Abstract:
Interleukin-10 (IL-10) , a single strand glycoprotein, is produced by both adaptive and innate immune cells. IL-10 specific receptor as a complex consists of two submits IL-10 R1 and IL-10 R2. IL-10 mainly plays biological functions via JAK-STAT signaling pathway. IL-10 is a crucial anti-inflammation cytokine taking part in immune regulation and having significant effect on autoimmune diseases. This paper reviews the relationships between IL-10 and autoimmune diseases according to research achievements in recent years.

Abstract:
Objective To construct the lentiviral expression vector of SOX9 gene and establish a LNcap cell strain with stable expression of SOX9. Me thods SOX9 gene was amplified by PCR and cloned into lentiviral expression vector p LVX-IRES-Puro. p LVX-IRES-FLAG-SOX9 recombinant plasmid was verified by restriction enzyme digestion and DNA sequencing. Then we gained recombinant virus particles in packaging cell HEK 293 and infected LNcap cell. The monoclonal LNcap cell strain stably expressed were obtained through puromycin screening.The m RNA level and protein level in the infected LNcap cell were detected by q RT-PCR and Western blot respectively. Re s ults Restriction enzyme digestion and sequencing demonstrated that SOX9 c DNA was successfully cloned into p LVX-IRES-FLAG lentiviral vector. After the transfection to LNCa P cells, the monoclonal cell strain of stably expressed SOX9 were obtained by puromycin screening, which showed expression of SOX9 m RNA detected by q RT-PCR. Western blot analysis revealed that SOX9 protein expressed markedly. Conclus ion The recombinant lentiviral vector bearing human SOX9 c DNA has been successfully constructed, and the exogenous expression of SOX9 in LNcap cells was achieved.
The Evaluation of F-VEP on the Efficacy of Using Hyperbaric Oxygen to Treat the Rats with Optic Nerve Crush

Abstract:
Objective To observe the effects of hyperbaric oxygen in the treatment of the rats with optic nerve crush. Me thods In this study, 24 rats with traumatic optic nerve injury were recruited. The rats' left eyes were injured in the optic nerve and the right eyes as the controls were applied with sham-operation. The rats were divided into the treatment group and the control group in accordance with the random number table. The experimental group was treated with two-course hyperbaric oxygen one hour a day, 10 days a course. The control group breathed fresh air under standard atmospheric pressure. F-VEP was detected on day 3, 10, 20 during the experiment. Re s ults In the control group, latency of F-VEP delayed (P < 0.05) and the amplitude of P100 decreased (P < 0.05) in the injured eye compared to the normal eye. In the experimental group, no obvious difference was found in the latency of F-VEP and the amplitude of P100 in the injured eye compared to the normal eye (P > 0.05) . The comparison of F-VEP data of the injured eyes showed that incubation period was in advance (P < 0.05) and amplitude increased (P < 0.05) in the experimental group. The comparison of F-VEP data tested at different time points was statistical significant. Conclusion The rat model of optic nerve injury was successful. Hyperbaric oxygen therapy showed no interference with the F-VEP data of the injured eye by causing no damage to the normal eye and was proved effective. Longer treatment provided better results

Abstract:
Objective To achieve the purpose of promoting movement function of the injury nerve by using the joint therapy of NT-3-HUMSCs and SOCS3 gene silencing on SD rats' spinal cord injury. Methods (1) We used adherence method in vitro human umbilical cord-derived mesenchymal cells ( HUMSC) during separation, purification and identification. (2) Then constructed NT-3 gene eukaryotic expression vector, which was transfected into its HUMSC, and constructed NT-3-HUMSC cell survival in vitro assay conditions and NT-3 expression. (3) We selected specific targets for SOCS3 screening and for sequence homology analysis.A negative control group was established. siRNA was designed and synthesized in vitro detection. (4) SD rats with spinal cord injury model were divided into two categories: (1) sham group with 10 rats; (2) T12 whole spinal cord injury model with 40 rats. The 40 rats were randomly divided into four groups with 10 rats in each group (saline treatment group, siRNA + NT-3-HUMSCs treatment group, NT-3-HUMSCs treatment group and siRNA treated group) . Motor function of the rats were evaluated respectively in 1, 2 and 3 months after the modeling was established successfully.Results (1) siRNA + NT-3-HUMSCs treatment group's BBB scores was significantly higher than NT-3-HUMSCs, SOCS3-siRNA and physiological saline groups (P <0.05) . (2) The grid climbing experiments showed that the neural functional recovery performed better in siRNA + the NT-3-HUMSCs treatment group compared to the NT-3-HUMSCs, SOCS3-siRNA and physiological saline groups (P <0.05) . Conclusion The NT-3-HUMSCs joint SOCS3 gene silencing in the treatment of SD rat spinal cord injury can improve the motor function of SD rat spinal cord injury.

Abstract:
Objective This study is designed to improve topical medication in the Department of Orthopedics in Kunming Municipal Hospital of Traditional Chinese Medicine by applying cataplasm made from the active ingredient (polyphyllin) of Paris polyphylla Smith var. yunnanensis (Franch.) Hand.-Mazz ( "Dian Chonglou" in Chinese) and to study the mechanism of anti-inflammation. Me thods SD male rats were randomly divided into two groups: control group and treatment group. The rats in two groups were all injected 50 ul 5% sodium urate suspension into tibiotarsal joint cavity to duplicate the model of acute gouty arthritis.The rats in the control group were applied with cataplasm without polyphyllin, whereas those in the treatment group applied with cataplasm of polyphyllin. After 4, 24, 48, 72 hours, the ankle diameter and circumference were measured. ELISA was used to test the interleukin-1β (IL-1β) andtumor necrosis factor (TNF) in peripheral blood. Western blotting was applied to detect the changes in TLR4 expression of monocyte.Re s ults Compared with the control group, the added value of the diameter and circumference of ankle decreased significantly in the treatment group 4, 24, 48 and 72 h after the treatment.IL-1β in the treatment group was lowered at 4, 24 and 72 h. TLR4 in the treatment group decreased at 48 h. Conclus ion Cataplasm with polyphyllin can alleviate the acute inflammatory reaction in gouty joint of rat by reducing the expression of IL-1βand TLR4.

Abstract:
Objective To investigate the changes in the expression of JNK3 and APP protein in retinal ganglion cells (RGCs) after optic nerve injury and its effect on the survival of RGCs. Me thods Optic nerve crush models were established using adult APP wild type mice. Immunohistochemistry and western blot for detecting the expression of JNK3 and APP protein in RGCs were used post-injury. The number of RGCs in APP KO mice group was counted to compare with APP WT mice group. Re s ults After optic nerve injury, JNK3 expressed in cytoplasm of RGCs; p-JNK transformed from cytoplasm to nuclei of RGCs; APP expressed in cytomembrane; p-APP expressed in cytoplasm and axons of RGCs. Western blot results showed that the protein expressions of JNK3, p-JNK, APP and p-APP were increased in the injured group (P<0.01) . The survival number of RGCs (147±7.0) in APP KO mice group increased significantly than APP WT mice group (127.7 ±7.0) (P <<0.05) .Conclus ion The upregulation of JNK3 and APP protein expression maybe play an important role in RGCs survival proceeds after optic nerve injury.

Abstract:
Objective To study the serotypes and molecular characteristics of foodborne Salmonella in Yunnan Province using pulsed field gel electrophoresis (PFGE) and to establish PFGE fingerprint database.Methods This study was carried out on the basis of the Serological typing of 322 strains of foodborne Salmonella which was isolated from Yunnan national foodborne disease surveillance from 2013 to 2016. The clustering analysis was conducted on 148 strains of Salmonella DNA restriction enzyme map by using the software of Bio Numerics. Fingerprint database was established through the comparison of clustering analysis correlation on bacterial strain. Results The serotype of 322 strains of Salmonella mainly included A, B, C, D, E, F, G and other 7 groups, among which Salmonella typhimurium was the major type, accounting for 11.4% (37/322) . Cluster analysis was applied using Bio Numerics software in 148 strains of Salmonella DNA restriction enzyme map.According to the different number and the different positions, electrophoresis strips were divided into 102 different PFGE patterns (Figure 1) , which was categorized into 39 clusters if 90% of the strips was similar. Conclusion Foodborne salmonella molecular classification is complex. Salmonella typhimurium is the major type. PFGE belt type presents diversity.

Objective Through the screening of candidate pathogenic gene among family members of a family with familial hypertrophic cardiomyopathy in Yuxi, Yunnan Province, the study is designed to analyze the relationship between genotype and phenotype and to provide an important theoretical basis for the research of molecular genetic mechanism, early screening and early intervention of familial hypertrophic cardiomyopathy.Me thods A detailed medical history was collected and physical examination and routine twelve lead electrocardiogram and cardiac ultrasonography examination were performed among the family members. The peripheral venous blood samples were collected for genetic testing. The genetic map was drawn and the genetic characteristics, genotype and clinical phenotype were analyzed. Re s ults In this family, the dominant inheritance mode of hypertrophic cardiomyopathy is X-linked dominant inheritance. Candidate genes screening showed that a missense mutation was found in the GLA, ZFPM2, SCN5 A genes and the translated amino acids were changed.Conclus ion X-linked dominant inheritance is the main genetic mode of HCM in this family. GLA c.167 G> A (p.Cys56 Tyr) heterozygous or hemizygous missense mutation may be the major pathogenic mutation in this family with non-obstructive hypertrophic cardiomyopathy. The clinical significance of ZFPM2 c.1332 G> C (p.Lys444 Asn) heterozygous missense mutation and SCN5 A c.5216 G> A (p.Arg1739 Gln) heterozygous missense mutation in this family is undetermined.

Abstract:
Objective To assess the income status of primary care providers and to explore the determinants of income in a county of Dali. Methods In August 2016, the questionnaire was employed to collect the data of income status of 191 rural health workers and 217 village doctors in the county. Results Through the study, we found that the income of rural health workers in the county was 34, 000 (26, 000, 46, 000) yuan with a satisfaction rate of 62.3% (95% CI 55.4%69.2%) and no change (74.7%) was seen in the income among majorities after implementing the Zero Mark-up Policy for essential medicines. For the village doctors, the income was 20, 000 (15, 000, 24, 000) yuan with a satisfaction rate of 40.6% (95% CI 34.0% 47.1%) and a fall of the income was found in more than half of the doctors after the implement of the policy.Conclusion Health care workers in towns are quite satisfied with their income whereas those in health stations of villages are not content, compared with the average income at the national level. We should increase government's investments on grass-root healthcare team, improve the incentive pay plans and promote the integrated management of health facilities in towns and villages.
The Effects of Oxygen Therapy on Pulmonary Rehabilitation in Patients with COPD

Abstract:
Objective To discuss the effects of oxygen therapy on partial pressure of oxygen, pulmonary function and quality of life among patients with chronic obstructive pulmonary disease (COPD) during respiratory rehabilitation (PR) . Me thods Eighty patients with COPD were selected and randomly divided into two groups (receiving oxygen therapy and receiving no oxygen therapy) with 40 in each group.Patients who participated in this study underwent a comprehensive 8-week outpatient PR program. After the 8-week treatment, patients' pulmonary function, blood gas analysis and St.George respiratory questionnaire (SCRQ) score were tested. Re s ults Significant improvements were observed in pulmonary function, partial pressure of oxygen and SCRQ score in both groups after the treatment.The group underwent the oxygen therapy showed better results in partial pressure of oxygen, FEV1 and SCRQ score during the rehabilitation compared to the group underwent no oxygen therapy.Conclus ion Oxygen therapy during PR plays a positive role in respiratory rehabilitation, which is worth generalizing.

Objective To investigate the association between rs11098403 of the NDST3 gene and schizophrenia among Han People in Yunnan. Me thods A total of 364 Han patients with schizophrenia and 287 healthy Han people were enrolled in our study. SNa P shot was used to detect the genotype of rs11098403. Positive and Negative Symptoms scale (PANSS) was used to assess the symptoms of schizophrenia.Re s ults No differences were found in the genotype and allele distributions of rs11098403 among patients and the control group (P>0.05) .The allelic frequencies of rs11098403 showed no significant association with positive symptoms, negative symptoms or general symptoms (P>0.05) . Conclus ions Our study indicated that the association between NDST3 gene and schizophrenia might be specific to European population.

Objective To study the correlation between VDR gene polymorphism and the transformation of bone markers in children with nephrotic syndrome and to observe whether active intervention treatment of vitamin D with nephrotic syndrome is influenced by VDR gene polymorphism. Methods We used polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) technology to detect the VDR genotypes of70 children with nephrotic syndrome (one group received hormone for 2 weeks and the other received no hormone therapy) .Then we detected bone metabolism index among patients with nephrotic syndrome and 30 normal children (control group) and compared the index between the two groups with and without hormone therapy.We divided 70 children with nephrotic syndrome into the AA + Aa genotype group and the aa genotype group to study the difference between various genotypes of bone metabolism indexes. After retesting bone metabolism index of those children receiving the same dose of calcitriol therapy after 2 weeks, we divided them into three groups AA genotype, Aa genotype and aa genotype to detect the changes of bone metabolism index in different genotypes.Results (1) The calcitonin and serum phosphorus of children with AA + Aa genotype were significantly lower than those of childrenwith aa genotype (P < 0.05) ; 25 (OH) D3 of children with aa genotype was significantly lower than that of ones with Aa + Aa genotype (P < 0.05) . (2) The changes of bone metabolism index in children with nephrotic syndrome after receiving the same dose of calcitriol for 2 weeks:the calcitonin and 25 (OH) D3 in AA + Aa genotype group were significantly higher after treatment (P <0.05) .Conclusions Vitamin D receptor gene Apa Ⅰ polymorphism may be a genetic susceptibility factors affecting bone metabolic abnormalities. AA and Aa genotype may be a protective factor of bone metabolic abnormalities.The AA+Aa genetype of Vitamin D receptor gene Apa Ⅰ polymorphism response to treatment of calcitriol is positive.

Abstract:
Objective From May 2010 to May 2013 and from June 2013 to June 2016, we compared the clinical analysis of patients with necrotizing enterocolitis (NEC) in the Department of Pediatrics, studied incidence rate of NEC and the relationship between stages of NEC and prognosis, and compared the prognosis between the two three-year periods to provide the experience of diagnosis and treatment for clinical practice in the future. Me thods The clinical data of patients diagnosed with NEC were retrospectively analyzed according to the diagnostic criteria of modified Bell staging classification using SPSS statistical software package. Early diagnosis rate and prognosis were compared between the two three-year periods. P <0.05 showed that statistical significance was found. Re s ults From May 2010 to May 2013, 40 cases of NEC were diagnosed in our hospital, among which 8 was in the first stage (20%) , 24 in the second stage (60%) , and 8 in the third stage (20%) . The number of premature infants was 32, accounting for 80%, among which 25 cases were infants with very low birth weight accounting for 62.5% and 8 cases were full-term infants, accounting for 20% (P < 0.05) . In the study, 32 cases (80%) received traditional treatment, which was effective in stage I. During the second stage, two abandoned the treatment was confirmed death in the follow-up, two underwent surgery successfully due to the progression of the disease and four (25%) died in the hospital. Statistic significance of treatment of internal medicine was found when comparing stage I with stage II (P<0.05) . Ten children underwent surgery, among which one gave up treatment after the surgery because of financial problems and was found dead in the follow-up and four (60%) died in the hospital. The comparison of surgical treatment in patients in stage II and III showed no statistical significance (P>0.05) . Fatality rates were statistically significant (P<0.05) . From June 2013 to June 2016, 41 children were diagnosed with NEC in our hospital, including 24 cases in stage I (59%) , 14 in stage II (34%) , and 3 in stage III (7%) . The number of premature infants was 33, accounting for 80%, among which 26 cases were infants with very low birth weight accounting for 62.5% and 8 cases were full-term infants, accounting for 20% (P<0.05) . In the study, 35 cases (85.3%) received traditional treatment, which was effective in stage I. During the second stage, one underwent surgery successfully due to the progression of the disease. Statistic significance of treatment of internal medicine was found when comparing stage I with stage II (P<0.05) . Four children underwent surgery, among which one gave up treatment after the surgery because of financial problems and was found dead in the follow-up and the surgery was successful in other three. The comparison of surgical treatment in patients in stage II and III showed statistical significance (P <0.05) . Fatality rates were statistically significant comparing the two three-year periods (P<0.05) . The comparison of the rates of early diagnosis in the two three-year periods showed statistically significance (P <0.001) . Conclus ion Early diagnosis of NEC is the key to reduce mortality.Intestinal fatty acid binding protein (I-FABP) is a serological marker for early diagnosis of NEC. As an important factor, the reduction of the incidence of premature birth will result in the drop in the incidence of NEC.
The Value of Using sPESI Score Combined with hs-TnT in the Evaluation of Short-term Prognosis in Hemodynamically Stable Pulmonary Embolism

Abstract:
Objective To explore the value of using sPESI score and hs-TnT in the evaluation of short-term prognosis in hemodynamically stable pulmonary embolism. Methods We collected 99 patients with hemodynamically stable PE from our department. According to the sPESI score and hs-TnT, patients were divided into high risk group (sPESI score ≥1) and low risk group ( sPESI score=0) , positive group (hs-TnT ≥0.014 ng/m L) and negative group (hs-TnT<0.014 ng/m L) . Then all patients were treated and followed up for 30 days. The sensitivity, specificity, positive predictive value and negative predictive value of adverse events of prognosis were calculated, and ROC curve was drawn to analyze the values in different grouping methods for the prognostic evaluation.Results Thirteen adverse events occurred in all patients. The single test showed that sensitivity, specificity, positive predictive value, and negative predictive value of hs-TnT for predicting adverse events were respectively 84.6%, 55.2%, 22%, and 96%. When sPESI was used alone, sensitivity, specificity, positive predictive value and negative predictive value were 92.3%, 48.8%, 21.1%, and 97.7%. Results of combined testing showed 100% sensitivity, 29% specificity, 17.6% positive predictive value, and 100% negative predictive value. The ROC curve area of the sPESI, hs-TnT, sPESI and hs-TnT are 0.832 ( 95% CI, 0.705-0.958) , 0.825 ( 95% CI, 0.694-0.957) , 0.872 ( 95% CI, 0.773-0.971) . ConclusionsPESI and hs-TnT have clinical value in evaluating the short-term prognosis of hemodynamically stable pulmonary embolism. sPESI combined with hs-TnT has higher significance, especially in patients with low-risk PE.
A Clinical Study of Combined Therapy of Colchicine and Benzbromarone Capsules in the Treatment of Gouty Arthritis at Non-Acute Episode Stage

Abstract:
Objective To explore and analyze the clinical values of combined therapy of colchicine and benzbromalone capsules in the treatment of gouty arthritis of non-acute episode. Methods From December 2015 to February 2017, 60 patients with gouty arthritis who were not in acute attack were randomly divided into the combined group and the control group, with 30 cases in each group. The control group was given benzbromarone capsules and the combined group was given colchicine combined with benzbromarone capsules. The changes of laboratory indexes [blood uric acid, erythrocyte sedimentation rate (ESR) , C reactive protein (CRP) , alanine aminotransferase (ALT) , glomerular filtration rate (GFR) ], and clinical symptoms (pain, lesion joint limitation, fever, redness and tenderness) were measured and compared in the two groups before and after the treatment. The daily living ability scale (SF-36) was used to evaluate the quality of daily life before and after the treatment, and the incidence rate of drug adverse reactions during the treatment was also counted. Results No significant differences were found in the laboratory related indexes, clinical symptoms and quality of life before the treatment between the two groups (P>0.05) . After the treatment, the levels of ALT and GFR were not significantly fluctuated in the two groups, but the levels of blood uric acid, ESR, CRP, pain symptoms and quality of life were improved, and the increasing degree in the combined group was more significant than that of the control group. and the number of episodes of acute gouty arthritis in the combined group was also lower than that in the control group, and the difference between the above indexes was statistically significant (P<0.05) . The total incidence rate of adverse reactions was 30% in the combined group, which was not significantly different from that in the control group with 26.67% (χ2 = 0.180, P = 0.857) . Conclusions The use of colchicine combined with benzbromalone capsules is as the safe as the single use of benzbromalone capsules and the effect of the former one is better than that of the latter one in the treatment of gouty arthritis at non-acute episode stage.

Objective To observe and compare the incidence of cough among patients with post-infection cough undergoing painless gastroscopy.Methods Seventy-one patients with post-infection cough undergoing painless gastroscopy were enrolled and randomly divided into two groups.The observation group (34 cases) received budesonide nebulization before gastroscopy and the control group ( 37 cases) was performed gastroscopy directly.We observed the incidence of cough at different time point and the changes of circulationduring the operation. Results The incidence of cough between the observation group and the control group showed statistically significant differences (P <0.01) . The changes of circulation showed statistical differences between two groups in the process of gastroscopy (P<0.05) .Conclusion Budesonide nebulization supplying can decrease the incidence of post-infection cough among patients undergoing painless gastroscopy.

Abstract:
Objective To investigate the value of color image scoring with shear wave elastography in the diagnosis of benign and malignant breast solid lesions.Me thods From February 2016 to February 2017, 125 patients with breast lesions were given shear wave elastography score, by type of image scoring method, image color method for differential diagnosis between benign and malignant lesions. Re s ults Among all the 125 patients with breast solid lesions, 175 were solid lesions, of which 85 cases (119) were benign lesions, accounting for68%, and 40 (56) were malignant lesions, accounting for 32%. The difference of type and color score between benign breast lesions and malignant lesions was statistically significant (P < 0.05) . A positive correlation was fourd between color image type score and color score of benign and malignant breast lesion, using shear wave elastography (P < 0.05) . The pathological findings were used as a gold standard, and the difference in the area under ROC curves of benign and malignant lesions of the breast was statistically significant by using type and color scoring method (P < 0.05) . Type scoring method and color scoring method have higher diagnostic efficacy when the diagnostic value is above 2 points. Conclus ion The shear wave elastography color image scoring method is of high clinical value in the diagnosis of benign and malignant breast solid lesions, and it is of high accuracy for the diagnosis of benign breast solid lesions.

Abstract:
Objective To study the curative effect of high frequency oscillatory ventilation on acute respiratory distress syndrome (ARDS) caused by smoke inhalation injury in the lungs. Me thods Fifty ARDS patients with lung injury caused by smoke inhalation were selected from 2010 to 2015. Patients were randomly divided into two groups, respectively choosing conventional mechanical ventilation or high frequency oscillatory ventilation.Re s ults PaO2, PaCO2, and PaO2/FiO2 were improved in both groups after the treatment (P < 0.05) . Patients in the treatment group improved significantly compared to those in the control group (P < 0.05) . The ventilation time and length of hospital stay reduced significantly in the treatment group (P < 0.05) . No significant difference was found in mortality and complication rates in both groups within 30 days (P > 0.05) . Conclus ion High frequency oscillatory ventilation can effectively alleviate symptoms of ARDS caused by smoke inhalation injury in the lungs by improving oxygenation index. No obvious improvement is found in prognosis.

Objective The aim of this study was to analyze the HCV seroprevalence in the general population visiting the Second Affiliated Hospital of Kunming Medical University. Me thods Between January 2013 and December 2015, a total of 160, 239 subjects were screened for the presence of anti-HCV antibodies in blood serum. Anti-HCV antibodies in serum samples were detected by enzyme-linked immunosorbent assay (ELISA) .The results of anti-HCV were analyzed in the features of year, sex and age. Re s ults The HCV seroprevalence in the general population from 2013 to 2015 was 1.11%, 1.04% and 0.91%, respectively, which was significantly higher in men than in women (1.30% vs. 0.91%, P < 0.05) . The highest HCV seroprevalence occurred in aged31-65 years. Conclus ions The analysis of the data suggests that the features of HCV-positive including year, sex and age could be beneficial for formulating scientific strategy and intervention measures of HCV infection and liver cirrhosis, liver failure and hepatocellular carcinoma caused by HCV in Kunming.

Abstract:
Objective To investigate the prevalence and explore the related risk factors for peripheral arterial disease (PAD) among patients with type 2 diabetes so as to provide the evidence and reference to the early intervention in community. Me thods A total of 662 patients with type 2 diabetes were selected in the community from June 2014 to June 2015 in Shenzhen province. All patients were divided into two groups according to the values of ankle-brachial index. The prevalence of PAD was calculated. Univariate and logistic regression analysis was applied to explore risk factors associated with the prevalence of PAD. Re s ults 25.83% (171/662) of the diabetic patients suffered from peripheral arterial disease. Univariate analysis results found that age, education level, family history, course of disease, smoking status, physical exercise, dietary habits, body mass index, with hypertension, with hyperlipidemia, tuberculosis were all significantly associated with the prevalence of PAD (P<0.05) . Logistic regression analysis finally showed that older age, with hypertension, with dyslipidemia, long course of disease, body mass index, with family history, smoking were found to be the valuable risk factors related to PAD (P <0.05) , whereas high education level and regular physical exercise were protective factors against PAD (P <0.05) . Conclus ions Among diabetic patients in community, the prevalence of PAD was higher and there were many risk factors. We suggest that the early screening of PAD and targeted intervention should be given full attention and strengthened. Based on controlling of blood glucose, diabetic patients should not only strictly control blood pressure, but also pay attention to blood lipid regulation, control BMI and ensure smoking cessation, especially for those with advanced age, long course of disease and family history, so that we can effectively reduce the prevalence of PAD.

Objective To observe microRNA-124 expression in the serum of patient with intracerebral hemorrhage and explore the relationship between the expression and associated clinical factors. Me thods Thirty serum specimen were collected among patients with intracerebral hemorrhage, who were checked up at the same time in the hospital. Relative expression of microRNA-124 in serum was detected. The variation of Micro RNA-124 expression between the two groups and clinical data of patients with intracerebral hemorrhage were analyzed. The relationship between microRNA-124 and clinical factors related to intracerebral hemorrhage was explored. Re s ults Compared with healthy people, microRNA-124 expression in serum increased among patients with intracerebral hemorrhage (P <0.05) . The expression is related to the bleeding volume and onset time (P <0.05) .Meanwhile, no obvious correlation is established between serum microRNA-124 expression and other factors in patients with intracerebral hemorrhage, such as gender, bleeding area, with or without surgical treatment (including minimally invasive and craniotomy) , a history of high blood pressure, fasting venous blood glucose levels (FPG) , and cerebrovascular disease history (P >0.05) . Conclus ion The serum microRNA-124 expression in patients with intracerebral hemorrhage is higher than that in healthy people. Bleeding volume positively increases expression. A higher expression is seen within one week at the onset of intracerebral hemorrhage compared to that one week after.

Abstract:
Objective To compare intrarenal pelvic pressure or endoureteral pressure and infection index between perfusion pump and low pressure of gravity drip during percutaneous nephrolithotomy (PCNL) .Methods From 2015 April to 2017 June, 107 patients with ureteral or renal calculus were randomly divided into two groups, among whom 46 cases underwent perfusion pump PCNL and 61 cases underwent low pressure gravity drip PCNL. No statistical significance of preoperative variables was found between the two groups such as age, gender, preoperative status of infection, status of hydrpnephrosis, size and location of stone (P >0.05) .A 5 Fr ureteral catheter was positioned into the renal pelvic or ureter, and the intrarenal pelvic or endoureteral pressure was measured by anesthesia monitor during PCNL for all patients.We recorded and compared removing calculus time, postoperative hemoglobin changing, fever rate, procalcitonin abnormal rate, white blood cell counts and calculi clearance rate.Results The average pelvic or endoureteral pressures in the group undergoing perfusion pump and the one undergoing low pressure gravity drip were (22.7 ±4.6) mm Hg VS (29.8 ±5.3) mm Hg and the maximum pressure were (28.8 ±6.56) mm Hg VS (50 ±9.25) mm Hg.The statistics was significantly different (P <0.05) .Compared with perfusion pump group, the group undergoing low pressure gravity drip had significant differences in fever rate [4.9% (3/61) VS.17.3% (8/46) ], procalcitonin abnormal rate [3.2% (2/61) VS.15.2% (7/46) ] and white blood cell counts abnormal rate [9.8% (6/61) VS.28.26% (13/46) ] (P<0.05) .However, no significant difference was seen in postoperative hemoglobin changing[ (8.3±3.7) g/L VS. (7.2±3.3) g/L] (P>0.05) and the time of removing calculus [ (42.4 ±14.5) min VS (43.6 ±13.5) min] (P >0.05) .Conclusions Compared with perfusion pump, low pressure gravity drip PCNL maintains lower renal pelvic or endoureteral pressure, which is less than reflux limit during calculi fragmentation. It might be contributed to reduce the postoperative fever rate, white blood cell counts abnormal rate and procalcitonin abnormal rate. Hemoglobin changing and the time of removing calculus show no obvious disadvantage.

Objective The aim of this research is to identify and compare the adjustment levels in patients with temporary and permanent colostomy, and to provide evidence to improve nursing performance. Me thods In the First, Second, and Third Affiliated Hospital of Kunming Medical University, 52 patients with colorectal stoma participated in the research and completed the questionnaire of OAS (Ostomy Adjustment Scale) and demographic data. The data were established using Epidata 3.1, then analyzed with SPSS 17.0. The results were described using frequency, mean, stand deviations, and p values.Re s ults Patients with permanent colostomy gained higher scores in OAS scores and psychological adaptation. Patients with temporary colostomy had higher scores in physical adaptation and social adaptation. Conclus ion The nursing staff will promote the quality of life for this population from multiple perspectives in the future.

Abstract:
Objective To investigate the application of Haiao prosthodontics membrane in the repair of palatal fistula. Me thods According to personal preference, 32 patients with palatal fistula were randomly divided into the experimental group (n=15 cases) and the control group (n=17 cases) . In the experimental group, the flap of the surrounding tissue of fistula was used as nasal layer and Haiao prosthodontics membrane was used as the oral layer for the repair of palatal fistula.In the control group, the flap of the same tissue was used as nasal layer and the flap of the ortho tissue was used as the oral layer to repair the palatal fistula. Several indexes were evaluated including the blood loss, operation time, hospitalization time, postoperative fever, postoperative complications and hospitalization expenses. Re s ults Less blood loss, shorter operation time and shorter hospital stay were observed in the experimental group compared with the control group (P <0.05) . The incidence of fever after operation, the temperature and the duration of the fever were significantly lower in the experimental group (P <0.05) . The incidence of bleeding complications after operation in the experimental group were significantly lower compared with that in the control group (P<0.05) . No significant difference of the refissure rate and hospitalization expenses was found between the two groups (P > 0.05) .Conclus ion Haiao prosthodontics membrane can be used to repair some palatal fistula. Advantages include safety, reliability, mild postoperative reaction and quick recovery.

Objective To carry out the investigation of common susceptibility factors of primary hypertension in young people, and to find the pathogenic factors of primary hypertension. Me thods 354 cases of young patients with essential hypertension were selected from 7360 young students in School of CAPF Frontier Guards Force and compared to 115 young healthy students at the same age. Re s ults The incidence of primary hypertension is increasing year by year. More common risk factors are seen among students with hypertension compared to those of the control group. Conclus ion Risk factors of primary hypertension are commonly seen among young students in School of CAPF Frontier Guards Force. It is necessary to prevent the risk factors, apply the targeted intervention and improve the management and issue a plan for prevention and control of high blood pressure.

Endothelial dysfunction is a major risk factor for vascular complication. Nitric oxide (NO) , the most important product of endothelial cells, lays a key role in regulating vasomotor as an endogenous vasodilatation factor. NO is produced by endothelial NO synthase (eNOS) on its substrate L-arginine. Arginase, which metabolizes L-arginine to urea and ornithine, competes directly with e NOS for L-arginine leading to the decrease of NO production. This paper reviews the basic role of arginase, the development of arginase inhibitors and its effects on diabetic vascular complications.

The prevention of occupational exposure to HIV has been carried out in China for many years, and achieved remarkable results. However, the practice of preventive drugs among people with non occupational exposure has many difficulties. Although the high-risk groups show high willingness and demand, their awareness of the preventive drugs is low due to the fear of increasing unprotected sex after the use of the drug. Other factors negatively affecting the application of the drugs are the cost and the side effects. In order to provide preventive treatment on time for the high-risk groups, the awareness of non occupational exposure prophylaxis should be raised.

Objective To explore how to apply the teaching mode combined flipped class with PBL teaching method to improve college students' ability of English reading and writing and the effectiveness of classroom.Me thods In the class of college English reading and writing, students at grade 2015 were randomly divided into two groups. The experimental group (43 students) was adopted the class teaching with “ flipped class and PBL teaching method” and the control group (43 students) was adopted traditional class teaching mode taught by the same teacher. The teaching effects of two groups were studied in a comparative way. The students' final grade, related data in the learning process, students' view of the two teaching modes are studied by analysis of basic dataand questionnaire. Re s ults At the end of the semester, the results of the experimental group are better than those of the control group. In the survey of the two class teaching modes, the students in the experimental group spent more time in the learning process than that of the control group.Students think that the teaching mode combined flipped class and PBL teaching method not only improves their autonomic learning ability, classroom participation, the English reading and writing ability, but also helps them perform better in the average scores in the final exam compared to the traditional classroom. Students are more satisfied with the new classroom teaching mode than with traditional one. Conclus ion The teaching mode combined flipped class with PBL teaching method can promote the students' autonomous learning, contribute to absorb and internalize knowledge, improve the students' learning interest and learning efficiency, and improve teachers teaching ability.

Objective The paper aimed to discuss the influence of case teaching of forensic pathology based on network platform on the critical thinking ability of forensic students.Methods Students majoring in forensic were randomly divided into the experimental group and the control group with 20 students per group. According to heterogeneity classification, the experimental group was divided into 4 subgroups. The subgroups participated in network cases learning whereas the control group received traditional case teaching. Participants were required to fill in California Critical Thinking Disposition Inventory-Chinese Version (CCTDI-CV) before and after learning. CCTDI-CV scores, the scores of final exam and the number of students who had improved in CCTDI-CV scores were compared between the two groups. Results For the experimental group, the total score of CCTDI-CVand the scores of items including looking for the truth, systematized ability, self-confidence, thirst for knowledge were significantly improved after learning. The performance of the experimental group was better than that of the control group at the end of teaching (P<0.05) . The scores of final exam were higher in the experimental group compared to the control group (P <0.05) .Conclusion Forensic pathology cases teaching based on network platform is an effective way to stimulate students' critical thinking ability and to improve the study ability.

Objective To explore the effects of clinical pathways of nursing on pulmonary function index and complication in patients with COPD. Me thods From October 2015 to June 2017, 94 patients with COPD were divided into two groups according to the order of admission and the nursing method: 47 patients undergoing routine nursing care and 47 patients receiving clinical pathways of nursing. We compared the incidence of complications, lung function, satisfaction, sense of comfort, anxiety, average hospitalization, health knowledge score and hours of ventilator use between the observation group and the control group. Re s ults The incidence of complications in the observation group was significantly lower than that in the control group (P<0.05) . The lung function index of the observation group was better than that of the control group, but the difference was not statistically significant (P > 0.05) . In the observation group, the satisfaction, the sense of comfort and the health knowledge score were higher.The anxiety degree was lower and the average hospital stay and hours of ventilator use were shorter compared to the control group (P<0.05) . Conclus ion The clinical pathways of nursing are used to reduce the incidence of complications, to improve the lung function index, the nursing satisfaction, sense of comfort and health knowledge score, and to reduce the average length of hospital stay, anxiety and hours of ventilator use in patients with COPD care.

Objective To provide suggestions on how to improve the ability of rehabilitation nursing by investigating the current situation of clinical nurses' knowledge, skills and cognition of rehabilitation nursing and analyzing the influential factors. Methods According to the cross-sectional survey method, a self-designed questionnaire was used to investigate the clinical nurses' current status of knowledge and skills, cognition of rehabilitation nursing.Totally 278 nurses from 7 hospitals were included in this study. Results The scores of clinical nurses' knowledge, skeills and cognition of rehabilitation nursing were 11.13 ±3.29 and 33.27±3.92.Variables associated with the scores of the rehabilitation nursing are age, nursing professional title, work duration, and degree of education. Conclusion The clinical nurses had positive attitude on rehabilitation, but their knowledge and skill were insufficient. Measures should be taken to improve nurses' ability of rehabilitation nursing.
