly higher than those of the control group (P<0.05) ; Active classroom atmosphere, active learning, improve teamwork and interpersonal communication of the experimental group were significantly higher than those of control group, et cetera (P<0.01) .Conclusion The effect of mobile network PBL in combination with the micro-lesson teaching method in surgical practice teaching is very good.

Objective To learn the effects on improving mental anxiety and depression in empty-nest elderly with hip replacement by accompany care work, and the factors influencing psychological anxiety and depression after hip replacement in empty-nest eldly. Then to explore comprehensive nursing strategies for hip arthroplasty in empty-nest eldly. Methods We investigated empty-nest elderly with hip arthroplasty by using homemade questionnaire, SAS and SDS scale between november 2015 and october 2017. Different nursing methods were used to treat empty-nesters in different groups, and the influencing factors of anxiety and depression were analyzed.Results It is not statistically significant in demographic characteristics (χ2<3.84, P>0.05) and anxiety (t = 1.2037, P=0.2308) and depression (t =1.0094, P=0.3142) at the time of entering the hospital, etc. Patients anxiety is statistically significant at time of discharge (t =-5.0159, P≤0.001) and one month after discharge (t =-9.5160, P < 0.001) , Patients depression is statistically significant at time of discharge (t =-2.9271, P = 0.0039) and one month after discharge (t =-8.2383, P<0.001) .The factors causing anxiety is nursing mode, number of chronic diseases and postoperative complications. The factors causing depression are economic income, nursing mode, and number of chronic diseases.Conclusion Accompany care work can improve the problems of anxiety and depression in an empty-nest eldly man after hip replacement. It is related to anxiety in nursing patterns, chronic diseases, postoperative complications.Meanhiwle while depression is related to economic income, nursing patterns, and types of chronic diseases, etc.

Objective To study the effect and application of 2-hour feeding in premature infants with very low birth weight.Methods 162 newborn cases with very low birth weight in the pediatrics department, The Third People's Hospital of Yunnan Province in September 2016-September 2017, were randomly divided into two groups:the experimental group with 81 cases (feeding every 2 hours) , and the control group with 81 cases (feeding every three hours) . The application effect was compared in the two groups.Results Comparing the gastrointestinal tract feeding tolerance, growth rate, feeding time, days of intravenous infusion of high nutrient solution, risk of intrahepatic cholestasis disease, and days of hospitalization between the experimental group and the control group, the data had significant differences (P < 0.05) . Conclusion 2-hour feeding in low birth premature infants can effectively stimulate the intestinal peristalsis, improve children's tolerance, speed up the growth rate, resume the gastrointestinal tract feeding as soon as possible, reduce the risk of cholestasis occurs and reduce the number of days in hospital. In clinical work, children's feeding tolerance, gastric retention, and bloating situation should be under strict observation and be delt with in time. These methods are worth promoting in the neonatal wards.

Abstract:

Objective To study the characteristics of ambulatory blood pressure in advanced age patients with hypertension during pregnancy and its effect on pregnancy outcome, and to explore the clinical significance of ambulatory blood pressure monitoring in advanced age women with hypertension.Methods A retrospective analysis from January 2016 to August 2017, in patients with pregnancy induced hypertension in the Second Affiliated Hospital of Kunming Medical University of production inspection and delivery in hospital as the research object, according to the age of pregnant women, they were divided into the advanced age group (35 years of age or older, n=100), and age group (age<35, n=100). All the pregnant women underwent 24-hour ambulatory blood pressure examination in the middle and third trimester of pregnancy to compare the difference of ambulatory blood pressure parameters between the two groups and the relationship between them and the adverse outcome of pregnancy. Results Systolic pressure index of the advanced age group 24 hours average systolic pressure, day mean systolic pressure, night mean systolic pressure, day systolic pressure load value, night systolic pressure load value were higher than the age group, the differences were statistically significant (P<0.05); The incidences of cesarean section and premature infant were higher than thase in the age group, and the differences were was statistically significant (P<0.05). The average diastolic blood pressure andthe day systolic load were correlated with fetal growth restriction in both the aged and age groups, and the 24-hour mean diastolic blood pressure was associated with preterm infants. Conclusions Ambulatory blood pressure monitoring has a positive significance in the diagnosis, evaluation and reduction of adverse pregnancy outcomes in the advanced age patients with hypertensive pregnancy, and can be used as prenatal routine examination for the advanced age pregnant women.
