Application of Coronary Computed Tomography Angiography in Overweight Patients: Comparison of Automated Tube Voltage Selection Versus 100 kVp Tube Voltage

Abstract:
  Objective   To compare the image quality and radiation dose of automated tube voltage selection (APSCM) and 100 kVp coronary computed tomography angiography (CCTA) in overweight patients.   Methods   A total of 115 patients with a body mass index (BMI) higher than 25 kg/m2 and less than 30 kg/m2 and body weight lower than 90 kg underwent high-pitch prospectively ECG-triggered CCTA on a second-generation dual-source CT scanner. All patients were classified into two groups: Group A, 75 patients were examined with APSCM; Group B, 40 patients with a fixed tube voltage of 100 kVp. The attenuation and image noise were measured and signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were calculated. Image quality was evaluated and radiation dose was recorded.   Results   In Group A, 80 kVp, 100 kVp and 120 kVp were automatically selected in 1 (1.3%), 51 (68.0%) and 23 (30.7%) patients, respectively. The mean CT attenuations and image noise in APSCM group were significantly lower than those in Group B (all P < 0.001). Group A showed a higher mean CNR but a lower mean SNR (all P < 0.05) than Group B. The subjective image quality scores have no significant difference between the two groups (all P < 0.05). The radiation dose in Group B was 22.2% lower than in Group A ( P = 0.004).   Conclusion   In patients with BMI higher than 25 kg/m2 and less than 30 kg/m2, 100 kVp CCTA allows lower radiation dose without compromising image quality when compared with CCTA using APSCM.
Correlation of Adult Urine Iodine Level and Thyroid Disease

Abstract:
  Objective   To understand the relationship between the distribution of urinary iodine level and thyroid diseases in adults in kunming, and to provide scientific basis for iodine supplementation in thyroid patients in this region.   Method   In this prospective study, 207 patients with thyroid diseases who were admitted to the department of endocrinology and outpatient department of the affiliated calmette hospital of kunming medical university from January to December 2018 were included as the case group, and were divided into hashimoto thyroiditis group, hyperthyroidism group, hypothyroidism group, and thyroid nodule group. At the same time, 59 healthy patients in the outpatient department were selected as the control group. Groups are used arsenic cerium catalytic spectrophotometry determination of urine iodine, and application of chemiluminescence immunoassay technology to three iodine determination of serum free thyroid glycine (FT3) and free thyroid hormone (FT4), thyroid stimulating factor (TSH), thyroid peroxidase antibody (TPOAb), thyroglobulin antibody (TgAb), the case group and the control group were performed thyroid ultrasound, and according to the test results, statistical analysis was carried out on the different urine iodine level of thyroid disease.   Results   (1) For patients in hashimoto thyroiditis and thyroid function hyperfunction group, hypothyroidism and thyroid nodule group and control group, the median of urinary iodine was 177.4 g/L , 171.6 g/L, 105.2 g/L, 151.3 g/L and 178.3 g/L, respectively. The median of hypothyroidism group was significantly lower than normal group, the difference was statistically significant (P < 0.05), there was no statistically significant difference between other groups ( P > 0.05). (2) A high proportion of high and low iodine was found in each thyroid disease group. (3) There was no statistical significance between different groups in the urinary iodine levels and FT3, FT4, TSH, TPOAb and TgAb ( P > 0.05).   Conclusion   The iodine nutrition of adults in Kunming is at a sufficient level, and the supplement of iodine should be individualized, scientific and reasonable. There is a correlation between low urinary iodine and thyroid diseases. Insufficient iodine intake can promote the high incidence of thyroid diseases.
Difference Analysis of First Liver Function Indexes of Newborns with Different Gestational Age and Birth Weight

Abstract:
  Objective   To study the difference in the first liver function indexes of newborns with different gestational ages and birth weights.   Methods   We retrospectively analyzed the the first liver function indexes including total bilirubin (TBIL), direct bilirubin (DBIL), indirect bilirubin (IBIL), total protein (TP) , albumin (ALB), alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), g-glutamyltransferase (GGT), total bile Changes in acid (TBA), prealbumin (PA), cholinesterase (CHE) of 204 premature infants with different gestational ages and 206 infants with different birth weights . Then we compared and analyzed the differences of each biochemical index in each group and between groups.   Results   There were differences in the first liver function indexes of newborns with different gestational ages and birth weights: (1) Late preterm infant group, mid-term preterm infant group and extremely preterm infant group, the levels of TP and ALB decreased sequentially with the decreasing gestational ages (P < 0.05); the levels of CHE had no statistically significant difference ( P > 0.05). The levels of DBIL and GGT in late preterm infants and mid-term preterm infants were better than extremely premature infants ( P < 0.05). (2) Compared with normal birth weight infants, only TBA increased in low birth weight infants ( P < 0.05); Compared with very low birth weight infants, DBIL and CHE increased in normal birth weight and low birth weight infants, however, only DBIL in low birth weight infants was statistically significant ( P < 0.05).   Conclusions   For newborns with different gestational ages and birth weights, the younger the gestational age, the lighter the weight, the worse the liver function. The influence of gestational age on liver function is far greater than the influence of body weight on liver function.
Correlation between Hepatitis B Virus and Microvascular Invasion in Hepatocellular Carcinoma

Abstract:
  Objective   To explore the correlation between hepatitis B virus (HBV) infection and antiviral therapy and the formation and development of microvascular invasion (MVI) for hepatocellular carcinoma (HCC).   Methods   Retrospective analysis was made on the clinical and pathological data of hepatocellular carcinoma patients undergoing radical hepatectomy in the Second Affiliated Hospital of Kunming Medical University from March 2016 to October 2018.   Results   The formation of MVI was related to preoperative antiviral therapy, HBVDNA quantification, AFP quantification and postoperative histopathological grading.Among hbV-associated HCC patients, multivariate Logistic regression analysis showed that antiviral therapy (OR = 0.44 p = 0.002), HBVDNA quantification ≥2×104~≤2×107 IU/mL (OR = 2.79 p = 0.036), and AFP quantification ≥400~≤1 000 ng/mL (OR = 0.31 p = 0.023) were independent risk factors for MVI, and antiviral therapy for more than half a year was independent protective factor for MVI. High levels of HBVDNA quantification (> 2×104 IU/mL) may indicate high differentiation of MVI (P < 0.05). In addition, significant HBVDNA quantification ≥2×10 4~≤2×107 IU/mL (AUC = 0.885 P < 0.001) may be of high diagnostic value in predicting the occurrence of MVI for HBV-associated HCC.   Conclusion   HBV infection and HBVDNA quantification levels are important factors in the formation of MVI in hepatocellular carcinoma. Antiviral therapy may have a preventive effect on the formation of MVI.A high level of HBVDNA quantification may indicate highly differentiated MVI. In addition, meaningful HBVDNA quantification is of certain value for THE occurrence of MVI in HBV-associated HCC.

Abstract:
  Objective   To study the effects of CLOCK rs4580704 site polymorphism on sleep quality, the effect on occurrence and development of type 2 diabetes mellitus, and the possible relevance between the two investigations.   Methods   We tested the polymorphism of CLOCK rs4580704 by time of flight mass spectrum, and compared indicators of genotypes and sleep quality of the case group and the control group respectively.   Results   The proportion of GC type and GG type of CLOCK rs4580704 in the case group was higher than that in the control group. The sleep quality of GG genotype was significantly degraded.   Conclusion   Allele G of CLOCK rs4580704 is possibly associated with sleep quality degradation and the occurrence and development of type 2 diabetes mellitus, while allele C is possibly the protective factor.
Effect of Dexmedetomidine Combined with Nalbuphine in Meningioma Surgery on Postoperative Cognitive Function

Abstract:
  Objective  To investigate the effect of dexmedetomidine (Dex)combined with nalbuphine in meningioma surgery on inflammation and postoperative cognitive function.  Methods  From July 2018 to March 2019, 60 patients who underwent elective meningioma resection in the First Affiliated Hospital of Kunming Medical University and met the inclusion criteria were randomly divided into Dex combined with nalbuphine group (DN group, n = 30) and Dex group (D group, n = 30). In the DN group, nalbuphine hydrochloride 0.2 mg/kg was injected intravenously 5 minutes before the skin incision, and Dex was injected with a pump of 1 μg/kg 30 minutes before induction, and then changed to 0.5 μg(kg·h), and the pump was continued until meninges are sutured. Give Dex the same plan for Group D. The rest of the anesthesia scheme is same.  Results  After extubation, the HR and MAP in the DN group were lower than those in the D group (P < 0.05), and the hospital stay in the DN group was significantly shorter (P < 0.05). At the same time, the concentrations of IL-6, IL-10, and TNF-α in the DN group were lower than those in the D group; compared with the same group, the IL-6, IL-10, and TNF-α concentrations in the DN group increased less than that in the D groupbefore the operation. The 6 h and 24 h MMSE scores in the DN group were significantly higher than those in the D group (P < 0.05); compared with preoperatively, the 6 h MMSE score in the DN group was significantly lower (P < 0.05), and the 6 h and 24 h simple mental scale in the D group (MMSE) score was significantly lower (P < 0.05).  Conclusions  Dex combined with nalbuphine can better maintain hemodynamic indexes for patients undergoing meningioma resection, reduce the inflammatory response of the patients, have a smaller impact on the postoperative cognitive function of the patients, and shorten the hospital stay.
Negative Regulation of hTERT Expression in Cervical Cancer Hela Cell Line by miR-21

Abstract:
  Objective  To observe the effect of miR-21 on hTERT expression in human cervical cancer Hela cell line, explore the regulatory effect of miR-21 on hTERT.  Methods  RT-PCR was used to detect the expressions of miR-21 and hTERT mRNA in human cervical cancer Hela cell line. West Blot was used to detect the expression of hTERT protein; miR-21 mimics, miR-21 inhibitor, mimics control and inhibitor control were designed and synthesized, then they were transfected into human cervical cancer Hela cell line after parceled by liposome 2000, blank liposome 2000 was designed as control group. The changes of expressions of hTERT mRNA and protein at 24 hours after transfection were detected.  Results  miR-21 and hTERT were positive in human cervical cancer Hela cell line; the relative expression levels of hTERT mRNA in mimics group, mimics control group, inhibitor group, inhibitor control group and liposome group at 24 hours after transfection were (0.51±0.33), (0.69±0.19), (1.33±0.39), (0.83±0.26) and (0.58±0.16), respectively; the relative expression levels of hTERT protein were (0.41±0.18), (0.62±0.18), (1.45±0.60), (0.83±0.15) and (0.82±0.30), respectively. The relative expression levels of hTERT mRNA and protein in mimics group were statistically significantly lower than those in mimics control group and liposome group (P < 0.05). The relative expression levels of hTERT mRNA and protein in inhibitor group were statistically significantly higher than those in inhibitor control group and liposome group (P < 0.05).  Conclusions  miR-21 and hTERT genes are highly expressed in HeLa cell lines. MiR-21 can regulate hTERT mRNA and protein expression levels, thus negatively regulating hTERT.
Clinical effect of Invisible MA Appliance and Twin-block Appliance in Treatment of Early Skeletal Class II Mandibular Retrusion Malocclusion

Abstract:
  Objective  To comapre the clinical effect of invisible MA and double plate appliance in the treatment of children with early skeletal Class II mandibular retrusion malocclusion.  Methods  We selected 30 children with skeletal Class II mandibular retrusion malocclusion, with an average age of 10.5 years, divided them into invisible MA appliance correction group (group A) and twin-block correction group (group B), 15 cases in each group. The changes of jaw and teeth before (T0) and after (T1) treatment were measured and analyzed.  Results  Bone indexes: after treatment, there were statistically significant increases in SNB Angle, decrease in ANB Angle, decrease in Wits value, and increase in Pg/OLP of mandibular position in both groups (P < 0.05), while there was no statistically significant difference between the two groups (P > 0.05). After correction, the anterior height ratio of s-Go/N-ME in both groups decreased, while the mandibular plane Angle SN/MP, the anterior lower height ratio and the anterior lower height ratio of ANS/N-ME in both groups increased (P < 0.05), and the differences between the two groups were statistically significant (P < 0.05). Dental properties: U1-SN of the upper incisor decreased, l1-MP of the lower incisor increased, coverage improved, and molar relationship improved after treatment in both groups, all of which were statistically significant (P < 0.05). Only l1-MP Angle was significantly different between the two groups (P < 0.05). Pancherz analysis was used to conclude that the bone effect of group A was greater than that of group B.  Conclusions  The two kinds of orthodontics can effectively promote the development of mandible and improve the intersagittal deformity of mandible. After the orthodontics, the mandible will rotate clockwise, but the invisible orthodontics rotate less than the double plate orthodontics. The invisible orthodontic device MA can control the axial inclination of the anterior teeth better, and the loss of the anterior anchorage is less, so the orthodontic treatment can obtain better bone effect.
Application Value of Endobronchial Ultrasound-guided Transbronchial Needle Aspiration in Diagnosis of Space-occupying Lesions of Lung and Mediastinum

Abstract:
  Objective  To investigate the value of transbronchial needle aspiration biopsy (EBUS-TBNA) guided by ultrasound bronchoscope in the diagnosis of lung and mediastinal space-occupying lesions and staging of lung cancer.  Methods  Ninety patients with space-occupying lesions of lung and mediastinum underwent transbronchial needle aspiration biopsy guided by ultrasound bronchoscope. 151 specimens were embedded in paraffin, stained with HE, and diagnosed by cytopathology. Ki67 was detected by immunocytochemistry. Lymph node staging was performed for lung cancer. The coincidence rate and sensitivity of cytological diagnosis were analyzed according to the criteria of histopathological diagnosis combined with clinical diagnosis.  Results  The success rate of EBUS-TBNA puncture was 100% (151/151), and the success rate of diagnosis was 91.1% (82/90). The diagnostic rate of lung and mediastinal space-occupying lesions after EBUS-TBNA was significantly increased (P < 0.01).Among 90 cases, malignant tumors accounted for 72.22% (65/90), benign lesions accounted for 18.89% (17/90), and 8.89% had no definite diagnosis (8/90). 57 cases of malignant cells were found by cytology. The sensitivity of cytopathological diagnosis was 87.69%. TNM staging was achieved in 80% of lung cancers (44/55). Ki67 positive index of pulmonary tuberculosis and granulomatous inflammation was 0-3%. The positive index of Ki67 in malignant pulmonary nodules ranged from 20% to 80%. There was significant difference in Ki67 expression between benign and malignant lung lesions (P < 0.01).  Conclusions  EBUS-TBNA is a safe, sensitive and accurate diagnostic method, which can significantly improve the diagnostic success rate of lung and mediastinal space-occupying lesions. It has important guiding significance and clinical application value for the classification, staging, treatment and prognosis of lung cancer. Ki67 positive index is an important reference index for judging the benign and malignant of EBUS-TBNA puncture cells.
Evaluation of Analysis Performance of Automatic Biochemical Analyzer in Detecting Serum Lipoprotein A

Abstract:
  Objective  To evaluate the assay performance of automatic biochemical analyzer in detecting serum lipoprotein A [LP(a)] by using latex agglutination-turbidimetry on a(Abbott C16000), so as to evaluate whether it can meet clinical requirements.  Methods  According to international and domestic performance evaluation documents and industry standards, the evaluation was carried out from the aspects of precision, accuracy a, analytical measurement range, clinical reportable range, anti-interference performance and biological reference range.  Results  The intra-batch imprecisions of LP(a)were 1.44%, 0.45% and 0.91% at quality control level 1, 2 and mixed serum samples, and the inter-batch imprecisions were 1.67%, 0.81% and 1.23%, respectively, which were lower than the standard declared by the manufacturer. The correlation coefficient R2 was 0.9990 when comparing the samples with the same testing system approved by ISO15189 laboratory. The analytical measurement range was 11.0-919.0 mg/L. The maximum allowable dilution was 10 times, and the maximum reportable range was extended to 9190.0 mg/L. Commercial interference substance detection results showed that the deviation of free bilirubin was less than 20 mg/dL, binding bilirubin was less than 20 mg/dL, chylous turbidity was less than 3 000 FTU, hemoglobin was less than 500 mg/dL on the detection result of LP(a)was less than 10%, indicating that these concentrations of substances had little interference on the detection result of LP(a). The reference interval cited by the laboratory was verified by 40 healthy population samples.  Conclusion  The analytical performance of automatic biochemical analyzer in detecting LP(A)is basically consistent with the manufacturer's statement, which can meet the clinical requirements and can be applied in clinical practice.
Association of ABCG2,SLC2A9,SLC17A3 and PRKG2 SNPs Variants with Gout in the Hani Population in China

Abstract:
  Objective   To analyze the genetic characteristics and genetic mechanisms in the Hani and Han population by studying the association of five single nucleotide polymorphisms (SNPs) in four gene with gout.   Methods   A total of 100 Hani gout group, 126 Hani normal control group, 100 Hani gout group, 108 Han normal control group were enrolled in this study. Genotyping of the ABCG2, SLC2A9, SLC17A3 and PRKG2 was determined by cloning and sequencing of PCR amplicons and SNaPshot. SPSS version 20.0 was used for statistical calculations.   Results   The results showed that the genotype (P = 0.002) and allele (P = 0.024) frequency at the rs2231142 site was significantly associated with Hyperuricemia in Hani populations. In the Han populations, the genotype (P = 0.018) and allele (P = 0.034) frequency at the rs7688672 site was significantly associated with Hyperuricemia. In addition, the genotype frequency at the rs1165205 site was significantly associated with Hyperuricemia in Han populations (P = 0.037).   Conclusion   ABCG2 rs2231142, SLC17A3 rs1165205 and PRKG2 rs7688672 polymorphisms are able to affect the risk of gout, which provides a potential candidate biomarker for screening, diagnosis, and treatment of gout and Hyperuricemia.
Application of Various Sealing Materials and Adhesive Systems in Extracorporeal Pit and Fissure Sealing

Abstract:
  Objective  To evaluate marginal integrity and microleakage of resin-based sealant, flowable composite resin, compomer and glass ionomer cement combined with certain adhesive systems, in order to explore a more convenient, effective and time-saving approach which intends to be applied in less developed areas.  Methods  65 maxillary premolar teeth were kept in artificial saliva, pits and fissures were cleaned by ART equipments. The samples were allocated into 5 groups randomly: (A) phosphoric acid etching + Concise (control); (B) AdperTM Easy One + Concise; (C) AdperTM Easy One + FiltekTM Z350XT flowable resin; (D) AdperTM Easy One + Dyract Extra compomer; and (E) GC Fuji VII. The pit and fissure sealant were applied, and the operation time was recorded in vitro. After thermocycling, specimens were evaluated by scanning electron microscope and stereomicroscope.  Results  i) All the groups showed various level of microleakage; ii) Group B and C showed similar microleakage as compared with the control group, while their operation time were shorter than that of the control group (P < 0.05); iii) Group D and E showed greater microleakage than other groups (P < 0.05); iv) No shedding of sealing materials during teeth cutting in Group D.  Conclusions  Resin-based sealant and flowable composite resin combined with self-etching adhesive system has advantages of operative convenience and effectiveness. Dyract Extra compomer has better abrasion resistance.
Clinical Application of Transcervical Guidewire or Epidural Catheter in Hysteroscopic Combined with Laparoscopic Oviduct Anastomosis

Abstract:
  Objective   To explore the clinical value of transcervical guidewire or epidural catheter in surgery of hysteroscopic combined laparoscopic oviduct anastomosis.   Methods   Seventy-four cases of fallopian tube obstruction from June 2012 to August 2019 were retrospectively analyzed. During laparoscopic combined hysteroscopic oviduct anastomosis surgery, 31 cases underwent oviduct anastomosis assisted by epidural catheter or transcervical guidewire, 43 case underwent conventional oviduct anastomosis.   Results   The oviduct suture time (16.871±3.052) min vs (19.488±5.795) min (F = 5.256 P = 0.025); the number of remedial stitches of oviductanastomosis[0(0-0)] vs [0(0-2)] (χ2 = 4.963 P = 0.032), the leakage rate of Meilan liquid [5/31(16.1%)] vs [14/44(31.8%)] (χ2 = 3.938 P = 0.047); the postoperative fallopian tube patency rate of hysterosalpingography (HSG) [70/76(92.1%)] vs [29/37(78.3%)] (Z = 2.409 P = 0.016), the conventional oviduct anastomosis was significantly better than that of the group assisted by epidural catheteror transcervical guidewire (P < 0.05). The operation time 130.161±20.584) min vs (122.907±15.053) min (F = 3.070 P = 0.083) and intrauterine pregnancy rate of 3-24 months [14/31(45.2%) vs 17/43(39.5%)] (F = 0.234 P = 0.628) had no statistical significance between the two groups.   Conclusion   The operation of oviduct anastomosis assisted by transcervical guidewire or epidural catheter has the shorter oviduct suture time, better exposure field, more simple and reliable oviduct sutureas compared with those in conventional oviductanastomosis.
Effect of Hyperhomocysteinemia on Cardiac Function and Mortality of Patients with Acute Myocardial Infarction Concomitant Hypertension underwent Primary Percutaneous Coronary Intervention after 6 Months

Abstract:
  Objective   To identify the effect of hyperhomocysteinemia on cardiac function and mortality in patients with acute ST-segment elevation myocardial infarction concomitant hypertension underwent primary PCI after 6 months.   Methods   192 patients in the Department of Cardiology, 920th Hospital of Joint Logistics Support Force with acute ST-segment elevation myocardial infarction concomitant hypertension who had undergone primary PCI was recruited from June 2015 to December 2016. 116 cases were excluded from the study owing to their incomplete follow-up data .The total remaining 78 patients were divided into two groups according to the level of homocysteine (Hcy) ≥15.0 mol/L: STEMI combined with H-type hypertension group (n = 39) and the STEMI combined with hypertension group (n = 38). Baseline data of the two groups, relevant data during hospitalization and PCI, and left ventricular ejection fraction and cumulative mortality 6 months after surgery were compared.   Results   There were statistically significant differences in male proportion, serum creatinine and HCY between the two groups (P < 0.05). There was statistically significant difference in the number of lesion vessels which was higher in the high Hcy group than in the control group (P < 0.012). No statistically significant difference was showed in left ventricular ejection fraction at the time of hospitalization and the 6th month, the difference value between two left ventricular ejection fractions and the cumulative death at the 6th month between the two groups (all P > 0.05).   Conclusion   Hyperhomocysteine has no effect on cardiac function and cumulative mortality 6 months after primary PCI in patients with acute ST-segment elevation myocardial infarction concomitant hypertension.
Lymph Node Metastasis of Mixed Early Gastric Cancer and Its Influence on Prognosis

Abstract:
  Objective   To investigate the regularity of lymph node metastasis in mixed early gastric cancer and its effect on long-term prognosis.  Methods   Eighty-seven patients with mixed early gastric cancer admitted to Suzhou Jiulong Hospital, Shanghai Jiaotong University School of Medicine from January 2014 to January 2016 were selected as the research objects. All patients received surgery. The patients were followed up to observe the overall survival time (OS) and the regularity of lymph node metastasis in early gastric cancer and the effect of lymph node metastasis on OS were analyzed.   Results  Among the 87 patients with mixed early gastric cancer, 20 had lymph node metastasis and 67 had no metastasis. The lymph node metastasis rate was 22.99%. Patients with lymph node metastasis had higher tumor diameter, lymphatic involvement rate, submucosal ratio, and ulcer type than those with non-lymph node metastasis, the differences were statistically significant (P < 0.05). Logistic analysis showed that the depth of infiltration, lymphatic involvement, and tumor size were the influencing factors of lymph node metastasis in Mixed group (P < 0.05). The follow-up time was 48~72 months. The average follow-up time was (62.8 ± 4.8) months. Six patients were lost to follow-up. The 3-year survival rate was 90.1%, and the 5-year survival rate was 86.4%. Kaplan-Meier method and Log-rank test showed that the survival time of patients with lymph node metastasis was shorter than that of patients with non-lymph node metastasis (P < 0.05). Cox analysis showed that lymphatic involvement, lymph node metastasis, vascular involvement, and depth of invasion were independent factors affecting the survival of patients with mixed early gastric cancer (P < 0.05).   Conclusions  Lymph node metastasis rate of mixed early gastric cancer is high. Patients with large tumor diameter, submucosa, and lymphatic involvement have an increased risk of lymph node metastasis. Patients with lymph node metastasis have a poor prognosis and a high mortality rate.
Clinical Effect of Digital Complete Dentures for Edentulous Patients

Abstract:
  Objective  To compare the clinical effect of digital complete dentures and traditional complete dentures for edentulous patients.   Methods  Thirty-four edentulous patients were randomly divided into two groups.The digital group used digital method complete denture, and the traditional group used traditional method complete denture. The clinical effects of the two groups were compared and statistically analyzed.   Results  Within 1 month, the total number of denture tenderness points in the digital group was less than that in the traditional group, the difference was statistically significant (P < 0.001). After 1 month, the chewing efficiency of the digital group was higher than the traditional group, the difference was statistically significant (P < 0.05). 6 months after, the alveolar bone absorption in anterior and posterior teeth of mandibular alveolar of the digital group was less than that the traditional group, the difference was statistically significant (P < 0.05). 1 week, 2 months later, the scores of comfort, retention and chewing function in the digital group were higher than the traditional group, and the difference was statistically significant (P < 0.05), but there was no significant difference in appearance and pronunciation (P>0.05).   Conclusion  The clinical effect of digital complete denture in edentulous patients is good, digital complete denture has advantages in alleviating pain, reducing alveolar bone absorption, and improving comfort, retention, chewing efficiency, and is worth popularizing.

Abstract:
  Objective   To investigate the cognition of endocrinologists and diabetic patients on periodontitis and the correlation between periodontitis and diabetes.   Methods   Random sampling was used to send questionnaires to hospitals in Haikou City, Hainan Province, in order to understand the cognition of doctors and patients on the two-way relationship between periodontitis and diabetes.   Results   Only 48.9% of the doctors agreed that periodontitis may affect the normal blood glucose level, 86.2% of the patients did not know that “treating periodontitis is beneficial to prevent the occurrence of diabetes”, and the cognitive level of periodontitis decreased with the increase of age and the decrease of education level, the difference was statistically significant (P All were < 0.05). 93.6% of the doctors said they would pay attention to the oral condition of the patients, but only 23.6% of the patients said they had been recommended by doctors for oral health examination.   Conclusion   The public, represented by the endocrinologists and diabetic patients in Haikou City, has a low level of cognition about the correlation between periodontitis and diabetes, so we should strengthen the education of the correlation between periodontitis and diabetes in the process of clinical treatment and management of patients.

Abstract:
  Objective  To provide clinical basis for the prevention and treatment of regional urinary calculi by retrospectively analyzing the clinical data of urolithiasis patients.  Methods  The medical records of patients with urinary calculi in 2016 and 2017 in the Department of Urology, Second Affiliated Hospital of Kunming Medical University were collected and retrospectively investigated.  Results  There were 6545 urolithiasis patients in 2016 and 2017, which accounted for 47.8% of hospitalized patients in urology department with the same period, with the ration of 2.12∶1 between male and female patients. In this group, the high-incidence age was between 41 and 60 years old (67.8%), and the farmers (33.9%) were the main occupation. Also, we found the in-patients of overweight and obesity was 30.5% and 18.5% respectively. Statistically, the upper urinary calculi were most common in urinary calculi, and the distribution of calculi was different in different genders(P < 0.05). The 463 reports our researchers investigated showed that the calcium oxalate stone composition accounted for the highest proportion, up to 68% and mixed component stones occupied 79%. The average CT value of calculi was 837.2±286.1 HU. 75.7% calculi with CT value less than 500 HU were uric acid stones, 67.2% calculi with CT value more than 800 HU were calcium stone, and 85% calculi with CT value between 500~800 HU were infectious calculi (magnesium ammonium phosphate/phosphorite apatite). The urinary PH of calculi patients was 6.3 ± 0.9. The main component of calculi in patients with urine pH < 5 was uric acid, accounting for 72.7%; the main component of calculi in patients with urine pH > 6.5 was infected calculi, accounting for 92%; the main component of calculi in patients with urine 5≤pH≤6.5 was calcium stone, accounting for 67.1%.  Conclusions  (1) This group of urolithiasis patients accounted for a higher proportion of urological inpatients in the same period, suggesting that urolithiasis is the most common urological disease in Yunnan Province; There are more men than women in urolithiasis, the high incidence age is 41-60 years old; The farmers are the main occupation. (2) Upper urinary calculi are more common in urolithiasis, and the distribution of calculi was different in different genders. (3) The CT values of stones with different components are different. The CT value of uric acid stones is below 500 HU; the CT value of infected stones ranges from 500 HU to 800 HU; the CT value of calcium-containing stones is above 800 HU. (4) Urine pH is an important factor affecting the formation of calculi. Acidic urine is easy to form uric acid stones, and alkaline urine are easy to form infected stones .
Efficacy and Safety of Sofosbuvir/Vipatavir Combined with Ribavirin in Treatment of Genotype 3 Patients with Chronic Hepatitis C

Abstract:
  Objective  To observe the efficacy and safety of combined with Ribavirin (SOF/VEL+RBV) in the treatment of genotype-3 patients with chronic hepatitis C.  Methods  We selected 80 genotype-3, and newly treated chronic hepatitis C patients, who were admitted to the Department of Liver Diseases, Kunming Third People's Hospital, and divided them into 2 groups. The combined treatment group, containing 40 patients, was treated with the antiviral therapy that combines Sofosbuvir/Velpatasvir and Ribavirin (SOF/VEL + RBV). The control group was 40 patients that were adopted with a single-agent antiviral treatment of Sofosbuvir/Velpatasvir. All patients were monitored with their continuous virological response, the changes in biochemical indicators and Fibroscan indicators during the treatment of 4 and 12 weeks as well as an additional 12 weeks after stopping the therapy.  Results  The combined treatment group showed their SVR rate at the 4th and 12th week were 90.00%, 70.00% and 97.50%, 80.00%, respectively and reached 97.50%, 80.00%, at 12th week after the termination of the treatment. The patients' serum biochemical indicators such as ALT, AST, GGT, and ALB decreased more significantly than the patients in the single-agent group (P < 0.05) and there was no significant difference between the two groups at the 12-week follow-up after the treatment termination (P > 0.05). For the single-agent group, at the 12th week after the treatment the Fibroscan value decrease significantly (F = 5.700, P = 0.018) and at 12th-week follow-up after the treatment termination there was no difference in Fibroscan values between the two groups (F = 0.012, P = 0.912). During the period of the treatment time, the adverse reactions from both groups were mainly fatigue, headache and dizziness.  Conclusions  For genotype-3 patients with newly treated chronic hepatitis C, the combined treatment of Sofosbuvir/Velpatasvir and Ribavirin (SOF/VEL + RBV) can have higher SVR rate and biochemical response rate, better efffect on improving liver fibrosis with better safety than single-agent treatment of Sofosbuvir/Velpatasvir.

Abstract:
  Objectives  To search the risk factors of coronary slow flow phenomenon (CSFP) and to investigate the relationship between the polymorphism of leptin gene -LEP rs7799039, leptin receptor gene- LEPR rs1137100, LEPR rs1137101 and CSFP.   Methods  Fifty-four CSFP cases and 46 NCF cases were selected. Fasting blood glucose (FBG) and fasting insulin level (FINS) were measured. Homeostasis model resistance index (HOMA-IR) was used to evaluate insulin resistance. The serum leptin and leptin receptor concentrations were measured by enzyme-linked immunosorbent assay (ELISA). The distribution frequency of LEP rs7799039, LEPR rs1137100 and LEPR rs1137101 were analyzed by SNaPShot technique to explore whether there is correlation between leptin and leptin receptor gene polymorphism and CSFP, and to explore the risk factors of CSFP by multivariate logistic regression analysis.   Results  1. Compared with NCF group, CSFP group had higher leptin level (P = 0.000) and insulin resistance index (P = 0.046). 2. Mean TFC of LAD (corrected), LCX and RCA in CSFP group were higher than those in NCF group. 3.There was no significant difference in the distribution frequencies of LEP rs7799039, LEPR rs1137100 and LEPR rs1137101 between CSFP group and NCF group. 4.Logistic stepwise regression analysis showed that LEP and HOMA-IR were related risk factors for CSFP.   Conclusions  1. LEP and insulin resistance are the risk factors for the development of CSFP. 2. There is no correlation between leptin and leptin receptor gene polymorphism and CSFP.

Abstract:
  Objective   To explore the value of combined IL-6, NSE and craniocerebral ultrasound in early diagnosis of white matter injury in premature infants in clinical practice.   Methods  The research objects of this study were 300 premature infants, 150 of whom were in the research group with white matter injury, and the early infants without white matter injury in the same period were in the control group. The duration of the study was from January 2018 to January 2020. IL-6 was tested in all preterm infants. NSE was diagnosed and the echo changes of white matter around the ventricle were observed by craniocerebral ultrasound. The specificity, sensitivity and accuracy of IL-6, NSE diagnosis were analyzed statistically.   Results  The detection rate of craniocerebral ultrasonography on day 1, day 3 and day 7 after birth was higher in the research group than in the control group (P < 0.05). The gray value of craniocerebral heavy brain white matter on day 1, day 3 and day 7 after birth was compared between the two groups, the research group was all higher than the control group (P < 0.05). Results of IL-6 and NSE on day 1, day 3 and day 7 after birth were compared between the two groups, the research group was higher than the control group (P < 0.05). According to cranial MRI as the gold standard, the sensitivity and specificity of cranial ultrasound combined with IL-6 and NSE were 97.3% and 95.4%, both of which were superior to IL-6 and NSE, and the difference of the dominant chi-square test was statistically significant (P < 0.01). Therefore, the three combined diagnoses could improve the diagnostic effectiveness.   Conclusions  IL-6, NSE and craniocerebral ultrasound are of significance for the diagnosis of white matter injury in premature infants. Combined with the above three kinds of information, the diagnosis has higher sensitivity, specificity and accuracy.

Atrial fibrillation (AF) is one of the most common sustained arrhythmia in clinical field, which often leads to high rates of disability and mortality.Atrial electrical remodeling, structural remodeling and reconstruction of metabolic remodeling are the main causes of AF. However, these atrial remodeling enhance the vulnerability of AF induction and maintenance, thus forming a vicious circle. This article mainly introduces the research progresses of relationship between atrial fibrillation and atrial metabolic remodeling, and the intervention measures for atrial metabolic remodeling.

Abstract:
Lung microbiome has become a hot topic in microbiome research field in recent years. Lung mircrobiota is different between healthy people and lung cancer patients. Environmental factors, smoking, genetic affect the lung microbiome and the development of lung cancer. Studies have found that the composition of the lower respiratory tract microorganisms of lung cancer patients is significantly different from that of healthy people. Therefore, improving lung microbiome to inhibit lung cancer, may promote the development of innovative strategies for early prevention and personalized treatment in lung cancer. This article reviewed the research on the corrrelation between lung microbiome between lung cancer.

Abstract:
Astrocyte-neuron transformation (ANT) is an important discovery in neuroscience field in the 21st century. Astrocytes are the largest number of glial cells and originate from the same precursor cells as neurons. Their potential to transform into neurons has brought new hope for the treatment of various neurological diseases characterized by neuronal damage or loss. There have been many successful experiments on the transformation of astrocyte-neuron in vitro, but the exploration in vivo is just beginning. Transcription factors, drugs, and microRNAs have been found to induce this transformation in animal models. Meanwhile, astrocyte-neuron transformation can be induced by pathological conditions such as Alzheimer’ s disease, spinal cord injury, focal stroke, and Parkinson’ s disease. In this paper, we systematically review the current status of studies on the transformation of astrocytes into neurons in normal and related disease animal models, and sort out the possible mechanisms of astrocytes’ reprogramming, neuronal transformation, then supplement and integration into damaged neural circuits.

MiRNA-145 is down-regulated in most malignant tumors of the digestive system and is widely considered as a tumor suppressor that can inhibit the growth of different tumor cells by acting on various cytokines and targeting various genes in multiple pathways, it can significantly reduce the proliferation of tumor cells, improve the sensitivity of tumor cells to chemotherapeutic drugs, and then exert its anti-tumor effect. In this paper, the expression and role of miRNA-145 in malignant tumors of digestive system were reviewed.

Reliability and Validity of A Questionnaire for International Students in China with Educational Service Satisfaction in English Version-Taking Kunming Medical University as A Case

Abstract:
  Objective   To evaluate the reliability and validity of a questionnaire for International Students in China with Educational Service Satisfaction English version (ISCESSE).   Methods   Item and performance indicators were selected after expert groups discussion and preliminary experiments to construct the ISESSE. Cronbach's α coefficient and split-half reliability were used to evaluate the reliability as well as composite reliability. Exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) were used to evaluate the validity.   Results   A total of 112 students were enrolled. The Cronbach's α coefficient, split-half reliability of the scale were 0.961, 0.907. The composite reliability of 3 dimension was 0.896、0.912、0.925, respectively. The Cronbach's αcoefficient of 3 dimension with 24 items was 0.926、0.935、0.917, respectively. EFA showed the cumulative variance proportion were 53.7%, 62.9%, and 68.9%. The correlation coefficients of each dimension and the total questionnaire ranged from 0.803 to 0.935 (P < 0.05). CFA showed that the goodness of fit index (such as χ2, RMSEA) indicated good model fitting and good structure validity. Except for one less than 0.7, the standardized parameters between each item and dimension were all close to or higher than 0.8, which also suggested a very favorable validity.   Conclusion   The established ISESSE is of good reliability and validity and could be used to evaluate the international students' Educational Service Satisfaction in Kunming Medical University.
Investigation on Sleep Disorders in Medical Students in An University during COVID-19 Pandemic

Abstract:
  Objective   To analyze the incidence and related factors of sleep disorders in medical students in an university during COVID19 pandemic.   Methods   In March 2020, the Pittsburgh sleep quality index (PSQI) and self-designed questionnaire were used to investigate the influencing factors of sleep quality of medical students in a university.   Results   During COVID19 pandemic, the incidence of sleep disorders in medical students in a university was 9.57%, and boys were more likely to occur than girls, P = 0.008. There was no significant difference in the incidence of sleep disorders among students of different grades, majors, educational levels and places of origin (P > 0.05). Irregular work and rest led to the disorder of biological clock, bad family sleep environment and emotional frustration had a significant impact on the occurrence of sleep disorders of medical students during the epidemic period, P < 0.05. In summary, the overall incidence of sleep disorders among medical students at Covid19 was not high, and students with sleep disorders could be targeted for guidance based on influencing factors.   Conclusion   During COVID19 pandemic, the incidence of sleep disorders is not high, and targeted guidance can be given to students with sleep disorders according to the influencing factors.

  Objective   To discuss the effects of continuing nursing based on structure-process-results 3d quality evaluation model in patients with coronary artery bypass grafting.   Methods   We selected 168 perioperative patients who underwent CABG from August 2018 to December 2019 as the research object, and used the random number table method to divide them into two groups, each group of 84 cases. The control group was given regular continue nursing, the intervention group was given continuing nursing based on structure-process-results 3d quality evaluation model. After intervention, the“Chinese cardiovascular patients quality of life assessment questionnaire”(CCQQ)score, medication compliance score and the incidence of major cardiovascular and cerebrovascular events (MACCE)were compared between the two groups.   Results   The CCQQ score, medication compliance score and the incidence of MACCE were higher in the intervention group than in the control group, with statistically significant differences (P < 0.05).   Conclusion   Donabedian structure-process-results continuous nursing constructed with three-dimensional quality evaluation model is helpful to improve the self-management efficacy and quality of life of patients after CABG, and is an effective way to improve the quality of continuous nursing.
Construction of the Body Restraint Assessment Scale for Patients in ICU of Tumour Hospital by Delphi

Abstract:
  Objective  To develop the body restraint assessment scale for patients in ICU of tumour hospital by Delphi and Optimal Sequence Diagram, to provide the basis for the develop ICU body restraint nursing guidelines and standard.  Methods  Literature review and semi-structured interviews were used to form an item pool. Delphi was used to select 26 experts from Grade 3A hospitals in Guangzhou, Tianjin, Changsha, Chengdu and Kunming to carry out two rou-nds of consultionto determine the items. The weight result of items determin-ed by Optimal Sequence Diagram was assigned score combined with clinical practice, to form the final scale.  Results  Both of the two rounds of the positive factors that experts determined were 100%. The authority factors were 0.916 and 0.910 respectively. The coo-rdination coefficients by experts' opinion were 0.309 (χ2 = 104.379, P < 0.001)and 0.349 (χ2 = 81.687, P < 0.001), The top five weight coefficients were delirium, degree of sedation-agitation, risk of suicide, pain and cooperation, which eventually formed a body constraint assessment scale containing 10 first itemsand 40 level items in tumor hospital ICU.  Conclusion  The body restraint assessment scale of patients in ICU of tumor hospital has good reliability and validity, which can be used as a effective tool for evaluating the body constraint of patients in ICU of tumor hospital.

Influence of DC-Chol Cationic Liposome Adjuvant on the Immune Effect of Tetravalent Influenza Vaccine

Abstract:
  Objective  To study the immunogenicity of the tetravalent influenza vaccine modified by cationic liposome DC-Chol.  Methods  DC-Chol liposomes were prepared by thin film dispersion-freeze-thaw- freeze-drying method, and the encapsulation rate was determined. The mice were divided into seven groups, DC-Chol liposome vaccine group (250 µg/mouse, 500 µg/mouse, 750 µg/mouse, 900 µg/mouse), PBS group, vaccine stock solution group, neutral liposome group (n = 3)Determine the optimal dose of DC-Chol liposomes by MTT method. The mice were divided into 4 groups, DC-Chol liposome group, vaccine stock solution group, neutral liposome group and PBS control group. Mice were immunized through the peritoneum and sacrificed on the 7, 14, and 28 days. The proliferation of mouse lymphocytes was detected by MTT method and T lymphocyte surface labeling experiment to study its immunogenicity.  Results  The MTT experiment showed that the optimal dose of DC-Chol modified liposomes was 500 μg/rat (P < 0.05); T lymphocyte surface labeling experiments showed that DC-Chol cationic liposomes significantly increased the number of splenic lymphocytes (P < 0.05).  Conclusion  Compared with neutral liposomes, DC-Chol cationic liposomes as an adjuvant of tetrovalent influenza vaccine has good immune enhancement effect.
Antibacterial Effect of Slightly Acidic Electrolyzed Water on Enterococcus faecalis Biofilm in Root Canal in Vitro

Abstract:
  Objective  To study the antibacterial effect of slightly acidic electrolyzed water (SAEW) as root canal irrigation on Enterococus faecalisE. faecalis) biofilm in root canal in vitro.  Methods  E. faecalis biofilms were cultured in the root canals of isolated teeth and washed with SAEW at concentrations of 50 mg/L and 60 mg/L for 1 min, 3 mins, and 5 mins, respectively, the 1% NaClO solutions was used as positive controls. The field emission scanning electron microscope (FE-SEM) was used to observe E. faecalis biofilm morphological changes, the confocal laser scanning microscope (CLSM) combined with Live/Dead fluorescence staining was used to observe the E. faecalis biofilms and analyze the proportion of dead bacteria in the biofilms.  Results  FE-SEM observation: 50 mg/L SAEW, 60 mg/L SAEW, 1% NaCl Osolutions with increasing washing time, E.faecalis in the biofilm decreased, the surface of the was rough, and the cells shrank. Confocal laser scanning microscope (CLSM) combined with Live/Dead fluorescence staining observation: 50 mg/L SAEW, 60 mg/L SAEW, 1% NaClO solutions with increasing washing time, the red fluorescence intensity gradually increased, and the proportion of dead bacteria gradually increased (P < 0.05), the sterilization ratio of 1% NaClO to E. faecalis at the time point of 1 min, 3 min, and 5 min was significantly higher than 50 mg/L SAEW, 60 mg/L SAEW (P < 0.05).  Conclusion  Slightly acidic electrolyzed water has a certain antibacterial effect on strains in E. faecalis biofilm, but the effect is lower than 1% NaCIO solutions.
miR-490-3p Suppresses EMT of SW1990 Pancreatic Cancer Cells

Abstract:
  Objective  To elucidate the role of miR-490-3p in regulating HMGA2 expression and affecting epithelial-mesenchymal transition (EMT) of SW1990 cells.  Methods  miR-490-3p expression was measured in HPDE and SW1990 cell lines by Real-time PCR. SW1990 cell lines were cultured in vitro and divided into four groups, including inhibitor-Negative control (inhibitor-NC), miR-490-3p inhibitor, mimic- Negative control (mimic-NC) and miR-490-3p mimic group, and the transfection efficiency was verified by RT-qPCR. After 48 h of transfection, CCK8 assay, wound healing assay, Transwell invasion assay andflow cytometric were used to detect the effects of malignant biological characteristics such as proliferation, migration, invasion and apoptosis of SW1990 cell linerespectively. Western blot was used to detect the expression of E-cadherin and N-cadherin. Meanwhile, database predicted the target genes of miR-490-3p, and dual luciferase assay confirmed the targeted relationship between miR-490-3p and HMGA2, western blot was used to detect the expression of HMGA2.  Results  1) The expression of miR-490-3p was downregulated in SW1990 cells (P = 0.215). 2) Overexpression of miR-490-3p significantly inhibited the proliferation (P < 0.0001), migration (P < 0.0001) and invasion (P < 0.0001) of SW1990 cell line and prompted apoptosis (P < 0.0001); In contrast, downregulation of miR-490-3p significantly promoted the proliferation (P < 0.0001), migration (P < 0.0001) and invasion (P < 0.0001) of SW1990 cell line and inhibited apoptosis (P < 0.0001). 3) Upregulation of miR-490-3p significantly decreased the expression of N-cadherin (P < 0.0001) and increased E-cadherin (P < 0.0001) expression in SW1990 cell line. In contrast, downregulation of miR-490-3p significantly upregulated (P < 0.0001) the expression of N-cadherin, decreased E-cadherin (P < 0.0001) expression. 4) HMGA2 was a target gene of miR-490-3p.  Conclusion  These findings indicate that miR-490-3p acts as a tumor suppressor during the process of EMT through targeting HMGA2, suggesting miR-490-3p is a potential new diagnostic and therapeutic target for the treatment of pancreatic cancer.
