Classification and Treatment of Solitary Iliac Artery Aneurysms

  • 摘要: 孤立性髂动脉瘤是临床上罕见的疾病,其起病隐匿,一旦破裂,救治困难,死亡率极高。因此,发现疾病并对其进行合适分型和选择适宜治疗方案极为重要。近年来,腔内治疗逐渐替代传统开腹手术已经成为一线治疗方案,少部分学者提出可根据动脉瘤的解剖形态、影像学检查等将孤立性髂动脉瘤进行临床分型,以便更好的选择合适的腔内治疗方案。因此,回顾现有已发表的文献,对孤立性髂动脉瘤的治疗进展及对现有学者提出的分型进行综述。

    Ⅰ型:仅累及髂总动脉;Ⅱ型:仅累及髂内动脉;Ⅲ型:同时累及髂总及髂外动脉;IV型:累及髂总及髂外及髂内动脉。

    2A型:双侧近、远端均有合适的锚定区;2B型:双侧近端均有合适的锚定区,1侧远端有合适的锚定区,1侧无;2C型:双侧近端均有合适的锚定区,远端均无;2D型:1侧或双侧近端无合适的锚定区,但至少1侧远端有合适的锚定区;2E型:1侧或双侧近端无合适的锚定区,双侧远端均无合适的锚定区。

    A 型:近、远端均有合适锚定区;B 型:近端有合适的 CIA 锚定区,但远端没有;C 型:远端有合适锚定区,近端没有;D 型:近、远端均没有合适的锚定区;E 型:继发于先前开放或血管内修复的IIAA,分为以下三类:E1:行支架术后的同侧髂动脉扩张的IIAA;E2:行腔内治疗后,在已栓塞的髂内动脉同侧髂动脉扩张的IIAA;E3:先前行腹主动脉瘤开放重建后的“吻合口旁”的IIAA。
