Abstract:
Cirrhosis has a high global incidence rate and serious harm. Numerous studies have shown that liver cirrhosis may cause cardiovascular abnormalities in patients, which are closely related to the severity of the condition and may have adverse effects on the prognosis of liver cirrhosis patients. Therefore, the pathogenesis and research progress in hyperdynamic circulation, portal hypertension pulmonary hypertension, cardiac systolic and diastolic dysfunction, cardiac electrophysiologic abnormalities, and cirrhotic cardiomyopathy are reviewed. To lay the foundation for further in-depth research on the prevention and treatment strategies of liver cirrhosis.

Domestication and Biological Characteristics of a Vero Cell Line in Suspension Culture

Abstract:
  Objective   To domesticate adherently cultured Vero cells into suspension-cultured cells and investigate their biological characteristics.   Methods   Cell culture was optimized using serum-free media and the "shaker adaptation method." Additionally, tumorigenicity, short tandem repeat (STR) profiles, morphological characters under transmission electron microscope (TEM), and influenza virus sensitivity were analyzed in these cells.   Results   Suspension cell line Vero-S was successfully established. The growth period of Vero-S cells ranged from 24 to 72 hours, with a plateau phase between 108 and 144 hours, and a decline phase after 144 hours. The population doubling time (PDT) was 36 hours, and cell density ranged from 14.01 to 14.88 × 108 cells/L. Average cell roundness ranged from 0.73 to 0.75, and cell diameter ranged from 12.12 to 14.44 µm. Vero-S cells formed tumors subcutaneously in nude mice, while Vero cells before suspension culture did not exhibit tumorigenicity. STR analysis indicated a primate origin of the cell line. Under electron microscopy, cell structure appeared intact. Vero-S cells showed insensitivity to influenza virus infection.   Conclusion   The established suspension cell line Vero-S exhibiting growth patterns similar to adherent cells, with uniform cell size and high-density growth supported by the established suspension culture medium. STR results demonstrated complete genetic stability of suspended Vero cells, with no apparent differences observed under transmission electron microscopy compared to adherent Vero cells. This study provides reference for suspension culture domestication techniques and can be applied in mechanistic studies of cell tumorigenicity.
Exploring the Effect of TRPC1 and BK-α expression on Diabetic Nephropathy

Abstract:
  Objective  To explore the effect of transient receptor potential channel 1 (TRPC1) protein and large conductivity Ca2+ activated K+ channel α subunit (BK-α) protein on diabetic nephropathy (DKD).   Methods  SD rats were randomly divided into a control group (n=15) and a model group (n=15). Construct a DKD model using high-fat feed and streptozocin (STZ). Using a blood glucose analyzer to detect changes in blood glucose levels in rats; Using a fully automatic biochemical analyzer to detect the renal function level of rats; HE staining was used to detect pathological changes in renal tissue to confirm successful modeling. Real time fluorescence quantitative PCR (RT-qPCR) and Western blotting were used to detect the mRNA and protein expression levels of TRPC1 and BK-α in renal tissue, respectively; Immunohistochemical detection of the distribution and expression of TRPC1 and BK-α.   Results  The fasting plasma glucose (FPG), urinary albumin excretion rate (UAER), blood urea nitrogen (BUN), and creatinine (Cr) in the model group were significantly higher than those in the control group (P < 0.01); the inner wall cells of the renal tubules in the model group showed swelling, with some cells detached; visible renal tubular lesions or death. In addition, neutrophils and their debris were found in many renal tubules and interstitial regions; the above HE staining results indicate that the DKD model has been successfully replicated. TRPC1 and BK-α were most abundant in the glomerulus, and the mRNA and protein levels of TRPC1 and BK-α in the renal tissue of the model group rats were significantly higher than those in the control group (P < 0.05).   Conclusion  Diabetic nephropathy affects the distribution and expression of TRPC1 and BK-α in renal tissue.
The Molecular Mechanism of miR-205-5p Targeting ERBB3 to Regulate PI3K/AKT/mTOR Pathway and Inhibit Angiogenesis in Hemorrhoids

Abstract:
  Objective  To explore the molecular mechanism of miR-205-5p targeting ERBB3 to regulate PI3K/AKT/mTOR pathway and inhibit angiogenesis in hemorrhoids.  Methods  The hemorrhoidal nucleus and normal perianal tissues of 12 patients from July 2021 to June 2022 were collected, the expression of miR-205-5p and ERBB3 in the samples was detected by qRT-PCR, and the targeting relationship was verified by dual-luciferase. miR-205-5p mimic, pcDNA-ERBB3, miR-205-5p inhibitor, sh-ERBB3, oe-ERBB3 and negative control plasmids were respectively or co-transfected into hemorrhoidal model rats and HUVEC cells. The pathological changes and apoptosis of perianal tissue in each group were observed by HE and TUNEL staining, and the localization of expression of ERBB3 and VEGFR2 proteins were detected by immunohistochemistry, and the expression levels of ERBB3, VEGFR2, Cyclin D1, cleaved-caspase 3, Bax, Bcl-2 and PI3K/AKT/mTOR pathway related proteins were detected by Western blot. The proliferation, migration, invasion, apoptosis and angiogenesis of HUVEC were detected by MTT, wound healing, transwell, flow cytometry and angiogenesis assay.  Results  MiR-205-5p was lowly expressed and ERBB3 was highly expressed in clinical samples of hemorrhoids (P < 0.001), and miR-205-5p targeted the 3'UTR of ERBB3 (WT) (P < 0.001). The expression of ERBB3 in hemorrhoidal rats and HUVEC cells in miR-205-5p mimic group was significantly decreased (P < 0.01, P < 0.001), and the levels of VEGFR2 (P < 0.001), Cyclin D1 (P < 0.01),Bcl-2 (P < 0.001), p-PI3K/PI3K, p-AKT/AKT, p-mTOR/mTOR (P < 0.001) were significantly down-regulated, the levels of Cleaved-caspase 3 and Bax were significantly up-regulated (P < 0.01), the perianal histopathology of rats was improved, the proliferation, migration and invasion activities of HUVEC cells were reduced (P < 0.001), the apoptosis rate was increased (P < 0.001), and the number and branches of angiogenesis were reduced (P < 0.001), and these effects were eventually reversed by pcDNA-ERBB3 (P < 0.001).   Conclusion  MiR-205-5p can reduce angiogenesis and relieve hemorrhoids by targeting the inhibition of ERBB3 expression, down-regulating the activity of PI3K/AKT/mTOR pathway, inhibiting cell proliferation, migration and invasion, and promoting apoptosis.
Effect of Sleeve Gastrectomy on Glucose Metabolism and Autophagy in Obese Mice

Abstract:
  Objective  To investigate the changes in liver autophagy factor activity in obese mice before and after sleeve gastrectomy(SG).   Methods  Male obese mice aged 6-8 weeks were randomly divided into a sham surgery group (Sham group, n=6) and a sleeve gastrectomy surgery group (SG group, n=6). The daily weight and food intake of the two groups of mice were recorded within one month after surgery, and the fasting blood glucose values at the tip of the mouse tail were measured at 15 and 30 days after surgery. Oral glucose tolerance tests were performed to evaluate glucose absorption; RT-qPCR was used to detect the mRNA expression changes of autophagy factors and glucose transporters at 15 and 30 days after surgery.   Results  Compared with preoperation, there was no statistically significant difference(P > 0.05) in the weight, daily food intake, and fasting blood glucose values of the sham surgery group at 15 and 30 days after surgery, while the SG group had a decrease in weight, daily food intake, and fasting blood glucose values at 15 and 30 days after surgery and had a statistically significant difference(P < 0.05); Compared with the sham surgery group, the weight, daily food intake, and fasting blood glucose of the SG group decreased and showed statistical differences(P < 0.05) at 15 and 30 days. The autophagy factor P62 in the SG group mice increased, while the mRNA expression of Beclin-1, LC3I/LC3II, and SGLT1 decreased and showed statistical differences(P < 0.05) at 15 and 30 days after surgery.   Conclusion  SG can improve obesity and reduce inflammatory mediators and adipokines, further regulating autophagic activity in mouse liver.
A Two-sample Mendelian Randomization Study of Oily Nut Intake and Primary Lung Cancer

Abstract:
  Objective  To investigate the potential causal relationship between oily nut consumption and primary lung cancer by two-sample Mendelian randomization method.   Methods  Single nucleotide polymorp hisms (P < 5×10-7) associated with oily nut intake were screened from the openGWAS public database of genome - wide association study as instrumental variables. The inverse variance weighting method was the main analysis method to evaluate the causal effect of this study, and MR-Egger regression and weighted median method were used to supplement the explanation. OR value and 95%CI were used to evaluate whether there was a causal relationship between oily nut intake and primary lung cancer. The mr_pleiotropy_test and MR-PRESSO were used to test the level pleiotropy, and the leave-one-out method was used to test the sensitivity.   Results  A total of 100 SNPs were extracted from the ukb-b-16576 dataset as instrumental variables. The MR-Egger regression, weighted median, and inverse variance weighting methods yielded OR values and 95% confidence intervals [0.63, 95%CI (0.11-3.65), P = 0.611], [0.34, 95%CI (0.19-0.61), P = 0.001], [0.32, 95%CI (0.21-0.48), P = 0.001], rerespectively. The mr_pleiotropy_test showed that the MR-Egger intercept was close to 0 (-7.66E-03) with a p-value of 0.441, indicating no significant horizontal pleiotropy. Similarly, the MR-PRESSO package GlobalTest yielded a p-value of 0.162, further confirming the absence of horizontal pleiotropy among the selected instrumental variables.After conducting the leave-one-out sensitivity analysis, no significant influence on the results was observed from any specific SNP locus, indicating that the results are highly stable and reliable.   Conclusions  The two-sample Mendelian randomization analysis revealed a negative causal relationship between oily nut intake and the risk of primary lung cancer. However, further verification is needed with larger sample sizes and data from individuals of different ethnic backgrounds to strengthen the findings.
Chemical Compositions of Brassica rapa L.

Abstract:
  Objective  To study the chemical constituents of Brassica rapa L. and find its active components.   Methods  The chemical constituents of Brassica rapa L. were extracted by methanol reflux, separated and purified by silica gel column chromatography, thin layer chromatography and Sephadex LH-20 column chromatography, and the structural spectrum was determined by spectral analysis (nuclear magnetic resonance hydrogen spectrum, carbon spectrum).   Results  Seventeen compounds were isolated and identified as: (1) 1-(4-hydroxyphenyl )-2-propanone; (2) 4-(2-butoxyethyl) phenol; (3) 1-methoxy-3-indoleacetonitrile; (4) 2, 2′-oxybis(1, 4)-di-tert-butylbenzene; (5) Glycerol monopalmitate; (6) 1-butyl phthalate-2-isobutyl phthalate; (7) Dibutyl phthalate; (8) 1-methoxy-3-indole formaldehyde; (9) 3, 7-Dimethyl-n-octan-3α-ol-1-yl-benzoate; (10) Di(2-ethylhexyl) phthalate; (11) β-sitosterol; (12) TgSSTg; (13) 3-Indolecarboxaldehyde; (14) Caffeic acid; (15) Quercetin; (16) auriculatum A; (17) Liquiritin.   Conclusion  1~5, 8~9, 12~13 were isolated from this plant for the first time. The discovery of the above compounds further enriched the chemical compositionof Brassica rapa L. and provided a preliminary experimental basis for the comprehensive development and utilization of Brassica rapa L. plant resources.
Pan-Cancer Analysis for the Effects of ANGPT1 and TEK on Human Tumor Progression

Abstract:
  Objective   To probe the expression level, prognostic value, and immune invasion of angipoietin-1 (ANGPT1)and its receptor tyrosine kinase (TEK)in pan-cancer.   Methods   TIMER2.0、Kaplan-Meier plotter and UALCAN etc. databases were used to analyze the expression, prognostic value of ANGPT1 and TEK in various types of tumors. SangerBox 3.0 and R software were used to analyze the relationship between ANGPT1 and TEK tumor mutational burden (TMB), microsatellite instability (MSI), immune checkpoint (ICP) and immune cell infiltration. String database, GO and KEGG analysis were used to identify proteins and regulatory pathways related to ANGPT1 and TEK.   Results   ANGPT1 and TEK showed low expression in most tumors (P < 0.05). Patients with low expression of ANGPT1 and TEK had a worse prognosis (P < 0.05). The expression levels of ANGPT1 and TEK were correlated with the clinical stages of clear cell renal carcinoma, bladder urothelial carcinoma, and invasive breast carcinoma (P < 0.05). The expression of ANGPT1 and TEK was significantly correlated with TMB, MSI and ICP in various tumors (P < 0.05), and was significantly positively correlated with immune invasion (P < 0.05). The genes were closely related to ANGPT1 and TEK include GRB2, PIK3R1, EGFR, etc., which were mainly involved in a series of biological processes and functions related to Grb2-EGFR, ERBB3:ERBB2, Shc-EGFR and other complexes, mainly involved ErbB signaling pathway.   Conclusion   ANGPT1 and TEK may play an important role in tumorigenesis and development through the ErbB signaling pathway, and are expected to be potential clinical prognostic markers for various types of cancer.
Preparation and in Vitro-in Vivo Evaluation of Aspirin Enteric-coated Tablets

Abstract:
  Objective   To prepare aspirin enteric-coated tablets and investigate its in vitro - in vivo release characteristics.   Methods   The powder was directly pressed into tablets, and a coating solution with a solid content of 20% was prepared with the aqueous dispersion of enteric-coated material L30D-55, talcum powder and triethyl citrate (1∶1∶0.1) for enteric coating, and the weight gain ranged from 15% to 20%, so that 0.1 g aspirin enteric-coated tablets were prepared, that is, the self-made preparation (T). Taking 0.1 g (Bayaspirin®) enteric-coated tablets (R) as reference preparation, the in vitro release and in vivo postprandial pharmacokinetics of the two preparations were investigated.   Results   The f2 factor of the in vitro release curves of T and R was greater than 50; Cmax, AUC0-t and AUC0-∞ of acetylsalicylic acid all meet the corresponding RSABE or ABE bioequivalence evaluation criteria.  Conclusion   There is a good consistency between the in vitro release of T and R and the postprandial pharmacokinetics in vivo.

A Meta-analysis of the Efficacy and Safety of Camrelizumab Combined with Chemotherapy in the Treatment of Non-small Cell Lung Cancer

Abstract:
  Objective  To discuss the difference in efficacy and safety of immune checkpoint inhibitor camrelizumab alone and combined chemotherapy in the treatment of non-small cell lung cancer. Aggregate analysis of multiple clinical trial data was conducted to obtain more accurate efficacy and safety evaluation, and bring effective information and medication guidance to clinical patients.   Methods  By searching PubMed, Embase, Cochrane Library, CBM database, CNKI, VIP Chinese database and Wanfang database, we collected the required randomized controlled trials or non-randomized controlled trials published until November 30, 2023. After data extraction, Revman5.4 software was applied for data analysis. According to the different treatment plan, the study subjects were divided into camrelizumab combined chemotherapy group and chemotherapy group alone for research discussion.   Results  A total of 19 articles were included, with a total of 1545 patients. Meta results showed: compared with chemotherapy alone, camrelizumab combined with chemotherapy in the treatment of non-small lung cancer was more effective in improving the overall response rate (OR = 2.50, 95%CI:2.02~3.08, P < 0.00001), increased the CD4+/CD8+ ratio (MD = 0.26, 95%CI:0.19~0.33, P < 0.00001) and effectively reduced the serum tumor marker CA125 (MD = -10.65, 95%CI:-14.34~-6.96, P < 0.00001) and CEA (MD = 5.56, 95%CI:-7.64~-3.47, P < 0.00001), CY211 (MD = -2.17, 95%CI:-3.22~1.13, P = 0.00001). Different adverse reactions showed different cluster analysis results. Among them, camrelizumab combined chemotherapy increased anemia (OR = 1.95, 95%CI:1.09~3.51, P = 0.03), hypothyroidism (OR = 5.61, 95%CI:1.82~17.25, P = 0.003) and capillary hyperplasia (OR = 2.86, 95%CI:1.19~6.88, P = 0.02) compared with chemotherapy alone, P values were all < 0.05, and the difference was statistically significant. However, rash (OR = 1.34, 95%CI:0.82~2.16, P = 0.24), myelosuppression (OR = 1.06, 95%CI:0.56~2.02, P = 0.86), leukopenia (OR = 0.78, 95%CI:0.40~1.52, P = 0.47), alopecia (OR = 1.29, 95%CI:0.57~2.92, P = 0.54), neurological abnormalities (OR = 1.17, 95%CI:0.39~3.56, P = 0.78), digestive tract abnormalities (OR = 1.10, 95%CI:0.79~1.53, P = 0.57), liver function abnormalities (OR = 1.54, 95%CI:0.92~2.59, P = 0.10), nausea and vomiting (OR = 1.16, 95%CI:0.58~2.31, P = 0.67), coughing up blood (OR = 0.87, 95%CI:0.30~2.49, P = 0.79), fever(OR = 1.22, 95%CI:0.65~2.30, P = 0.54) and others (OR = 1.32, 95%CI:0.76~2.31, P = 0.32) had no significant change in the incidence of symptoms, and all P values were greater than 0.05, the difference was not statistically significant. Taking into account the incidence of all kinds of adverse reactions mentioned above, in general, camrelizumab combined chemotherapy did not increase the incidence of adverse reactions compared with chemotherapy alone (OR = 1.32, 95%CI:1.13~1.55, P = 0.23), with a P value greater than 0.05, and the difference was not statistically significant.   Conclusion  Compared with chemotherapy alone, camrelizumab combined chemotherapy can more effectively improve the overall response rate and CD4+/CD8+ ratio in patients with non-small cell lung cancer, reduce the absolute value of serum tumor markers, and does not increase the incidence of adverse reactions, with good safety.
Association between Lifestyle Behaviors and Depressive Symptoms of middle school students in rural area of Miao nationality in Pingbian, Yunnan Province

Abstract:
  Objective  To explore the association between lifestyle behaviors and depressive symptoms of middle school students in rural area of Miao nationality in Pingbian, Yunnan Province, and to provide a data reference for changing unhealthy lifestyle behaviors for improving depressive symptoms in adolescents.   Methods  A total of 819 middle school students from 2 middle schools were randomly selected. A face-to-face questionnaire survey was conducted to collect data on the lifestyle and behavior of middle school students. Depressive symptoms were assessed using the Depression Anxiety Stress Scale (DASS-21), and the association between lifestyle and depressive symptoms was analyzed using a generalized linear model.   Results  The detection rate of depressive symptoms in middle school students was 33.21%, and the detection rate of depression symptoms in girls was higher than that in boys (39.20% vs 26.72%), the difference was statistically significant (P < 0.05). Learn daily screen time (β= 0.283, 95%CI = 0.114 - 0.452), outdoor time during the day (β= -0.292, 95%CI = -0.512 - -0.071), breakfast behavior (β= -0.263, 95%CI = -0.455 - -0.071), fresh food consumption (β= -0.291, 95%CI = -0.529 - -0.053), snack consumption (β= 0.294, 95%CI = 0.019 - 0.569) were associated with depressive symptoms in middle school students (all P < 0.05). Learn daily screen time (β= 0.297, 95%CI = 0.074 - 0.520), Learn daily screen time (β= -0.369, 95%CI = -0.673 - -0.065), breakfast behavior (β= -0.288, 95%CI = -0.567 - -0.010), fresh food consumption (β= -0.423, 95%CI = -0.735 - -0.110) were associated with depressive symptoms in boys (all P < 0.05). Learn daily screen time (β= 0.306, 95%CI = 0.056 - 0.557), smoking (β= 0.679, 95%CI = 0.044 - 1.314), breakfast behavior (β= -0.304, 95%CI = -0.576 - -0.031), western fast food consumption (β= -1.278, 95%CI = -2.425 - -0.131) were associated with depressive symptoms in girls (all P < 0.05).   Conclusion  The study suggests that the prevalence of depressive symptoms is high among middle school students in rural area of Miao nationality in Pingbian, and it is related to unhealthy lifestyle behaviors. Changing unhealthy lifestyle behaviors may be an effective way to improve middle school students in the area.
Study on Influencing Factors of Cognitive Impairment in Late-life Depression Patients

Abstract:
  Objective  To explore the influencing factors of cognitive impairment in elderly patients with depressive disorder.   Methods  119 elderly patients with depression were recruited in the 1st Affiliated Hospital of Kunming Medical University. MoCA was used to evaluate the cognitive function of the patients, and a regression model was established to analyze the influencing factors of cognitive impairment in elderly patients with depression.   Results  Age had a significant effect on the total score of cognition, visuospatial ability, executive function and language function (P < 0.05). The degree of anxiety had a significant effect on the total score of cognition, visuospatial ability, executive function, attention and working memory (P < 0.05). Dyslipidemia had a statistically significant effect on the total score of cognition, visuospatial ability and executive function (P < 0.05). The past medical history had a significant effect on the total score of cognition and language function (P < 0.05). The sex, onset time, degree of depression, ApoE gene, occupation status and psychotic symptoms had statistically significant effects on individual cognitive function (P < 0.05).   Conclusion  Patients with late-life depressive disorder have obvious cognitive impairment. Age, anxiety, dyslipidemia, and past medical history may be the risk factors for cognitive impairment in elderly patients with depressive disorder.

  Objective   To investigate the hemodynamic changes after the occurrence of Stanford type A aortic dissection.   Methods   Computed tomography angiography(CTA) images of the aorta were collected from 4 Stanford A type aortic dissection patients and 2 healthy volunteers, and the three-dimensional models of the aorta were established and meshed. The above models were numerically simulated by fluid simulation software.   Results   The blood flow in the aorta of healthy volunteers was approximately laminar. The blood flow field in the true lumen was approximately laminar. Turbulence not only occurred near the intimal tear of the true lumen, but also near the area where the false lumen compressed the true lumen. The wall pressure of the true lumen was higher than that of the false lumen. During the systolic phase, the peak wall pressure of the false lumen was located at the primary entry tear of the dissection, the high wall shear stress appeared around the true lumen compressed by false lumen and the tear of false lumen. Moreover, stress concentration occurred at the intimal tear. Compared to aortic dissection with a single tear, aortic dissection with two tears exhibited lower wall shear stress around the intimal tear.   Conclusion   Individualized computational fluid dynamics analysis can provide reference for personalized diagnosis and treatment of aortic dissection.

Abstract:
  Objective  To investigate the characteristics of parathyroid hormone (PTH) and estrogen receptor (ER) gene polymorphisms in patients with type 2 diabetes mellitus (T2DM) complicated with osteoporosis (OP).   Methods  110 patients with T2DM from Kunming area were selected in the 1st People’ s Hospital of Kunming Endocrinology Department from November 2022 to October 2023, and were divided into T2DM without OP group(n = 68, T≥-1.0), T2DM with OP group(n = 42, T≤-2.5), according to the results of bone mineral density , the genotype and allele frequency of PTH and ER were detected, and the differences between them and gender, height, weight and other clinical indicators were compared.   Results  Regression analysis showed that T2DM with OP was related to gende and weight (P < 0.05), but not related to PTH gene, or ER gene polymorphism (P > 0.05).   Conclusion  Gender and weight are independent risk factors for osteoporosis in patients with type 2 diabetes mellitus; gene polymorphism of PTH and ER are not related to the genetic susceptibility of T2DM with OP in Kunming area.

Abstract:
  Objective  To explore the clinical efficacy of acetylcysteine solution retained perfusion inhalation treatment under electronic bronchoscopy in children with severe bacterial pneumonia.   Methods  From January 1, 2021 to December 31, 2022, 86 children with severe bacterial pneumonia treated in the Department of Respiratory Medicine of Kunming Children’ s Hospital were randomly divided into a control group (n = 43) and a study group (n = 43). The children in the control group received electronic bronchoscopy under physiological saline pulmonary alveolar lavage treatment. The study group received acetylcysteine solution treatment retained after the end of the pulmonary lavage treatment. The clinical treatment effects, inflammatory indicators, tidal breathing function, and adverse reactions of the two groups were compared.   Results  After treatment, compared with control group, the hospitalization time, cough disappearance time and comorbidities disappearance time of children in study group were significantly shortened (P < 0.05), and the total effective rate of study group was significantly higher than that of control group (P < 0.05). The levels of white blood cell count, procalcitonin, C-reactive protein, interleukin-6, and tumor necrosis factor-α of study group were significantly lower than those of control group (P < 0.05). The tidal volume per kilogram of body weight, peak time ratio, and peak volume ratio levels of study group were significantly higher than those of control group (P < 0.05).There was no statistically significant difference in the adverse drug reactions between two groups (P > 0.05).   Conclusion  Under electronic bronchoscopy, preserving perfusion and inhaling acetylcysteine solution can significantly improve the clinical symptoms, inflammatory indicators, and lung function of children with severe bacterial pneumonia, and it is relatively safe.

Abstract:
  Objective   To investigate the value of vectorcardiography in the diagnosis of abnormal cardiac electrical activity in hypertension, and to analyze the influencing factors of early cardiac target organ damage.   Methods   80 cases in the Department of Cardiovascular of Kunming Municipal Hospital of Traditional Chinese Medicine from January 2022 to February 2023, were selected.Cardiac structural abnormalities were excluded by ultrasound cardiogram. The detection of abnormal ventricular depolarization and repolarization indexes was compared between electrocardiogram and vectorcardiogram. 80 cases were divided into normal group (n = 40) and abnormal group (n = 40) according to the results of vectorcardiogram, the related influencing factors of early cardiac damage were compared between the two groups, and the indicators with statistical significance were included in the binary Logistic regression analysis to screen out the independent influencing factors of early heart damage.   Results   Vectorcardiogram was superior to ECG in detecting abnormal ventricular repolarization indexes (P < 0.05), and there was no significant difference in detecting abnormal depolarization indexes between the two groups (P > 0.05). There were significant differences between the normal group and the abnormal group in age, regular medication, family history, diabetes, 24 h average systolic blood pressure (24 h SBP), daytime average systolic blood pressure (DSBP), night average systolic blood pressure (NSBP), blood pressure load, morning blood pressure, and pulse pressure (P < 0.05). Binary Logistic regression analysis showed, that age [OR (95%CI) = 0.891, 0.998] and NSBP [OR (95%CI) = 1.018, 2.10], family history [OR (95% CI) = 0.029, 0.499], diabetes [OR = 0.042 (95%CI), 0.916] were the independent influencing factors of early cardiac damage in hypertension.   Conclusion   Vectorcardiogram is an effective method to detect early cardiac target organ damage in hypertension.

Abstract:
  Objective  To investigate the application value of transvaginal three-dimensional ultrasound in the determination of endometrial receptivity.   Methods  A retrospective study was conducted on 106 patients who were scheduled to undergo assisted reproductive treatment at Kunming Maternal and Child Health Hospital from April 2022 to June 2023. All patients underwent transvaginal three-dimensional ultrasound examination before pregnancy. They were divided into a pregnant group (n = 48) and a non-pregnant group (n = 58) based on whether they achieved clinical pregnancy. The transvaginal three-dimensional volume probe was used to record the endometrial type, endometrial blood flow classification, endometrial volume, pregnancy rate, and endometrial and subendometrial blood flow parameters [Flow Index (FI), Vascularization Index (VI), and Vascular Flow Index (VFI)].  Results  When comparing the endometrial type, endometrial blood flow classification, and endometrial volume between the two groups, statistically significant differences were observed (P < 0.05). The pregnancy rate of endometrial morphology type A was higher than that of type C, and the pregnancy rate of endometrial blood flow classification III was higher than that of type I, with statistically significant differences (P < 0.05). There were statistically significant differences in endometrial and subendometrial FI, VI and VFI between the two groups (P < 0.05).  Conclusion   Transvaginal three-dimensional ultrasound technology not only enables effective evaluation of endometrial receptivity, accurate prediction of pregnancy outcomes, but also provides an important basis for clinical treatment plans.

Abstract:
  Objective  To investigate the nutritional status and its influencing factors among the elderly in some communities of Kunming, in order to provide more targeted suggestions for preventing malnutrition among the elderly in the above communities, thereby improving the quality of life and health status of the elderly in the community.  Methods   It is convenient to select 2506 elderly people from 3 communities in Kunming from August 1, 2022 to August 1, 2023, and use a general information questionnaire, the mini nutritional assessment short-form Form (MNA-SF), and the geriatric depression scale-15 Form (GDS-15) for comprehensive assessment, to investigate the factors influencing the nutritional status of the elderly.  Results  The nutritional status score of 2506 elderly individuals in the community was 11.0 (10.0, 12.0) points, with 71 cases (2.8%) of malnutrition, 1459 cases (58.2%) at risk of malnutrition, and 976 cases (38.9%) with normal nutrition.Univariate analysis showed that marital status, living conditions, suffering from hypertension, coronary heart disease, chronic obstructive pulmonary disease, or stroke, and depression status had statistically significant differences in their impact on nutritional status (P < 0.05).Multivariate ordinal logistic regression analysis showed that coronary heart disease, chronic obstructive pulmonary disease, stroke, and depression were independent risk factors for the nutritional status of the elderly, while a higher body mass index (BMI) was a protective factor.  Conclusion  The risk of malnutrition of the elderly in this survey is high and influenced by many factors. It is suggested to comprehensively evaluate the nutritional status of the elderly with various indicators, so as to identify the malnutrition factors early and intervene in time to ensure healthy aging.

Abstract:
  Objective  To investigate the therapeutic efficacy and safety of the combination of monosialotetrahexosylganglioside (GM1) injection and Batroxobin in treating sudden deafness.   Methods  The clinical data of 162 patients with sudden deafness in Affiliated Hospital of Tangshan Vocational and Technical College between January 2018 and December 2022 were selected for retrospective analysis and grouped according to the treatment protocol, 81 cases in the control group were treated with batroxobin and 81 cases in the observation group were treated with GM1 injection combined with batroxobin. The efficacy and symptom recovery time of the 2 groups were compared, as well as the levels of pure tone hearing threshold, inflammatory indexes [interleukin-10 (IL-10), high sensitive C-reactive protein (hs-CRP), and tumor necrosis factor-α (TNF-α)] before and after treatment, and safety.   Results  The total effective rate of the observation group was 92.59%, which was higher than that of the control group (59.26%) (P < 0.05); the observation group had shorter time for dizziness to disappear, hearing to return to normal, and tinnitus to disappear than the control group (P < 0.05); pure tone hearing thresholds in the observation group were lower than those in the control group after 7 d and 14 d of treatment (P > 0.05); after treatment, the serum IL-10 level in the observation group was higher than that in the control group, while the levels of hs-CRP and TNF-α were lower than those in the control group(P < 0.05); the incidence of adverse reactions in the observation group was 11.11%, compared with 9.88% in the control group, the difference was not statistically significant (P > 0.05).   Conclusion  GM1 injection combined with batroxobin can effectively control the inflammatory response in patients with sudden deafness, improve hearing status, accelerate the recovery process, and has a certain degree of safety and efficacy.

Abstract:
  Objective  To establish the derived indicators from blood cell analysis of the elderly in Anning region of Yunnan province, lymphocyte to monocyte ratio (LMR), neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR) reference interval, Observed differences from multicenter surveys, To determine the necessity to establish this reference interval.   Methods  The analysis results of people above 60 years old for physical examinations in the information system of Anning First People’ s Hospital from June 2022 to September 2022 were collected retrospectively. The biological reference interval for healthy elderly people in Anning City was established according to the indirect method in the international CLSI EP28-A3c guidelines.   Results  A total of 8877 cases were included. Red blood cell count (RBC), hemoglobin (HGB), hematocrit (HCT), mean red blood cell hemoglobin concentration (MCHC), platelet count (PLT), and lymphocyte percentage (LYM%) decreased with age, and the difference between men and women was statistically significant (P < 0.05). Only the broadband coefficient of variation of red blood cell volume distribution (RDWCV) increased with age. White blood cell count (WBC), absolute value of neutrophils (NEU#), absolute value of eosinophils (EOS#), percentage of neutrophils (NEU%), percentage of eosinophils (EOS%), mean red cell volume (MCV), standard deviation of red blood cell volume distribution width (RDWSD), platelet volume distribution width (PDW) were statistically significant only between men and women (P < 0.05), while the remaining items were not significant by age and sex (P > 0.05). NLR increased with age, and men were significantly higher than women of the same age. Both LMR and PLR were higher in females than in males.   Conclusion  The results of WBC, RBC, HGB, PLT and HCT in the self-built reference interval were quite different from the reference interval of the multi-center survey.
Effectiveness and Safety Analysis of Cyclopol Combined with Remifentanil in Anesthesia for Endoscopic Gastric Cancer Radical Surgery

Abstract:
  Objective   To investigate the effectiveness and safety of cyclophenol combined with remifentanil in anesthesia for laparoscopic radical gastrectomy for gastric cancer.   Methods  A total of 100 patients who underwent laparoscopic radical gastrectomy for gastric cancer admitted to 900 Hospital of the Joint Logistic Support Force from August 2022 to July 2023 were randomly divided into two groups: the propofol combined with remifentanil group (50 cases) received propofol combined with remifentanil anesthesia, and the cyclopol combined with remifentanil group (50 cases) received cyclophenol combined with remifentanil anesthesia. The vital signs (respiratory rate, heart rate, and blood oxygen saturation) of the two groups of patients were observed. Anesthesia indicators (onset time of anesthesia, recovery time of spontaneous breathing, awakening time, time of leaving the room), anesthesia effect, and adverse reactions (Injection pain, hypotension, hypoxemia, respiratory depression, dizziness, drowsiness) were also observed.   Results  The interaction effect of respiratory rate between two groups, time points, and between groups/time points showed no statistically significant difference (P > 0.05); The interaction between heart rate and blood oxygen saturation between two groups, time points, and between groups and time points showed statistically significant differences (P < 0.05). There was no statistically significant difference (P > 0.05) in the respiratory rate between the two groups of T0 to T5. The heart rate of T1 and T2 in the combination of propofol and remifentanil group was higher than that in the combination of propofol and remifentanil group, and the difference was statistically significant (P < 0.05). There was no statistically significant difference in heart rate between the two groups at other times (P > 0.05). The blood oxygen saturation of T2 in the combination of propofol and remifentanil group was higher than that in the combination of propofol and remifentanil group, and the difference was statistically significant (P < 0.05). There was no statistically significant difference in blood oxygen saturation between the two groups at other times (P > 0.05). There was no statistically significant difference in the onset time of anesthesia between the two groups (P > 0.05). The spontaneous breathing recovery time, awakening time, and departure time of the group treated with propofol combined with remifentanil were shorter than those of the group treated with propofol combined with remifentanil, and the differences were statistically significant (P < 0.05). The total effective rate of the combination of propofol and remifentanil group was 96.00%, which was higher than the 84.00% of the combination of propofol and remifentanil group, and the difference was statistically significant (P < 0.05). The incidence of adverse reactions in the combination of propofol and remifentanil group was 10.00%, which was lower than the 28.00% in the combination of propofol and remifentanil group, and the difference was statistically significant (P < 0.05).   Conclusion  The application of cyclophorol combined with remifentanil in anesthesia for laparoscopic radical gastrectomy for gastric cancer has a good effect, can stabilize the patient's intraoperative vital signs, improve anesthesia indicators, reduce adverse reactions, and can be promoted.

Abstract:
Circular RNA (circRNA), formed by reverse splicing of pre-mRNA, constitutes a closed-loop RNA structure with diversity in molecular characteristics, biological functions, and regulatory mechanisms. In recent years, there has been growing recognition of the roles of circRNA in various human diseases, serving potentially as molecular biomarkers for disease diagnosis and prognosis prediction, including the classical autoimmune disease systemic lupus erythematosus (SLE). Currently, dysregulated expression of some circRNAs has been observed in SLE, which can promote the occurrence and development of SLE through different mechanisms such as "molecular sponge" effects, modulation of signaling pathways, inhibition of protein kinase activation, and induction of cell apoptosis. Another part may serve as potential clinical biomarkers for SLE, holding translational potential. This paper reviews the research on circRNA in SLE over the past years, aiming to provide scientific evidence and references for subsequent studies on circRNA in SLE.

Abstract:
The current situation of anesthesia nursing unit construction is described, along with the theoretical basis and methodology for the construction of its nursing-sensitive quality indicators. A review is presented on the construction and application of nursing-sensitive quality indicators in anesthesia nursing units both domestically and internationally. The deficiencies of the current research are summarized and analyzed, and directions for future development are proposed, aiming to provide references for the establishment of uniform, standardized, and scientific nursing-sensitive quality indicators in anesthesia nursing units in the future.

Abstract:
  Objective  To compare the differences in the competency of the students in the standardized training of residents with different learning styles, and explore more effective clinical teaching strategies.   Methods  Kolb learning style and clinician post competency of resident trainees in the First Affiliated Hospital of Kunming Medical University were investigated with a questionnaire, and the differences of resident trainees with different learning styles in clinician post competency were compared.   Results  The main learning style was aggregation (46.1%), followed by adjustment (23.2%) and divergence (17.1%), and absorption (13.7%). There were statistically significant differences in post competence, basic clinical ability, doctor-patient communication ability, mastering and applying medical knowledge, teamwork ability and academic research ability of clinicians with different learning styles (P < 0.05). Among them, absorptive learning style of clinical medicine student clinician post competency was the highest.   Conclusion  Through the understanding of learning style, the teaching teachers can carry out targeted teaching design to improve the post competence of clinical doctors in Pearson Residence.

The Mediating Effect of Hospital Unit Safety Climate between the Nursing Work Environment and Job Satisfaction

Abstract:
  Objective  To explore the relationship and influence path between nurses’ perceived work environment, hospital safety climate and nurses’ satisfaction.   Methods  Using convenience sampling method, 444 in-service nurses in a third-class A hospital in Chengdu City were selected as research subjects from September to November 2021, and the nurses were surveyed with the Nurses’ Perceived Work Environment Questionnaire. Hospital Safety Climate Scale, and Job Satisfaction Scale, and the correlation between hospital safety climate, Nurses’ perceived work environment and satisfaction were analyzed by Pearson correlation, and a path relationship model was established based on the correlation test, and a mediation effect test was conducted by Bootstrap method. Based on the results of the correlation test, a path relationship model was established, and the mediation effect test was conducted by Bootstrap method to deeply analyze the mediation effect of hospital safety atmosphere between Nurses’ perceived work environment and satisfaction.   Results  Nurses’ perceived work environment was positively correlated with satisfaction (r = 0.428, P < 0.01), hospital safety climate was positively correlated with Nurses’ work environment (r = 0.693, P < 0.01), hospital safety climate was positively correlated with Nurses’ satisfaction (r = 0.466, P < 0.01), Nurses’ perceived work environment positively predicted hospital safety climate (β = 0.773, P = 0.003), while the positive predictive effect on satisfaction was not statistically significant (β = 0.060, P = 0.530). The value of the indirect effect of hospital safety climate between Nurses’ perceived work environment and satisfaction was 0.414 (95%CI 0.557-1.531), which accounted for 87.34% of the total effect.   Conclusion  Hospital safety climate acts as a complete mediator between Nurses’ perceived work environment and satisfaction, promoting a healthy and stable nursing workforce.

  Objective  To investigate the current status of psychological resilience of clinical nurses in a cancer hospital, identify the potential profile categories of psychological resilience of nurses, and explore its subgroup characteristics and influencing factors, so as to provide a basis for the construction of targeted and scientific psychological resilience intervention strategies.  Methods  Convenience sampling method was used to select 812 clinical nurses from oncology hospitals in Southwest China, using a general information questionnaire, mental toughness scale, potential profile analysis using Mplus 8.3 to categorise the characteristics of clinical nurses' mental toughness in oncology hospitals, and χ2 test and unordered multiclassified logistic regression using SPSS 26.0 to analyse the effects of each factor on different categories.  Results  Three potential profiles of mental toughness of clinical nurses in oncology hospitals were identified, which were named as mental toughness “low toughness group”, “general toughness group” and “high toughness group”, accounting for 16.2%, 54.6% and 19.2%, respectively.The results of unordered multiclassified logistic regression analysis showed that ethnicity, marital status, department, title, years of experience, annual income, and personality traits were the influencing factors (P < 0.01).  Conclusions  Clinical nurses' mental toughness in oncology hospitals is at an intermediate level, and there are obvious heterogeneous characteristics affected by many factors. Nursing managers should identify their categories and influencing factors when carrying out relevant training, make adjustments to the training programme, and make targeted interventions to strengthen the construction of nurses' mental toughness at all levels, to safeguard nurses' physical and mental health, and to improve their work efficiency.

The differences of postoperative recovery, sputum output and complications between the two groups were compared, and the changes of cardiopulmonary function, dyspnea and motor function before and 3 months after operation were recorded.   Results  The time of chest tube removal, thoracic drainage and postoperative hospital stay in the observation group were shorter than those in the control group (P < 0.05). The sputum output of the observation group was lower than that of the control group on the 1st, 2nd and 3rd day after operation (P < 0.05). The incidence of postoperative complications in the observation group was lower than that in the control group (P < 0.05). 3 months after operation, LVEF (left ventricular ejection fraction) increased and LVEDD (left ventricular end diastolic diameter) decreased in both groups, and FEV1(forced expiratory volume for 1 second), FVC (forced vital capacity) and PEF (peak expiratory flow) decreased compared with that in the first day before operation. Three months after operation, LVEF was higher, LVEDD was lower and FEV1, FVC and PEF were higher in the observation group than in the control group (P < 0.05). 3 months after operation, the mMRC (multiple myeloma research consortium) scores of both groups were higher than those of preoperative one day, and the 6MWT (6-min walking distance) was lower. mMRC score was lower and 6MWT was higher in the observation group than in the control group 3 months after operation (P < 0.05).   Conclusion  The training of active circulatory breathing technology based on 5A mode can promote the early recovery and postoperative sputum production in lung cancer patients undergoing thoracoscopic surgery. It also helps to prevent postoperative complications, improve postoperative motor function, and effectively reduce postoperative breathing difficulties.
