Chemical Compounds Isolated from Cremastra Appendiculata and Their Bioactive Activity

Abstract:
  Objective  To study the chemical compounds from the medicinal plants of Cremastra appendiculata and find its antitumor bioactive compounds.   Methods  The compounds were extracted by alcohol (95%) and isolated by column chromatography on silica gel and Sephadex LH-20. Their structures were identified by spectroscopic analysis (1H NMR, 13CNMR and EIMS). The antitumor activity of the compounds was studied by MTT assay in vitro.   Results  Twelve compounds were obtained and identified as bisbenzopyran (1), (22E)-ergosta-6, 22-dien-3β, 5α, 8α-triol (2), 3β-hydroxycholesta-5-ene (3), β-sistosterol (4), pinoresinol (5), 5, 4′-dihydroxy-7- (4-hydroxybenzoyl) -3′- methoxyflavone (6), 4-methoxy-2, 3, 7- trihydroxyphenanthrene (7), 2-hydroxy-4, 7-dimethoxyphenanthrene (8), 4, 4′-dimethoxy-[1, 1′-biphenanthrene]- 2, 2′, 7, 7′-tetrol (9), 4, 7, 4′, 9′-tetramethoxy-[1, 1′-biphenanthrene] 2, 2′, 7, 7′-tetrol (10), Bavachinine (11), 3-hydroxyphenpropionic acid- (2′-methoxy-4′- carboxy-phenol) ester (12); . The results of anti-tumor activity test of compounds 7~12 showed that 9 and 10 showed good inhibitory activity on MCF-7/S cell line.  Conclusions   Compounds 2, 3, 5, 7, 10 and 12 were isolated from this plant for the first time. IC50 of 9 and 10 on MCF-7/S cell lines were 2.16 and 5.09 respectively μmol/L.
Effect of TLR4/MyD88/NF- κB Signal Pathway on the Hippocampus of Methamphetamine-dependent CPP Rats

Abstract:
  Objective  To study the effect of TLR4/MyD88/NF- κB signal pathway on the hippocampus of methamphetamine (MA) dependent conditioned place preference (CPP) rats, and inhibition of Toll like receptor 4 (TLR4) by specific inhibitor TAK-242, thereby reducing MA induced hippocampal neuroinflammation.   Methods  We established a model of MA (10 mg/kg, ip, 14 d) dependent CPP in rats. Rats were randomly divided into 4 groups: normal saline group, MA group, TAK-242 group, and MA+TAK-242 group. TAK-242 group and MA+TAK-242 group were intraperitoneally injected with inhibitor TAK-242 (3 mg/kg), and MA+TAK-242 group was intraperitoneally injected with MA (10 mg/kg) one hour later. Protein and mRNA expression of TLR4, MyD88, TRAF6, IκB- α, p-IκB- α, NF- κ B p65, p-NF-Κb p65 in hippocampus of MA dependent CPP rats were tested by Western Blot test and fluorescent quantitative PCR, respectively.   Results  Compared with normal saline group, the expression of protein and mRNA of TLR4, MyD88, TRAF6, NF- κBp65 in MA group increased (P < 0.001 or P < 0.01).The protein of IκB- α and mRNA expression of lactamase decreased (P < 0.01), the expression of p-IκB- α and p-NF- κBp65 increased (P < 0.01 or P < 0.05). Compared with MA group, the protein and mRNA expression of TLR4, MyD88, TRAF6, NF- κBp65 in MA+TAK-242 group decreased (P < 0.001, P < 0.01 or P < 0.05) . The expression of IκB- α protein and mRNA was increased (P < 0.01), the expression of p-IκB- α and p-NF- κBp65 decreased (P < 0.01 or P < 0.05).   Conclusions  MA dependency can be achieved by activating TLR4/MyD88/NF- κ B signal pathway, which can induce neuroinflammation on the hippocampus of methamphetamine-dependent CPP rats. The use of specific TLR4 inhibitors can attenuate MA induced neuroinflammation.
Effect of Resveratrol on Metastasis of Nasopharyngeal Carcinoma and Its Related Mechanism

Abstract:
  Objective  To observe the effect of resveratrol on metastasis of nasopharyngeal carcinoma and explore its molecular mechanism.   Methods   (1)The inhibiting ability of resveratrol on nasopharyngeal carcinoma cells CNE2 was evaluated by CCK8 assay at different concentrations and times (24 h/40 μmol/L, P < 0.01; 24 h/80 μmol/L and above, P < 0.001; 36 h/20 μmol/L, P < 0.05; 36 h/40 μmol/L and above, P < 0.001; 48 h/10 μmol/L, P < 0.01, 48 h/20 μmol/L and above, P < 0.001). (2) Resveratrol of different concentrations treated CNE2 cells for 24 hours: the migration and invasion ability of CNE2 cells were detected by Transwell experiment; Western Blot was used to explore the expression of E-cadherin, Vimentin, Hsp27, Ezrin and MMP; the nuclear translocation of NF-κB/p65 in CNE2 cells were detected by immunofluorescence assay increased the expression of E-cadherin (20, 40 μmol/L, P < 0.01; 80 μmol/L, P < 0.001), inhibited the expression of MMP9 and MMP2 (MMP2, 20 μmol/L, P < 0.05; 40, 80 μmol/L, P < 0.001; MMP9, 10, 20 μmol/L, P < 0.05; 40, 80 μmol/L, P < 0.01). (3)The LN metastasis experiment was constructed by injecting CNE2 cells into the left hind footpad of BALB/c nude mice. HE staining and immunohistochemistry staining was used to observe the effect of different doses of resveratrol gavage on the metastasis of nasopharyngeal carcinoma cells to popliteal lymph nodes Hsp27 and Ezrin (Hsp27, 20, 40 μmol/L, P < 0.05; 80 μmol/L, P < 0.01; Ezrin, 40 μmol/L, P < 0.05; 80 μmol/L, P < 0.01).   Results   (1) CCK8 experiments showed that resveratrol had a toxic effect on nasopharyngeal carcinoma cell CNE2, and its toxicity was dose and time-dependent. (2) Transwell test found that resveratrol could inhibit the migration and invasion of nasopharyngeal carcinoma cell CNE2 in a dose and time-dependent manner. Western Blot assay showed that resveratrol significantly inhibited the expression of MMP9 and MMP2, and increased the expression of E-cadherin. Resveratrol inhibited the phosphorylation levels of Ezrin and Hsp27. Immunofluorescent experiments showed that resveratrol dose-dependently inhibited nuclear translocation of NF-κB/p65. (3) Animal experiments showed that resveratrol gavage treatment had no effect on the body weight of tumor-bearing nude mice, but significantly reduced the degree of swelling of the popliteal lymph nodes. Histological examination showed that resveratrol gavage significantly inhibited the metastasis of nasopharyngeal carcinoma cells from the subcutaneous tumor formation site to the popliteal lymph nodes in a dose-dependent manner.   Conclusions  Resveratrol can inhibit the metastasis of CNE2 cells in a time and dose-dependent manner. The mechanisms of resveratrol inhibiting the invasion and migration of CNE2 cells may relate to inhibiting the nuclear translocation of NF-κB/p65 by decreasing the phosphorylation value of Ezrin and Hsp27, and up-regulating the level of E-cadherin and down-regulating the expression of MMP2 and MMP9.
Remineralization Effect of Caesalpinia Sappan L. on Artificial Demineralization of Enamel Caries

Abstract:
  Objective  To observe the effects of Caesalpinia Sappan L. ethanol extract, ethyl acetate, n-butanol and water extract on artificial demineralization enamel caries.   Methods  Bovine incisors were made into artificial demineralized enamel caries. The effects of Caesalpinia Sappan L. ethanol extract and different extraction parts on the microhardness of enamel surface of bovine incisors were determined. The effects of different experimental groups on the surface morphology of enamel caries were observed by scanning electron microscope.   Results  Both ethanol extract and all extracts improved the surface hardness of enamel caries, and water extract had the best effect on enamel hardness recovery, with the hardness recovery rate reaching 66.9% (P < 0.05). The surface of enamel caries in each experimental group was covered with a large amount of granular sediments.   Conclusion  The ethanol extract of Caesalpinia Sappan L. and its extracts have the effect of remineralization on artificial demineralization of enamel caries.

Abstract:
  Objective   To investigate the intestinal flora in male uricase-deficient rats (Kunming-DY, KDY rats) compared with wild-type SD rats of the same sex and age.   Methods   45-day-old male uricase-deficient rats were weighed, and their blood was collected to obtain serum after they were anesthetized with uratan. Their small intestine was divided into 18 segments, and the colon was divided into 2 segments (5 cm each). The serum uric acid (SUA) was detected by phosphotungstic acid method. DNA in intestinal segments and fresh feces was extracted using DNA extraction kit. High-throughput sequencing was performed after amplification with specific 16S rDNA primers.   Results   At 45 days of age, the body mass of KDY rats decreased (P < 0.05), but the SUA level increased significantly ( P < 0.05). The intestinal microbiota diversity in KDY rats decreased ( P < 0.05), but their fecal microbiota diversity did not change significantly ( P > 0.05). The abundance of Lactobacillus in intestinal tract and feces of KDY rats was significantly increased (P < 0.05).   Conclusion   Compared with wild-type SD rats, the intestinal microbiota diversity of KDY rats decreased, but the abundance of Lactobacillus in intestinal tract and feces increased significantly.
Effects of Different Doses of Gastrodin on TLR4/MyD88/NF-κB Inflammatory Signal Pathway in Hippocampus of Methamphetamine Dependent CPP Rats

Abstract:
  Objective   To investigate the effects of different dosages of gastrodin onTLR4/ MyD88 / NF-κB signal pathway in the hippocampus of methamphetamine (meth)dependence rats.   Methods  The conditioned place preference (CPP) model of meth was established, meth (10mg/kg) was administered (i.p, 14 d), and then, different dosages (10, 30, 100 mg/kg) of gastrodin were used to intervene in 14 days. Western blotting and RT-qPCR were used to explore the expression level of protein and mRNA of key factors in TLR4/ MyD88 / NF-κB signal pathway in hippocampus.   Results  Gastrodin decreased the protein and mRNA expression of TLR4, MyD88, TRAF6, NF-κBp65 in a dose-dependent manner (P < 0.001, P < 0.01 or P < 0.05), and the protein expression of p-NF-κB p65 was reduced with showing a significant dose-response relationship (P < 0.01 or P < 0.05). Gastrodin increased the protein and mRNA expression of IκB-α with showing a significant dose-response relationship (P < 0.01 or P < 0.05). Gastrodin reduced the protein expression of p-IκB-α (P < 0.01).   Conclusion  Gastrodin intervention has a protective effect against hippocampal neuroinflammation in methamphetamine-dependent CPP rats, and is closely related to the function of TLR4/MyD88/NF-κB signal pathway.
Analysis of Chlamydia Trachomatis Genotype among Female Sex Workers in Yunnan Province

Abstract:
  Objective  To investigate the serovar distribution and epidemiological characteristics of Chlamydia trachomatis (CT)in female sex workers (FSWs) from Yunnan Province.   Methods  A survey of Chlamydia trachomatis in the genital tract was conducted in 2020 in conjunction with HIV sentinel surveillance, and 374 nucleic acid-positive samples of Chlamydia trachomatis in the genital tract were collected. The OmpA gene region of CT was amplified by nested polymerase chain reaction (PCR) and analyzed.   Results  Seven genotypes were detected in 173 samples successfully genotyped, among which genotype E (24.9%, 43/173), genotype F (21.4%, 37/173), genotype J (19.1%, 33/173), genotype D (17.9%, 31/173) were the main genotypes, and the others included genotype G (9.3%, 16/173), genotype K (4.6%, 8/173)and genotype H (2.9%, 5/173). No statistically significant differences were found among the different genotypes in characteristics such as survey area, household registration, whether it was a border state (city), age, ethnicity, education level, marital status, sample source, and place class.   Conclusion  Multiple subtypes of CT exist in FSWs in Yunnan Province, and their distribution does not differ in terms of major demographic characteristics. This study may provide guidance for further control of CT transmission and provide basic information for future treatment and vaccine studies.
Possible Roles of MiR-219-5p Targeting SOX5 in the Development of Oral Cancer

Abstract:
  Objectives  To explore whether the expressions of miR-219-5p and / or SRY-related high-mobility-group box 5 (SOX5) in oral squamous cell carcinoma cells are differ from normal cells. We also aim to confirm the transcription regulation in SOX5 by miR-219-5p.   Methods  Human oral squamous cell lines SCC4 and SCC9 were employed. The expression levels of miR-219-5p and SOX5 were detected in cancer cells (indicated by RT-qPCR and Western blot). Genetic analysis and luciferase assay were employed to verify whether miR-219-5p targets SOX5 by binding to the specific region of 3'-UTR.  Results  Our data showed that high miR-219-5p and low SOX5 levels were detected in vitro SCC4 and SCC9 cells (P < 0.05). miR-219-5p was negatively correlated with SOX5 expression. Genetic analysis and luciferase assay revealed that miR-291-5p regulated SOX5 expression by targeting the 3'untranslated region (3'-UTR) of the SOX5 mRNA.   Conclusions  This study demonstrates that miR-219-5p is involved in the development and prognosis of oral cancer by targeting the expression of SOX5, and can be a novel candidate for clinical prognosis and/or therapy.
Correlation between KRAS Gene Polymorphism and Non-small Cell Lung Cancer in Yunnan Han Population

Abstract:
  Objective   To investigate the correlation between KRAS gene polymorphism and the occurrence and development of non-small cell lung cancer in Yunnan Han population.   Methods   In this study, 455 patients with non-small cell lung cancer and 391 healthy controls were selected as the research objects. Three single nucleotide polymorphism (SNP) sites rs12587 (G > T), rs12245 (A > C), rs1137282 (A > G) in the 3′UTR region of KRAS gene were identified by Taqman probe genotyping. According to the typing results, the correlations of alleles, genotypes and haplotypes with the occurrence, pathological types (squamous cell carcinoma, adenocarcinoma) and clinical stages (I+II, III+IV) of non-small cell lung cancer were analyzed.   Results   The distribution frequency of rs12587 (G > T) allele G in the non-small cell lung cancer group was significantly higher than that in the control group (P = 0.008, OR = 1.365, 95%CI 1.086~1.716). It was carried in the dominant mode Individuals with the T allele (G/T+T/T) had a significantly lower risk of developing non-small cell lung cancer (P = 0.011, OR = 0.70, 95%CI 0.53~0.92). Case stratification analysis found that the allele and genotype frequencies of the rs12587 (G > T) locus were significantly different between the squamous cell carcinoma group and the control group (P < 0.001, P = 0.001); Individuals with the T allele (G/T+T/T) had a significantly lower risk of lung squamous cell carcinoma (P < 0.001, OR = 0.45, 95%CI 0.30~0.68). There was no significant difference in the distribution frequency of the rs12245 (A > C) locus allele and genotype between the non-small cell lung cancer case group and the control group (P > 0.05); Individuals (A/T+A/A) had a significantly lower risk of developing non-small cell lung cancer (P = 0.028, OR = 0.73, 95%CI 0.55~0.97). Case stratification analysis found that the allele and genotype frequencies of the rs12245 (A > C) locus were significantly different between the squamous cell carcinoma group and the control group (P = 0.003, P = 0.001); Individuals with genotype A/T had a significantly lower risk of lung squamous cell carcinoma (P < 0.001, OR = 0.43, 95%CI 0.28~0.67).   Conclusions   The G allele of the SNP site rs12587 (G > T) in the 3′UTR region of KRAS gene may be a risk factor for the occurrence of non-small cell lung cancer and squamous cell carcinoma in Yunnan Han population. SNP rs12245 (A > C) allele A may be a protective factor for the occurrence of non-small cell lung cancer in Yunnan Han population.
Study on Osteogenic Ability of Dental Pulp Stem Cells at Different Concentrations

Abstract:
  Objective   To explore the optimal osteogenic ratio of Dental pulp stem cells (DPSC) with different concentrations in quantitative Bio-Oss bone powder.   Methods   The primary cells were isolated and cultured by adherent method. The surface markers of DPSC were identified by flow cytometry, and the multidirectional differentiation potential of DPSC was identified by osteogenesis and adipogenesis. DPSC was transfected with GFP lentivirus, and GFP-DPSC was screened with puromycin. The cells in groups 1×105, 2×105, 4×105 and 8×105 were inoculated with 0.05 g bone powder and cultured for osteogenesis, while the control group did not add bone powder. Alkaline phosphatase activity was measured on day 3 and day 7. On the 7th day of culture, the morphological changes of the cells in bone powder were observed under scanning electron microscope.   Results   The DPSC was derived from mesenchymal stem cells with high expression of related surface markers and could differentiate into adipogenic and osteogenic cells. GFP was successfully transfected into DPSC, and alkaline phosphatase activity was detected on the 3rd and 7th day, and the experimental group was higher than the control group, and the difference was statistically significant (P < 0.05). Scanning electron microscopy showed that DPSC crawled on the surface of bone powder, and some cells extended pseudopodia, and the cells of 4×10 5 and 8×105 groups were more dense.   Conclusions   The combined culture of 4×105-8×105 DPSC and 0.05 g Bio-Oss bone powder has a good osteogenic effect, which can shorten the osteogenic cycle. If the cells are inoculated too much, the osteogenic effect is inhibited, resulting in the waste of stem cells.

Application of NICS Testing at Different Stages in PGT Treatment of Patients with Reciprocal Translocation

Abstract:
  Objective  To study the application of NICS of different stages in the PGT treatment for patients with balanced translocation.   Methods  The parameters analyzed included testing results (tested successfully and rested failed) and CNVs results (normal and abnormal); the samples were divided into D3 NICS, D5 NICS, whole embryos and biopsied cells (embryo /TE) groups according to the samples types. The testing failure rate and CNVs results distribution were compared among the groups.   Results  There were significant differences between D3 NICS group and embryo/TE group on testing failure rate and CNVs results distribution (P < 0.001/P < 0.01), while no significant difference was shown in results between D5 NICS group and embryo /TE group (P > 0.05).  Conclusions  Compared with D3 NICS, D5 NICS has more advantages in the application of balanced translocation embryos diagnosis. As a non-invasive test, D5 NICS is expected to replace the traditional biopsy technique in treatment of balanced translocation patients with the progress of manipulation methods and sequencing technology.
Correlation of Metastatic and Prognostic Factors in 318 Cases of Primary Gastric Cancer

Abstract:
  Objective   To explore the influencing factors of metastasis and prognosis in patients with primary gastric cancer (GC), and trying to find out the clinical indicators for early prediction of metastasis.   Methods   A retrospective study was conducted on the clinicopathological data of 318 patients with primary GC who were admitted to Yunnan Cancer Hospital from January 2018 to December 2021 and were divided into 2 groups: metastasis group (213 cases) and non-metastasis group (105 cases) according to whether metastasis occurred. The relationship between clinicopathological factors and GC metastasis and prognosis was analyzed, and the diagnostic efficacy of these factors for GC metastasis was explored.   Results   (1) The median overall survival (OS) and average progression-free survival (PFS) were 27 months and 22.16 months respectively in the metastasis group. While the median OS and average PFS in the non-metastasis group were 45 months and 41.98 months respectively. The differences in OS and PFS between these two groups were statistically significant (P < 0.0001). (2) Multivariate analysis suggested that infiltration depth > mucosal layer, no chemotherapy and carbohydrate antigen-125 (CA-125)≥14.4KU/L were independent risk factors for GC patients with metastasis ( P < 0.05). (3) The drawing of receiver operating characteristic curve (ROC curve) suggested that the combination of infiltration depth, prealbumin and CA-125 had more accurate sensitivity and specificity for predicting GC patients’ metastasis than the combination of double single or indexs. (4) Log-rank test and COX multivariate analysis suggested that total chemotherapy cycle < 8 cycles and CA-125≥14.4KU/L were independent risk factors for OS in patients with metastasis. The independent risk factors for PFS in patients with metastasis were no surgical treatment in the primary lesion, combined with metastasis at initial diagnosis, combined with distant metastasis during the course of disease and CA-125≥14.4KU/L, with statistically significant differences ( P < 0.05).   Conclusions   The prognosis of GC patients with metastasis is truly poor, clinical attention should be paid more attention to the early detection, diagnosis and treatment. The combination of multiple indexs has been proven to be more accurate in sensitivity and specificity for the diagnosis of GC metastasis, suggesting that GC patients should be actively evaluated and individualized treatment should be developed to diagnose and treat after metastasis as soon as possible, to provide a theoretical basis for improving patients’ prognosis, prolonging survival and improving life treatment.

  Objective  To provide a practical basis for the field of elderly burn injury prevention and treatment by retrospectively analyzing the clinical data of elderly burn patients.   Methods  We retrospectively analyse the medical records of elderly burn patients who met the inclusion criteria admitted to the Second Affiliated Hospital of Kunming Medical University (hereafter referred to as the author’s unit) from January 2014 to December 2020, including gender, age, chronic disease, place of residence, place of injury, prehospital emergency trauma treatment, route of medical treatment, cause of injury, season of onset, burn site, burn area, and depth of burn. The data were analyzed by cardinality fit to optimize the data. The data were subjected to chi-square goodness-of-fit test, c2 test, linear association, Fisher’s exact probability test, rank sum test, one-way and multi-way Cox regression analysis, and multiple linear regression analysis.  Results   (1) A total of 471 patients met the inclusion criteria, there was an overall increasing trend of the proportion of elderly patients to inpatients in the same period with the increase of years (P < 0.01). The male to female ratio was 1.75∶1 (300/171), significantly more males than females (P < 0.001); the age distribution of patients was 69 (64, 77) years. In total, 247 patients (52.44%) had combined chronic diseases, and the common chronic diseases were hypertension 141 times (33.41%), diabetes mellitus 71 times (16.82%), old cerebral infarction 51 times (12.09%) in that order. (2) There were significantly more patients living in rural areas than in urban areas, and significantly more burns at home than in public places (all P < 0.05). There were 381 (80.89%) patients with untreated wounds in the prehospital emergency; only 34 were treated by 120; the common causes of injury were flame burns 274 (58.17%), hot liquid burns 155 (32.91%) in that order. The most frequent seasons were winter (168 cases, 35.67%) and spring (130 cases, 27.60%). The common burn sites were lower extremity 350 cases (74.31%) and upper extremity 230 cases (48.83%) in that order. The total burn area ≤10% TBSA was 272 cases (57.75%), of which 170 cases (36.09% of the total) were combined with third-degree burns. The severity of burns, in descending order, was moderate in 234 cases (49.68%), mild in 102 cases (21.66%), severe in 80 cases (16.99%), and extra-severe in 55 cases (11.68%). (3) In this group, 288 patients (61.15%) were finally cured and improved, 164 (34.82%) were automatically discharged, and 19 (4.03%) died. The age, total burn area, third-degree burn area, inhalation injury, post-injury complications, and combined chronic diseases were independent risk factors for the death in elderly patients (all P < 0.05). The duration of hospitalization was 11 (5, 22) d, and the hospitalization cost was 14320.62 (6347.92, 39638.06) yuan, which was influenced by the duration of hospitalization, total burn area, third-degree burn area, treatment modality, and complications after taking its logarithm because of the skewed distribution of hospitalization cost.   Conclusions  Elderly burns often occur in rural homes in winter and spring, with a male predominance, and flame burns and hydrothermal burns are the main causes. Patients are mostly young elderly people, and small-area deep burns are common, mainly concentrated in the lower and upper limbs, with high treatment costs. Most families lack knowledge of first aid for burns, and 120 emergency personnel are relatively inadequate regarding emergency treatment of burn trauma. Specific emergency prevention and treatment strategies for elderly burns should be optimized, education on first aid for burns and prevention of chronic diseases should be increased, and social support should be strengthened to reduce the incidence and morbidity of elderly burns and reduce economic losses.
Correlation between Serum RANKL,OPG,FGF23 and Renal Function and Calcium-phosphorus Metabolism in Patients with Chronic Renal Disease

Abstract:
  Objective  To analyze the correlation between serum receptor activator of NF- κ B ligand (RANKL), osteoprotegerin (OPG), fibroblast growth factor 23 (FGF23) with renal function and calcium and phosphorus metabolism in patients with chronic kidney disease (CKD).   Methods  A total of 62 patients who met the diagnostic criteria of CKD in our hospital from January 2021 to February 2022 were enrolled as the research group, and 26 healthy persons were included in the control group. Renal function and calcium and phosphorus metabolism were measured, and serum levels of RANKL, OPG and FGF23 were measured by enzyme-linked immunosorbent assay (Elisa).   Results  The serum levels of RANKL, OPG and FGF23 in the research group were significantly increased, and the serum levels of RANKL, OPG and FGF23 in the CKD5 stage group were higher than the CKD2-3 stage group and the CKD4 stage group. Correlation analysis showed that the serum levels of RANKL, OPG and FGF23 were negatively correlated with eGFR , serum RANKL levels were positively correlated with PTH, serum FGF23 levels were positively correlated with PTH and P levels, and negatively correlated with Ca.   Conclusions  The levels of serum RANKL, OPG and FGF23 are increasing with the progression of CKD disease. Serum RANKL and FGF23 are sensitive indicators for early detection of calcium and phosphorus metabolism disorders in patients with CKD.
Value of Binary Logit Regression Model Based on FNA-Tg and Serum Tg for Determining Lymph Node Metastasis or Recurrence of Thyroid Cancer

Abstract:
  Objective  To establish a prediction model for lymph node metastasis and recurrence of thyroid cancer by analyzing patients’ lymph node FNA-Tg values, serum Tg values, and their ratios by binary logit regression.   Methods  1. 64 patients with preoperative differentiated thyroid cancer with suspected lymph node metastasis were collected. 2. The diagnostic value of FNA-Tg value, serum Tg value, and their ratios for the differentiation of benign and malignant lymph nodes were initially evaluated by the ROC curve. 3. The diagnostic value of the above three indices was further evaluated by binary logit regression analysis and a prediction model was established.   Results  1. ROC curve: the AUC value corresponding to lymph node FNA-Tg was 0.998 (95% CI 99.27%-100.34%), with a cut point value of 0.82; the AUC value corresponding to serum Tg was 0.824 (95% CI 70.64%-94.14%), with a cut point value of 18.27; the AUC value corresponding to lymph node FNA-Tg/serum Tg The AUC value of FNA-Tg/serum Tg in lymph nodes was 1.000 (95% CI 100.00%-100.00%), and the cut point value was 0.461. 2. Preoperative FNA-Tg, serum Tg and their ratios have good diagnostic value in determining the benignity and malignancy of thyroid nodules. 2. The binary logit regression model (stepwise method) formula based on serum Tg can better predict the benignity and malignancy of thyroid nodules.
Evaluation of Cardiotoxicity in Trastuzumab Treatment of Breast Cancer Using Three-dimensional Speckle Tracking Echocardiography

Abstract:
  Objective  To investigate the application of three-dimensional speck tracking echocardiography in the evaluation of cardiotoxicity in trastuzumab treatment of breast cancer.   Methods  A total of 65 breast cancer patients treated with trastuzumab in the First Affiliated Hospital of Kunming Medical University from November 2020 to July 2022 were selected. Conventional echocardiography and three-dimensional speckle tracking technique (3D-STI) were performed 3, 6, 9, and 12 months after treatment. The differences in cardiac parameters at different time points were observed. The parameters included left ventricular ejection fraction (LVEF), left atrial volume index (LAVI), early diastolic mitral flow velocity/early diastolic mitral annular flow velocity (E/Em), early diastolic mitral annular flow velocity/late diastolic mitral annular flow velocity (Em/Am), early diastolic mitral annular flow velocity (Sm), and early diastolic mitral annular flow velocity (E/Em). Peak E), peak late diastolic mitral flow velocity (peak A), early diastolic mitral annular velocity (Em), late diastolic mitral annular velocity (Am), left ventricular torsion (LVtw), global longitudinal strain (GLS), global circumferential strain (GCS), global radial strain (GRS), global area strain (GAS).   Results  Compared with those before treatment, LAVI, E/Em and Am increased 3 months, 6 months, 9 months, and 12 months after treatment, while Em/Am and Em decreased. LAVI and E/Em were positively correlated. Compared with before and after treatment, LVEF, GLS, GRS, LVtw, GCS and GAS decreased 3, 6, 9 and 12 months after treatment (P < 0.05).   Conclusions  The application of three-dimensional echocardiographic speck tracking technology in the treatment of breast cancer patients with trastuzumab has an ideal effect, which can judge the cardiotoxicity more early and accurately, provide a reference for early treatment, and has value in improving prognosis and clinical outcome.

Abstract:
  Objective   To explore the lipid metabolism levels and related factors in children with KD in different stages of disease, so as to provide more scientific and standardized long-term standardized treatment and health management for KD children in Yunnan Province.   Methods   A total of 400 children who were hospitalized in our hospital from January 2015 to December 2021 and met the diagnostic criteria of Kawasaki disease were collected as the observation group. They were divided into acute phase, subacute phase, recovery phase. There are four sequelae phases, and 100 children with KD are to be included in each phase; 100 healthy children who underwent physical examination in our hospital during the same period were selected as the control group. Two groups of laboratory indicators were detected, including total cholesterol (TC), triacylglycerol (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and apolipoprotein A1 (ApoA1), apolipoprotein B (ApoB100), apolipoprotein E (ApoE), lipoprotein small a (Lpa), the general demographic data were collected for univariate analysis, and the multiple stepwise regression analysis screened out the influencing factors of LDL-C in children with KD.   Results   The levels of TC, HDL-C and ApoA1 in the observation group were lower than those in the control group, while the levels of TG, LDL-C, ApoB100, ApoE and Lpa were higher than those in the control group (P > 0.05).There are abnormal blood lipid metabolism in children with KD during different courses of disease. The levels of TC, HDL-C and ApoA1 in acute phase > subacute phase > recovery phase > sequelae phase, TG, LDL-C, ApoB100, ApoE, Lpa levels in acute phase > sub-acute phase Acute stage > recovery stage > sequelae stage ( P > 0.05), in which serum HDL-C was still low in the recovery stage and the sequel stage. The serum levels of TC, HDL-C, ApoA1, TG, LDL-C, ApoB100, ApoE, and Lpa in children with KD were stable and normal at half a month, 1 month, 3 months, half a year, and 1 year after discharge, and at different time points after discharge Compared with the changes of blood lipid metabolism indexes, there was no significant difference ( P > 0.05). Multiple linear regression analysis showed that male, age > 5 years old, body mass index > 24.0 kg/m 2, ethnic minorities, and post-term infants were independent risk factors for HDL-C levels in children with KD (P > 0.05).   Conclusions   KD children with different durations of disease have abnormal blood lipid metabolism, which may have a long-term impact. Gender, age, body mass index, ethnicity and expired infants are the influencing factors of HDL-C levels in KD children. It is critical to develop targeted interventions to reduce the long-term impact of dyslipidemia in children with KD.

Abstract:
  Objective  To explore the risk factors of ovarian necrosis caused by ovarian incarcerated indirect inguinal hernia in female infants.   Methods  This retrospective study enrolled 57 female infants of incarcerated inguinal hernia (referred to as incarcerated hernia of ovary)who underwent surgery at Children’s Hospital of Kunming from June 1, 2018 to June 1, 2021. And the age, clinical characteristics, laboratory tests, whether there was ovarian torsion during operation, were analyzed. The risk factors of ovarian necrosis were identified by univariate and multivariate Logistic analysis.   Results  57 female infants were included in this study, with a median age of 46 days, a minimum age of 19 days, and a maximum age of 2 years and 5 months. All 57 children underwent surgical treatment, and all of them were confirmed to have ovarian incarceration. Univariate analysis showed that white blood cells, CRP, and ovarian torsion were associated with ovarian necrosis (P < 0.05). Multivariate analysis indicated that ovarian torsion was an independent risk factor for intraoperative ovarian necrosis (OR = 1.390, 95% CI1.056-1.829, P = 0.019).   Conclusion  Ovarian torsion is an independent risk factor for ovarian necrosis in infants with ovarian incarcerated inguinal hernia.

  Objective  In this study, the clinical, pathological and genetic data of PTC patients in The Third Affiliated Hospital of Kunming Medical University were collected, sorted out and statistically analyzed to explore the correlation between PTC gene mutations and the aggressiveness of PTC. Besides, the correlation between BRAF V600E mutation and the occult metastasis of central lymph nodes in PTC was discussed, so as to refine the precise treatment of PTC.   Methods  PTC patients who met the inclusion criteria and agreed to be enrolled in the Third Affiliated Hospital of Kunming Medical University from April 2018 to April 2022 were selected. The genetic, pathological and clinical data were collected and analyzed. (1) Each gene mutation group was used as the observation group and the wild-type was used as the control group to compare the difference between each gene mutation and wild type. (2) Multiple gene mutations were used as observation group and single gene mutations were used as control group to compare the difference between multiple gene mutation and single gene mutation. (3) In cN0 patients: BRAF V600E single gene mutation was used as observation group and without gene mutation was used as control group to analyse the correlation between the BRAF V600E and he occult metastasis of central lymph nodes in PTC .   Results  (1) BRAF V600E mutation: CLNM and N1 accounted for a large proportion and the number of central lymph node metastasis was high , TPO-Ab reduced (P < 0.05); TERT mutant: Elderly, male, lateral neck lymph node metastasis accounted for a large proportion and the number of lateral neck lymph node metastasis was high, T stage was later (P < 0.05). RET mutant: Lateral neck lymph node metastasis accounted for a large proportion and the number of lateral neck lymph node metastasis was high, T stage was late (P < 0.05). NTRK3 and ETV6 were fused together, and the results were consistent: in the mutant group, the invasion of thyroid capsule accounted for more (P < 0.05). RAS mutation: Tg-Ab was reduced (P < 0.05); TP53、CCDC6: There was no significant difference between the mutant group and the wild-type group (P ≥ 0.05). (2) Thyroid capsule invasion, lateral neck lymphatic metastasis accounted for a large proportion and more metastatic lymph nodes in the lateral neck, M stage was later in Multiple gene mutations group (P < 0.05); (3) In patients with cN0: CLNM accounted for a large proportion and the number of lateral neck lymph node metastasis was high in BRAF V600E single mutation group (P < 0.05).  Conclusions  (1) BRAF V600E mutation is associated with CLNM in PTC. (2) Multi-gene mutation shows stronger lateral neck lymph node and distant metastasis ability than single gene mutation. (3) BRAF V600E mutation is associated with the occult metastasis of the central lymph node in cN0 PTC patients.
Diagnostic Value of MSCT Combined With MRI in T Staging of Gastric Cancer

Abstract:
  Objective  To explore the diagnostic value of multispiral computed tomograpay (MSCT) combined with magnetic resonance imaging (MRI) in preoperative T staging of gastric cancer.   Methods  The preoperative imaging data of 57 patients with gastric cancer confirmed by surgery and pathology were collected. All patients were examined by MSCT and MRI within two weeks before surgery. Three methods were used for T staging: (1) MSCT, CT axial position and multiplanar reformation (MPR); (2)MRI; (3) MSCT combined with MRI. The above preoperative T staging results were compared with the postoperative pathological results to calculate the accuracy, sensitivity, specificity, positive predictive value and negative predictive value of the T staging diagnosis of gastric cancer lesions, analyze and compare the differences between the three methods of T staging diagnosis accuracy, and use Kappa value to test the consistency of the three methods of preoperative T staging diagnosis and pathological results.   Results  The Kappa values of preoperative T staging and pathological results of MSCT, MRI and MSCT combined MRI were 0.583 (moderate consistency), 0.646 (strong consistency) and 0.848 (strong consistency), respectively. The overall accuracy rates were 70.18%, 75.44% and 89.47%, respectively. Analysis and comparison showed that there were statistically significant differences in the accuracy rates of preoperative T staging diagnosis between MSCT and MRI, MSCT and MSCT combined MRI (P < 0.05). There was no statistically significant difference in the accuracy rates of preoperative T staging diagnosis between MRI and MSCT combined MRI (P > 0.05).   Conclusions  The accuracy of preoperative T staging with MRI is higher than that with MSCT. The combined use of MRI and MSCT can effectively improve the accuracy of preoperative T staging diagnosis, which is conducive to the preoperative evaluation of gastric cancer patients and the selection of treatment plans.

Abstract:
  Objective   To investigate the correlation between the expression of miR-192 in serum and left ventricular hypertrophy in patients with essential hypertension.   Methods   The clinical data of 187 patients with essential hypertension treated from June 2018 to December 2019 were selected. All patients were accepted echocardiography to obtain the indexes of LVPWd, IVST, LVEDD, LVESD, LVEDV, LVESV, LAD, LVEF and LVMI. And according to the results of echocardiography, the patients were divided into the hypertrophic group and control group. The expression of miR-192 in serum was detected by real-time fluorescence quantitative PCR. The levels of TGF-β1, type I procollagen carboxy terminal peptide (PⅠCP) and type III procollagen amino terminal peptide (PⅢNP) were detected by using ELISA.   Results   According to the results of echocardiography, 76 cases of patients had left ventricular hypertrophy (hypertrophy group), and 111 cases had no left ventricular hypertrophy (control group). Compared with the control group, LVPWT, IVST, LVEDD, LVESD, LVEDV, LVESV, LAD, LVMI, and hs-CRP in the hypertrophic group were all increased (P < 0.05), the serum levels of miR-192, TGF-β1, PⅢNP and PⅠCP in the hypertrophic group were increased (P < 0.05). Pearson correlation analysis showed that the serum level of miR-192 in patients with essential hypertension was positively correlated with the LVPWT, IVST, LVEDD, LVESD, LVEDV, LVESV, LAD, LVMI, TGF-β1, PⅠCP and PⅢNP (P < 0.05). The results of multiple linear regression analysis showed that the LVPWT, IVST, LVEDD, LVESV, TGF-β1, PⅠCP, and PⅢNP were related factors that affected serum level of miR-192 in patients with essential hypertension (P < 0.05).   Conclusion   The serum level of miR-192 is elevated in patients with essential hypertension and left ventricular hypertrophy, and is related to the degree of left ventricular hypertrophy and myocardial fibrosis.
Application of High Epidural Combined General Anesthesia in Laparoscopic Liver Resection

Abstract:
  Objective  To investigate the application of high epidural combined general anesthesia in laparoscopic liver resection.   Methods  A total of 86 patients with laparoscopic hepatectomy in the Second People’s Hospital of Qujing City from March 2020 to September 2022 were randomly selected and divided into 2 groups with 43 cases in each group by the number table method. The control group received general anesthesia during perioperative period, while the observation group received high epidural combined general anesthesia. The perioperative hemodynamic level, Perioperative hemodynamic level, T lymphocyte subsets, intraoperative blood loss and anesthesia related complications were recorded in the two groups.   Results  The perioperative hemodynamics of the two groups increased first and then decreased. The levels of SBP, DBP and HR at T1 and T2 in the two groups were higher than those at T0, and the differences were statistically significant (P < 0.05 ). The levels of SBP, DBP and HR at T1 and T2 in the observation group were lower than those in the control group, and the differences were statistically significant ( P < 0.05 ). The level of T lymphocytes in the two groups decreased in different degrees at 48 h after operation. The levels of CD3+, CD4+ and CD4+/CD8+ in the observation group were significantly higher than those in the control group (P < 0.05 ). There was no significant difference being found in the comparison of the incidences of apnea, delayed recovery, hypoxemia, respiratory depression and urinary retention between the two groups (P > 0.05 ).  Conclusion  The application of high epidural combined general anesthesia in laparoscopic liver resection can stabilize the hemodynamic level, reduce the central venous pressure of patients, and does not increase intraoperative bleeding, has little effect on the level of T lymphocyte subsets, and has high anesthesia safety, which is worthy of promotion and application.

  Objective  To explore the relationship between quantitative analysis of real-time ultrasound elastography and the prognosis of percutaneous transbiliary drainage (PTCD) in elderly patients with advanced malignant obstructive jaundice.   Methods  Retrospectively selected 112 elderly patients with advanced malignant obstructive jaundice who underwent PTCD treatment in our hospital between May 2017 and May 2021 were divided into poor prognosis (n = 42) and good prognosis groups (n = 70) according to their prognosis. We compared the clinical data of the two groups, the quantitative indexes of real-time ultrasound elastography before and after PTCD, analyzed the correlation between the quantitative indexes of real-time ultrasound elastography and Child-Pugh score, the quantitative indexes of real-time ultrasound elastography after PTCD and the prognosis of PTCD in elderly patients with advanced malignant obstructive jaundice, and drew the receiver operating characteristic (ROC) curve, and evaluated the value of real-time ultrasound elastography quantitative indicators for the prognosis of elderly patients with advanced malignant obstructive jaundice after PTCD.   Results  There were statistically significant differences in the site of obstruction, Child-Pugh score, and total bilirubin level between the two groups (P < 0.05). The mean value of strain of real-time postoperative ultrasound elastography quantitative index of PTCD in the poor prognosis group was lower than that in the good prognosis group, and the percentage of blue field was higher than that in the good prognosis group (P < 0.05). The mean value of strain and blue field percentage were associated with Child-Pugh score (P < 0.05). Strain mean value and blue field percentage were independent factors influencing PTCD prognosis in elderly patients with advanced malignant obstructive jaundice (P < 0.05).The AUC for the combined diagnosis of strain mean value and blue field percentage in elderly patients with advanced malignant obstructive jaundice for PTCD prognosis was 0.862.   Conclusions  Real-time ultrasound elastography quantitative measurement of mean strain and percentage of blue area in elderly patients with advanced malignant obstructive jaundice are related to the liver function and prognosis. Real-time ultrasound elastography quantification after PTCD can help predict the prognosis of the disease. It is of great significance in clinical work.
Effect of Dolisetron Mesylate Combined with Dexmedetomidine on Plasma Motilin and Postoperative Nausea and Vomiting in Patients Undergoing Laparoscopic Sleeve Gastrectomy

Abstract:
  Objective  To observe the effect of dolisetron mesylate combined with dexmedetomidine on plasma motilin levels and postoperative complications nausea and vomiting in patients undergoing laparoscopic sleeve gastrectomy.   Methods  One hundred and twenty ASA Ⅰ-Ⅱ patients, aged 18-60 years, scheduled for elective laparoscopic sleeve gastrectomy, were randomly divided into four groups, each group of 30 patients. Group A: dexmedetomidine+dolasetron mesylate group; Group B: dexmedetomidine group; Group C: dolasetron mesylate group; group D: Saline control group. The general situation, time of operation, blood loss and fluid infusion volume of the four groups were recorded. The plasma motilin level was detected by phletometry before operation (T0) and 24 hours after operation (T1). The degree and incidence rate of nausea and vomiting after 24 hours and 48 hours were recorded.   Results   (1) There were no obvious differences in age, gender, BMI, operation time, intraoperative blood loss and intraoperative infusion volume among the four groups. (2) The incidence of grade III and IV postoperative nausea and vomiting and the use of postoperative antiemetics in group A, B and C were lower than those in group D at 24 and 48 hours after operation, and the differences among the four groups were statistically significant (P < 0.01). (3) The plasma motilin levels in groups A, B and C 24 h after operation were lower than those before operation, and the differences among the three groups were statistically significant (P < 0.01, P < 0.05, P < 0.05). The motilin levels in groups A, B and C were lower than those in group D 24 hours after operation, and the differences were statistically significant (P < 0.01, P < 0.05, P < 0.05).   Conclusion  Dolasetron mesylate combined with dexmedetomidine can significantly reduce the plasma motilin level 24 hours after laparoscopic sleeve gastrectomy, and effectively reduce the severity and incidence of postoperative nausea and vomiting, and is worthy of clinical application.
Epidemiological Analysis of 771 Meningiomas

Abstract:
  Objective  To explore the pathological classification of meningioma and the composition of patients, and obtain the epidemiological data of single-center meningioma of the Third Affiliated Hospital of Kunming Medical University, so as to provide the basis and reference for future work.   Methods  The data of meningioma patients from January 2013 to December 2021 of the Third Affiliated Hospital of Kunming Medical University were collected, and the data were analyzed by statistical software. Combined with the results obtained, the epidemiological data and summary report of single-center meningioma in the Third Affiliated Hospital of Kunming Medical University were obtained.   Results  Of the 771 meningioma patients, 173 were male and 598 were female, with a ratio of 1∶3.46, the median age of male 51, the median age of female 52, the youngest 16, the maximum 82, and the mean age (52.07±10.538). Patients were concentrated in the age group of 51 to 60 years. Among the 771 patients, the WHO grade I patients were the most frequent, with 722 patients, or about 94%, and the remaining two categories were relatively small. Among the 771 cases, there were 2 cases of meningeal cortaneous type, accounting for 0.26%; 3 cases of fibrous type, accounting for 0.39%; 1 case of gravel body type, accounting for 0.13%; 2 cases of hemangioma, accounting for 0.26%. Among the 771 cases, there were 714 cases at WHO I level, accounting for 92.61%; 35 cases at WHO level II, accounting for 4.54%; 14 cases at WHO III level, accounting for 1.82%, and the more common types were fibrous, meningeal cutaneous, hemangioma, and gravel. The most common site of meningioma was the convex surface of the brain, followed by the base of the skull, parasickle of the brain, cerebellum, and pontocerebellar angle.   Conclusion  In this study, the peak age of meningioma was 51 to 60 years, the ratio of men to women was 1∶3.46, and the main site of onset was the convex surface of the brain. It provides data for the epidemiology of meningioma in the Third Affiliated Hospital of Kunming Medical University, fills in some gaps of meningioma epidemiological data, and provides the basis and reference for the future work.

Recent studies on neuroendocrine in patients with depression have shown that disorders of the hypothalamic-pituitary-adrenocortical axis (HPA axis) plays an important role in the pathogenesis of depression. This article reviews the research progress of HPA axis and depression in recent years, in order to provide new ideas and reference value for the diagnosis and treatment of depression.

Abstract:
  Objective  To explore the effect of solution-focused nursing intervention on treatment compliance and quality of life of hepatitis C patients.   Methods  A total of 100 patients with chronic hepatitis C in the Liver disease Center of Kunming Third People’s Hospital from October 2020 to October 2021 were selected as the research objects. Random number table method was used to divide them into control group and experimental group with 50 cases each. The control group was treated with routine nursing intervention. The experimental group implemented focused solution mode of nursing intervention [including: (1) intervention plan; (2) health education; (3) Evaluation and feedback; (4) nursing evaluation; (5) Nursing optimization], all intervention for 6 months. The quality of life, treatment compliance and nursing satisfaction of the two groups were compared and analyzed, and the application value of focused solution mode nursing intervention in patients with chronic C viral hepatitis was judged.   Results  After intervention, the quality score of SF-36 in the experimental group was significantly higher than the control group (120.64 ± 8.71 vs 113.97 ± 8.43). After intervention, treatment compliance (98.00% vs 86.00%) and nursing satisfaction (100.00%) versus 92.The experimental group was higher than that of the control group.05.   Conclusions  Intensive nursing intervention is effective for the care of patients with chronic hepatitis C. The nursing intervention mode based on this model can significantly improve the quality of life of patients, improve patients’ compliance behavior and nursing experience, and is worthy of clinical application.
